Mycothiol biosynthesis is essential for ethionamide susceptibility in Mycobacterium tuberculosis.

Spontaneous mutants of Mycobacterium tuberculosis that were resistant to the anti-tuberculosis drugs ethionamide and isoniazid were isolated and found to map to mshA, a gene encoding the first enzyme involved in the biosynthesis of mycothiol, a major low-molecular-weight thiol in M. tuberculosis. Seven independent missense or frameshift mutations within mshA were identified and characterized. Precise null deletion mutations of the mshA gene were generated by specialized transduction in three different strains of M. tuberculosis. The mshA deletion mutants were defective in mycothiol biosynthesi…

A clinical trial of ethionamide and prothionamide for treatment of lepromatous leprosy.

In 1982-1984 we conducted a six-month clinical trial in 50 previously untreated lepromatous leprosy patients randomly assigned to directly observed monotherapy with one of two thioamides, ethionamide or prothionamide, each given six times a week at doses of either 250 mg or 500 mg. The findings of this study have only recently been analyzed, and the potential for the use of these thioamides in leprosy patients placed in perspective. However, because of the small number of patients included in this study, the results must be interpreted with some caution. Clinical improvement was noted in 74% o…

[Case of SIADH caused by ethionamide in a patient with pulmonary tuberculosis].

Cases of syndrome of inappropriate secretion of antidiuretic hormone (SIADH) associated with tuberculosis have been reported, however, in most of these cases, tuberculosis disease is miliary or severe. Here we report the first case of SIADH induced by ethionamide (TH). The case is a 76 year-old woman. She noticed cough in April 2004 and chest X-ray showed infiltrative shadows on the right upper lung field. Sputum examination revealed positive for TB-PCR, and she was referred to our hospital. Treatment was started with the combination of isoniazid (INH), rifampicin and ethambutol, however susce…

Hepatotoxicity of the combination of rifampin-ethionamide in the treatment of multibacillary leprosy.

During treatment of multibacillary leprosy with the combination rifampin (RMP) 600 mg, ethionamide (ETH) 500 mg, and either dapsone (DDS) or clofazimine (CLO) 100 mg, hepatotoxicity was observed in 4.5% of 596 patients. Hepatitis appeared after 5-186 days, with a mean of 93 days and a median of 76 days. Mortality was 26%. ETH and DDS or CLO were administered daily in all regimens in which hepatitis occurred. RMP was given either daily or daily during the first two weeks or eight weeks, followed by a once-weekly dose. It is concluded that the combination RMP + ETH is the toxic component. In som…

Ethionamide versus ethambutol-containing first-line regimens for TB meningitis.

Ethionamide has emerged as an alternative to ethambutol for the treatment of tuberculous meningitis (TB meningitis). Here, we show that ethionamide- and ethambutol-containing first-line TB regimens achieved similar bactericidal activities in brain tissues and reduction in neuroinflammation in a mouse model of TB meningitis. However, ethionamide, but not ethambutol, achieves brain concentrations above the minimum inhibitory concentration for Mycobacterium tuberculosis.

Ethionamide cross- and co-resistance in children with isoniazid-resistant tuberculosis.

BACKGROUND: Ethionamide (ETH) is a structural analogue of isoniazid (INH). Both are pro-drugs requiring activation by separate and common enzyme pathways, which could lead to co- and/or cross-resistance. OBJECTIVE: To characterise paediatric INH-resistant mycobacterial isolates to investigate the presence of ETH resistance and mutations in the katG gene and the inhA promoter region. METHODS: Forty-five INH-resistant and 19 INH-susceptible Mycobacterium tuberculosis control isolates from children from the Western Cape Province, South Africa, were analysed to quantify INH minimal inhibitory co…

A comparison of the blood levels and urinary excretion of ethionamide and prothionamide in man.

The blood levels and urinary excretion of the anti-mycobacterial drugs ethionamide and prothionamide have been compared after oral dosage in man. High pressure liquid chromatographic methods were used to determine the two closely related thioamides and their microbiologically active sulphoxide metabolites after the ingestion of both single and combined doses of the two drugs. Both drugs were rapidly eliminated from the body, the half-life for the urinary excretion and removal from the plasma of prothionamide being slightly less than that of ethionamide. Less than 0.1% of the orally administere…

[The hepatotoxicity of ethionamide remains a topical subject. Apropos of a case of acute hepatitis].

