Hardware complications related to the surgical fixation of slipped capital femoral epiphyses.

Slipped upper femoral epiphysis is a disabling condition with an annual incidence of 2-13 per 100,000. In situ surgical fixation is the preferred initial treatment for both stable and unstable slipped capital femoral epiphysis (SCFE) cases. The main aim is to avoid further slippage and complications such as osteonecrosis and chondrolysis. The choice of medical implants in managing this condition has changed along the years from large nail-like devices to cannulated screws. The biomechanical properties of different fixation techniques have been studied. All implants have been associated with co

Narrow window of bone age in children with slipped capital femoral epiphyses.

Pelvis radiographs of 30 children with slipped capital femoral epiphyses (SCFE) were reviewed by four readers to determine the skeletal age. The average chronologic age for girls was 12.1 +/- 1.0 years and that for boys was 14.4 +/- 1.3 years; the average pelvic bone age was 13.2 +/- 0.6 for girls and 15.1 +/- 0.6 years for boys. The chronologic age range was 98 months, and skeletal age range was only 50 months. Pelvis bone age was advanced in the youngest children, normal in most children, and mildly delayed in older children. We conclude that there is a uniform skeletal age or "narrow window

Slipped capital femoral epiphyses complicating renal osteodystrophy: a report of three cases.

Three adolescents with bilateral slipped capital femoral epiphyses complicating renal disease are presented. In one case, the severity of the deformities necessitated total hip replacement. Pathological specimens were available for evaluation. In all 3 cases, epiphysiolysis was accompanied by severe subperiosteal reabsorption along the medial aspect of the femoral neck, widening of the cartilaginous growth plate, and coxa vara. The radiographic diagnosis of a minimally displaced femoral epiphysis may precede the clinical symptoms. Early recognition of this complication is important, since the

Bilateral epiphyseal migration following fixation for slipped capital femoral epiphyses in a hypothyroid child.

Progression of slipped capital femoral epiphysis following in situ screw fixation typically occurs through loosening of the screw in the metaphysis. Epiphyseal migration off the screw due to physeal growth is rare. We report epiphyseal migration off bilateral screws in a child undergoing thyroid replacement therapy. Patients with mild and moderate slipped capital femoral epiphysis and endocrine disease should be followed-up with radiographs taken at intervals which reflect the rate of growth. Fixation should be revised if the tip of the screw approaches the physis and initial fixation with two

Tibial torsion in chronic, stable slipped capital femoral epiphyses: evaluation by CT scan.

Externally rotated gait is a hallmark of slipped capital femoral epiphysis (SCFE) attributed to the change of proximal femur structure. This study investigated the potential role of abnormal tibial torsion in 44 patients with chronic, stable SCFE. Comparison of the tibiae of the involved side with the uninvolved side and with previously published normal values showed no evidence of tibial torsional abnormalities in these patients.

Predicting Risk of Contralateral Slip in Unilateral Slipped Capital Femoral Epiphysis: Posterior Epiphyseal Tilt Increases and Superior Epiphyseal Extension Reduces Risk.

BACKGROUND: Femoral morphology may influence the etiology of slipped capital femoral epiphysis (SCFE). We investigated whether radiographic parameters of femoral head-neck morphology are associated with a subsequent contralateral slip in patients presenting with unilateral SCFE. METHODS: We evaluated 318 patients treated for unilateral SCFE between 2000 and 2017. There were 145 males (46%), and the mean age in the series was 12.4 ± 1.7 years. The patients were followed for a minimum of 18 months or until the development of a contralateral slip (70 patients, 22%). We measured the epiphyseal ti

Modified Dunn procedure versus percutaneous pinning in moderate/severe stable slipped capital femoral epiphyses.

BACKGROUND: The modified Dunn procedure (MDP) has risen enthusiasm in treating slipped capital femoral epiphyses (SCFE) due to the anatomic reduction and high patients' satisfaction rates at long-term follow-up. Main aim of this study is to compare clinical and radiographic outcomes of 2 cohorts with moderate to severe stable SCFE treated by MDP and in situ fixation. METHODS: Medical records were analysed to collect demographic data, comorbidities and time from slip to surgery. The collected postoperative data were: avascular necrosis (AVN); complications; progression of osteoarthritis and su

The Atypical Patient With Slipped Capital Femoral Epiphyses May Be at Increased Risk for a Missed Contralateral Slip.

