Extracorporeal shockwave therapy in musculoskeletal conditions: a narrative review.

Extracorporeal shockwave therapy (ESWT) has evolved from its initial application in urologic lithotripsy into an increasingly utilized noninvasive treatment modality for musculoskeletal disorders. This narrative review summarizes the physical principles, mechanisms of action, treatment protocols, contraindications, and clinical applications of ESWT across a range of orthopedic and sports medicine conditions. Two primary forms of ESWT are discussed: focused shockwave therapy (FSWT) and radial shockwave therapy (RSWT), which differ in wave generation, tissue penetration, and energy delivery. Pro…

[Using extracorporeal shockwave therapy in orthopedics--a meta-analysis].

AIM: Up to now ESWT is not a standard therapeutic technique in orthopaedics. The mechanisms of the induced analgesic effect or the mechanism of shock-waves in bony defects are still unknown. By metaanalysis successrates and indications for ESWT are worked out as well as adequate impulse- and energyrates according to actual state of knowledge. Aim of this study is to rate the published cases. METHOD: 105 papers referring to ESWT of the locomotor system are rated. Validation was performed for each paper according to the international accepted system of the American Association of Spine Surgery

Is Extracorporeal Shockwave Therapy Combined With Isokinetic Exercise More Effective Than Extracorporeal Shockwave Therapy Alone for Subacromial Impingement Syndrome? A Randomized Clinical Trial.

UNLABELLED: Study Design Single-blind randomized trial. Background Extracorporeal shockwave therapy (ESWT) has been shown to produce good results in the treatment of subacromial impingement syndrome (SAIS). The efficacy of a combined administration of ESWT and isokinetic exercise (IE) has not yet been studied. Objectives To evaluate the efficacy of focused ESWT combined with IE for the rotator cuff versus focused ESWT alone in the treatment of SAIS. The secondary objective was to assess the isokinetic torque recovery (external rotation at 210°/s, 180°/s, and 120°/s). Methods Thirty participant…

Extracorporeal Shockwave Therapy in the Management of Sports Medicine Injuries.

Treatment of musculoskeletal conditions in athletes with extracorporeal shockwave therapy (ESWT) is gaining popularity as greater evidence supports its use. ESWT protocols (describing energy flux density, number of impulses, type of shockwave (focused or radial), number/frequency/duration of treatment session, area of application, and postprocedural therapy protocols) can be adjusted in the clinical setting. Protocols vary across studies, and optimal protocols for most indications are yet to be determined. ESWT can safely be used to treat various musculoskeletal conditions in athletes, includi…

Effects of Extracorporeal Shockwave Therapy on Wound Healing in Rats.

BACKGROUND: Wound healing remains a major challenge in orthopedic surgeries involving thin soft-tissue injuries, such as calcaneal fractures and open wounds. Recent advancements, such as negative pressure wound therapy, have demonstrated efficacy but have practical limitations, such as patient immobility and fall risk, restricting their broader application. Extracorporeal shockwave therapy, recognized by the International Society for Medical Shockwave Treatment, has shown potential in treating complex wounds, ulcers, and burns. However, the mechanism through which extracorporeal shockwave ther…

Extracorporeal shockwave therapy in plastic, hand, aesthetic, and reconstructive surgery.

Plastic surgery is a multidimensional surgical discipline including reconstructive surgery, hand surgery, aesthetic surgery, and burn surgery, all on the fundament of plastic surgical research. In all of these disciplines, extracorporeal shockwave therapy (ESWT) has been studied both experimentally and clinically. In reconstructive surgery, experimental data clearly demonstrate better survival of flaps when either pre- and/or postconditioned with ESWT, with fewer complications. In hand surgery, ESWT has been described for bone indications like scaphoid non-unions or lunate necrosis aka Kienböc…

Irritant contact dermatitis following extracorporeal shockwave therapy: a case report.

Extracorporeal shockwave therapy (ESWT) has been widely used in the treatment of various musculoskeletal disorders with remarkable efficacy, with no relevant severe complications being reported. In this study, we report a rare case of acute irritant contact dermatitis following ESWT with serious skin damage, which has never been previously reported. A 42-year-old male patient with osteonecrosis of the femoral head (ONFH) was subjected to two sessions of ESWT. In the first session of low-energy ESWT (0.15 mJ/mm2, 3 Hz, and a total of 2,000 impulses), no local or systematic adverse reactions occ…

[Low-intensity extracorporeal shockwave therapy for erectile dysfunction: An update].

Low-intensity extracorporeal shockwave therapy (LI-ESWT) is a novel treatment for erectile dysfunction (ED). With the property of angiogenesis, LI-ESWT acts on vasculogenic ED by improving penile hemodynamics and endothelial function. LI-ESWT is proved to be safe and effective in the treatment of vasculogenic ED in various prospective clinical studies, including randomized, double-blind, and sham-controlled trails. With more multi-centered larger-sample randomized controlled trials, LI-ESWT will play a valuable role in the treatment of ED.

Cellulite and extracorporeal Shockwave therapy (CelluShock-2009)--a randomized trial.

BACKGROUND: Cellulite is a widespread problem involving females' buttocks and thighs based on the female specific anatomy. Given the higher number of fat cells stored in female fatty tissue in contrast to males, and the aging process of connective tissue leads to an imbalance between lipogenesis and lipolysis with subsequent large fat cells bulging the skin. In addition, microcirculatory changes have been suggested, however remain largely unknown in a controlled clinical setting. We hypothesize that the combination of extracorporeal shockwave and a daily gluteal muscle strength program is supe