The authors discuss the case of a 22 year-old patient treated for lepromatous leprosy who developed acute hepatitis 2 years after beginning polychemotherapy associating thalidomide, rifampin, ethionamide and clofazimine. Besides the ethionamide, there was no hepato-toxic factor and the hepatic anomalies quickly disappeared upon termination of treatment with this product. This again draws attention to the hepato-toxicity of ethionamide.

Ethionamide-induced pellagroid dermatitis resembling lichen simplex chronicus: a report of two cases.

Pellagra is a niacin deficiency disorder characterized clinically by diarrhea, dermatitis, and dementia. However, few drugs also cause pellagroid dermatitis. Recently, we encountered two cases of pellagroid dermatitis; both were on second line of antituberculosis drugs. Case 1 was of multidrug-resistant pulmonary tuberculosis. Patient was on ethionamide since one year before developing pellagroid dermatitis. Case 2 was of central nervous system tuberculoma and was on second line of antitubercular drugs. This patient was on ethionamide and isoniazid (INH) since six months before developing pell…

Identification and characterization of ethR and ethA genes impacting the sensitivity of Mycobacterium abscessus to ethionamide.

Mycobacterium abscessus is a rapidly growing non-tuberculous mycobacterium with rising global incidence. This pathogen exhibits intrinsic resistance to most antibiotics, presenting a major public health threat. Ethionamide (ETH) requires bioactivation by monooxygenase EthA to form the active ETH-NAD adduct. We previously identified MAB_3513 (NudC) as a phosphohydrolase that confers intrinsic resistance to M. abscessus by hydrolyzing this adduct. However, deletion of nudC results in only partial susceptibility to ETH, indicating the existence of additional resistance mechanisms. This study iden…

Experimental and computational analyses for elucidation of structural, electronic, thermal, and vibrational properties of ethionamide crystal.

Ethionamide (ETH) is a second-line drug widely used to treat multidrug-resistant tuberculosis (MDR-TB), but its low aqueous solubility compromises bioavailability and limits its therapeutic efficacy. To understand the ways to improve these properties, we combined experimental and computational approaches to elucidate the structural, electronic, thermal, and vibrational properties of ETH crystals. Powder X-ray diffraction analysis revealed a monoclinic crystal system (C1c1-space group), stabilized by intermolecular interactions, primarily H⋯H (49.3%) and H⋯S/S⋯H (22.1%) contacts. Energy framewo…

The MAB_3513c gene plays a key role in intrinsic resistance of Mycobacterium abscessus to isoniazid and ethionamide.

Isoniazid (INH) and ethionamide (ETH) are important antitubercular analogs that inhibit mycolic acid synthesis. However, Mycobacterium abscessus (M. abscessus) exhibits intrinsic resistance to both drugs. The molecular mechanisms underlying this resistance remain poorly understood. We identified MAB_3513c (nudC), encoding a functional NADH pyrophosphatase NudC that hydrolyzes drug-NAD adducts, as being crucial for intrinsic resistance to INH and ETH in M. abscessus. Susceptibility to both antibiotics was markedly enhanced upon either deleting nudC or introducing the P226Q point mutation in thi…

Xpert MTB/XDR for detection of pulmonary tuberculosis and resistance to isoniazid, fluoroquinolones, ethionamide, and amikacin.

BACKGROUND: The World Health Organization (WHO) End TB Strategy stresses universal access to drug susceptibility testing (DST). DST determines whether Mycobacterium tuberculosis bacteria are susceptible or resistant to drugs. Xpert MTB/XDR is a rapid nucleic acid amplification test for detection of tuberculosis and drug resistance in one test suitable for use in peripheral and intermediate level laboratories. In specimens where tuberculosis is detected by Xpert MTB/XDR, Xpert MTB/XDR can also detect resistance to isoniazid, fluoroquinolones, ethionamide, and amikacin. OBJECTIVES: To assess th…