Slipped capital femoral epiphysis (SCFE) is a commonly encountered hip disorder. The goal of this study was to describe the incidence of missed contra-lateral SCFE as well as to identify risk factors. The authors hypothesized that contralateral slips are more often missed in patients with severe involvement of the treated side. After institutional review board approval was obtained, a retrospective chart review was performed of all pediatric patients who were treated for sequential and bilateral SCFE at a single institution during an 18-year period. Medical records were reviewed for demographi

Severity of Asynchronous Slipped Capital Femoral Epiphyses in Skeletally Immature Versus More Skeletally Mature Patients.

BACKGROUND: Routine prophylactic screw fixation for skeletally immature patients with slipped capital femoral epiphysis (SCFE) continues to be debated. The purpose of this study was to assess the slip severity of a second SCFE in skeletally immature versus more mature patients and determine necessity of contralateral hip prophylactic screw fixation. METHODS: All patients treated for SCFE at 3 pediatric hospitals over a 10-year time period (January 1, 2002 to December 31, 2011) were evaluated. Patients were included if they had a unilateral SCFE and a contralateral asynchronous SCFE, and were

Intraoperative Monitoring of Epiphyseal Perfusion in Slipped Capital Femoral Epiphysis.

BACKGROUND: The purposes of this study were to validate an innovative, percutaneous method of monitoring femoral head (epiphyseal) perfusion intraoperatively in patients with slipped capital femoral epiphysis (SCFE) and to investigate an association between intraoperative perfusion and the subsequent development of osteonecrosis. METHODS: A percutaneous screw fixation technique for SCFE was utilized. A fully threaded, cannulated, stainless-steel 7.0-mm screw was inserted into the epiphysis. The guidewire was removed, and a sterile intracranial pressure (ICP) probe was placed through the screw

Epiphyseal growth after pinning of slipped capital femoral epiphysis.

Seventy-seven hips in 61 patients (mean age, 13 years) with slipped capital femoral epiphysis treated with in situ pinning were retrospectively evaluated to determine the frequency and magnitude of epiphyseal growth after pinning. Threaded Steinmann pins, Knowles pins, and cannulated screws were used. Evidence of the epiphysis "growing off" the pins was seen in 29 and 18% of the hips treated with Steinmann and Knowles pins, respectively. No hips with cannulated screws continued to grow after surgery. The use of one cannulated screw in the treatment of mild and moderate slipped capital femoral

Slipped capital femoral epiphysis: assessment of epiphyseal displacement and angulation.

Nineteen hips with slipped capital femoral epiphysis underwent assessment of the severity of epiphyseal displacement on anteroposterior and lateral roentgenograms and by computerized tomography. The head-shaft angle, the percentage of maximal epiphyseal displacement, and the computed tomographic head-neck angle were determined. The head-shaft angle showed great variability when compared with the percentage of epiphyseal displacement and with the head-neck angle. The percentage of epiphyseal displacement showed less variability when compared with the computerized tomographic head-neck angle; ho

Manganese levels and the morphology of the epiphyseal plate in broilers with slipped tendons.

Manganese deficiency in chickens results in perosis and a higher incidence of slipped tendon. Perosis is associated with a disorganization of the epiphyseal growth plates and changes in the chemical composition of the cartilage matrix. Male broilers with slipped tendons were selected from a commercial broiler farm over a 9 week growing period. Their growth rates, epiphyseal cartilage histology and tissue manganese concentrations were examined and compared with (a) normal broilers from the same farm, and (b) similar broilers raised on control and manganese deficient diets. Field broilers with s

[Fractures and slipped epiphyses of the proximal humerus in children. Place and methods of surgical treatment].

Fractures and Epiphyseal separations of the proximal humerus are usually non-operatively treated with a generally good result. However, sometimes, a surgical treatment may be required. Twenty fractures of the proximal humerus occurring in children were treated operatively. From these 20 cases, the authors precise the indications and the ways of surgical treatment in this type of lesion. They insist upon the value of the angulation on the lateral radiograph of the shoulder which, if important, may often signify an interposition of the tendon of the long head of the biceps between the fracture f
