Necrotizing fasciitis.

Necrotizing fasciitis is a rapidly progressing soft-tissue infection characterized by extensive necrosis of subcutaneous fat and fascia. It is frequently accompanied by moderate to severe systemic toxicity and can be fatal without prompt recognition and aggressive surgical treatment. The podiatric physician must be aware of these infections because the extremities are common sites of involvement. Necrotizing, fasciitis must be treated as a medical emergency calling for prompt surgical intervention and high doses of broad-spectrum antibiotics.

Necrotizing fasciitis.

Necrotizing fasciitis is an uncommon soft-tissue infection, usually caused by toxin-producing, virulent bacteria, which is characterized by widespread fascial necrosis with relative sparing of skin and underlying muscle. It is accompanied by local pain, fever, and systemic toxicity and is often fatal unless promptly recognized and aggressively treated. The disease occurs more frequently in diabetics, alcoholics, immunosuppressed patients, i.v. drug users, and patients with peripheral vascular disease, although it also occurs in young, previously healthy individuals. Although it can occur in an…

Necrotizing fasciitis.

Ten cases of necrotizing fasciitis are reviewed. Three patients died but only two of these deaths were due to uncontrolled septicaemia. All isolated organisms were sensitive to a combination of piperacillin and ampicillin which we now regard as the initial antibiotic combination of choice. Prompt and aggressive surgical debridement remains the cornerstone of management.

Necrotizing fasciitis.

Early recognition of necrotizing fasciitis is the key to successful treatment. However, initial presentation of this disease is variable. In this case report, the first clinical evidence was a gram stain of the deep surface of the superficial fascia.

Plantar fasciitis.

Thirty patients presenting with the heel pain syndrome, commonly referred to as "plantar fasciitis", were studied prospectively over a two year period. The pain was associated with a calcaneal spur in 21 patients (70%). In a control series of 25 patients without heel symptoms, calcaneal spurs were present in only 4 out of 50 heels (8%). This difference is highly significant (p 0.001). Seven patients (22%) in the plantar fasciitis group complained of ipsilateral sciatica.

Necrotizing fasciitis.

Necrotizing fasciitis is a synergistic infection caused by aerobic and anaerobic organisms, resulting in liquefaction and necrosis of the fascia. Clinical findings typically include septicemia, anemia and radiographic evidence of gas in the soft tissue. Most patients have a history of diabetes or atherosclerosis. Morbidity and mortality can be reduced by early diagnosis, extensive surgical debridement and aggressive nutritional support.

Fasciitis eosinophilica.

Hypergamma-globulinemia and eosinophilia in the peripheral blood were found in a 4-year-old girl with scleroderma-like lesions within the upper and lower limbs. Histological examination excluded scleroderma. After 6 weeks of treatment with steroids, almost a complete retreat of scleroderma-like lesions was achieved.

[Eosinophilic fasciitis].

A three year old is described with clinical findings of oedema and induration of skin in the right leg, eosinophilia, hypergammaglobulinemia, and pathological lesions of eosinophilic fasciitis in the deep muscle's fascia. 94 cases published in the literature are reviewed and clinico-biochemical pathological findings and outcome of this syndrome discussed. In general this entity has a good prognosis with steroid therapy. Authors emphasise the rarity of the disease in children, and the early onset in their patient.

Necrotizing fasciitis.

Two patients had limited necrotizing fascilitis. The morbidity and mortality from this infectious necrotizing process of the superficial fascia have not changed significantly since its initial description in 1924. The characteristic clinical findings of necrotizing fascilitis makes early recognition possible. Within 24 to 48 hours, redness, pain, and edema rapidly progress to central patches of dusky blue discoloration, with or without serosanguineous blisters. By the fourth or fifth day, these purple areas become gangrenous. Although necrotizing fascilities originally was associated exclusive
