An evaluation of fecal incontinence in women with urinary incontinence.

OBJECTIVE: To investigate the occurrence of and risk factors for fecal and/or gas incontinence in female patients having urinary incontinence. PATIENTS AND METHODS: 741 consecutive adult female patients presenting with urinary incontinence alone (group 1, n = 700) or urinary incontinence with fecal and/or gas incontinence (group 2, n = 41) were enrolled into the study. As potential risk factors for fecal and/or gas incontinence in this population, the following variables were investigated: age, body mass index, type of urinary incontinence, childbirth history, menopausal symptoms, history of

Validation of a Novel Fecal Incontinence Scale: The Rapid Assessment Fecal Incontinence Score (RAFIS).

OBJECTIVE: The development and validation of a new version of the fecal incontinence (FI) scale "Rapid Assessment Fecal Incontinence Score" (RAFIS) incorporating domains for severity, type of stool loss, and global perception of the effect of incontinence on quality of life (QoL). BACKGROUND: FI negatively impacts on QoL. Currently used incontinence questionnaires have outstanding limitations on the global assessment of the impact of the disease on QoL that patients perceive. We developed a new version of RAFIS with a more complete questionnaire. MATERIALS AND METHODS: A 3-phase study was pe

Sacral nerve stimulation for treatment of fecal incontinence: a novel approach for intractable fecal incontinence.

PURPOSE: Many patients with fecal incontinence demonstrate a functional deficit of the internal anal sphincter or the external sphincter muscles without any apparent structural defects. Few patients are amenable to repair or substitution of the sphincter. However, sacral nerve stimulation appears to offer a valid treatment option for fecal incontinence. The objectives of this study were: to evaluate the efficacy of temporary stimulation of the sacral nerve roots (percutaneous nerve evaluation) in patients with functional fecal incontinence; to determine the mechanisms of possible improvement;

Fecal incontinence - Challenges and solutions.

Fecal incontinence is not a diagnosis but a frequent and debilitating common final pathway symptom resulting from numerous different causes. Incontinence not only impacts the patient's self-esteem and quality of life but may result in significant secondary morbidity, disability, and cost. Treatment is difficult without any panacea and an individualized approach should be chosen that frequently combines different modalities. Several new technologies have been developed and their specific roles will have to be defined. The scope of this review is outline the evaluation and treatment of patients

Surgical therapies for fecal incontinence.

PURPOSE OF REVIEW: Fecal incontinence is a significant source of morbidity and decreased quality of life (QOL) for many. Until recent years, few therapies beyond medical management were available for patients. Surgical treatment of fecal incontinence has evolved from colostomy and direct repair of muscle defects to interventional techniques such as nerve stimulation and bulking agents. We review the most recent surgical options for the treatment of fecal incontinence within the context of established therapies. RECENT FINDINGS: Overlapping sphincteroplasty is an established therapy that impro

Surgical management of fecal incontinence.

The surgical approach to treating fecal incontinence is complex. After optimal medical management has failed, surgery remains the best option for restoring function. Patient factors, such as prior surgery, anatomic derangements, and degree of incontinence, help inform the astute surgeon regarding the most appropriate option. Many varied approaches to surgical management are available, ranging from more conservative approaches, such as anal canal bulking agents and neuromodulation, to more aggressive approaches, including sphincter repair, anal cerclage techniques, and muscle transposition. [Fecal incontinence--a treatable problem!].

Faecal incontinence has an enormous negative impact on patients' quality of life. Although the causes for faecal incontinence may vary (idiopatic, neurogenic, due to pregnancy, birth trauma or anorectal surgery), nowadays different therapies are available to cure or support patients after failure of conservative treatment. During the past decade, sacral nerve stimulation has shown to provide major improvement to patients with faecal incontinence. Regardless of the underlying disease (sphincter defect, diabetes mellitus, multiple sclerosis or idiopathic) the success rate of SNS lies over 80 %.

Medical management of fecal incontinence.

Diarrhea and constipation are known risk factors for fecal incontinence. This report reviews how to diagnose and medically treat patients with chronic diarrhea, chronic constipation with overflow incontinence, and incontinence resulting from rectal mucosal prolapse secondary to hemorrhoids. Antidiarrheal agents (including loperamide, diphenoxylate, and difenoxin) and the tricyclic antidepressant amitriptyline improve continence in patients with diarrhea-associated incontinence. Other antidiarrheal agents are under investigation. The mechanism is believed to be decreased intestinal motility and

Fecal incontinence: evaluation and treatment.

Fecal incontinence affects men and women of all ages, leading to personal disability and high financial costs. The evaluation of the patient should clarify the pathophysiology of the symptoms and provide guidance in choosing the appropriate treatment. A comprehensive history and physical examination including endoscopic assessment is able to identify the cause of most cases of fecal incontinence. If necessary, functional methods can be used to confirm the diagnosis. Patient selection for suitable treatment is most important and should be based on clinical and physiologic findings. Conservative

[Surgical therapy of fecal incontinence].

The algorithm of the treatment of the stool-incontinence starts with the differentiation between soiling and incontinence for flatus, liquid or formed faeces. In case of soiling, first all the causes that can be surgically solved should be treated. If this is not successful, the scheme of the incontinence-treatment should be followed. The first step is a dietary consultation, which includes prescription of appropriate drugs against diarrhoea, constipation and proctitis. The anal-physiological examinations will inform about the elasticity of the ampulla recti, which can be reduced in inflammato

Fecal incontinence: a clinical approach.

"Fecal incontinence" is defined as the involuntary loss of stool at any time of life after toilet training. It is a socially and psychologically devastating condition for patients and their families, and a topic which both patients and physicians are reluctant to approach. Although the true prevalence of fecal incontinence is unknown, studies have reported it to be as high as 2. 2% in the general population, with significantly higher rates among nursing home residents and hospitalized elderly. Risk factors include advancing age, female gender and multiparity. An understanding of pelvic floor a

[Anal plug in fecal incontinence].

There is evidence that a considerable number of the adult population, most of them women (30-72 years), are suffering from faecal incontinence. Treatment is either conservative sphincter reconstruction or stoma when no other treatment is successful. As an alternative to stoma, the Conveen anal plug was tested in seven women not suitable for surgery. Three patients had substantially improved social life. One of these patients used the plug daily, the other two at specific occasions. The main advantages were that they did not have to use a pad and they were not bothered by inconvenient smell and

Electrodiagnostic evaluation of fecal incontinence.

The aim of this study was to assess the utility of electrodiagnostic testing (EDT) for the evaluation of fecal incontinence (FI). Over a 5-year period, 225 patients (174 females) with FI were prospectively studied with anal manometry, anal ultrasonography, anal electromyography (AEMG), and pudendal nerve terminal motor latency (PNTML) assessment. The mean age was 60 (range 12-94) years. Causes of FI identified by clinical evaluation were obstetric injuries (45), rectal prolapse (43), iatrogenic or other trauma (42), neurologic disease (23), and idiopathic (72). EDT revealed abnormalities in 76

Fecal incontinence and rectal prolapse.

The majority of patients who suffer from full-thickness prolapse can be treated successfully and safely. The small proportion who remain incontinent of feces after correction of prolapse may at a later date be helped by postanal repair. If dietary indiscretion and abnormal patterns of defecation are major etiologic factors in this condition, then measures to improve dietary and defectory habits must be instituted mainly to prevent further problems after the deformity has been cured surgically. There seems no doubt that following rectopexy there is an increased tendency to constipation.

Fecal incontinence. Effective nonsurgical treatments.

Management of the patient with fecal soiling begins with a careful evaluation of possible contributing factors, followed by anorectal examination, neurologic and psychosocial testing, and workup for chronic diarrhea, if present. In many patients, these procedures should be supplemented by radiologic and manometric studies to determine if structural or functional anorectal abnormalities are present. Therapeutic approaches include behavioral, pharmacologic, and surgical methods, which should be carefully considered in relation to the underlying cause of incontinence. For the experienced physicia

Fecal incontinence. Diagnosis and treatment.

This is a survey of fecal incontinence. The etiology is multifactorial. It may develop after traumatic lesion of the anal sphincters, neurological disorders, rectal prolapse or idiopathic. The treatment may be conservative including diet, drugs and sphincter exercise or consisting of suture of the sphincter, post anal repair, free autogenous muscle transplantation. In some cases colostomy is the only way of choice.
