Impact of Intraoperative Femoral Fractures During Cemented Hemiarthroplasty for Femoral Neck Fractures.

BACKGROUND: There have been several studies on intraoperative femoral fractures (IFFs) during primary total hip arthroplasty, but it is not well understood how this complication affects the patient population undergoing cemented hemiarthroplasty. This study aimed to analyze the impact of IFFs sustained during cemented hemiarthroplasty for the treatment of femoral neck fractures. METHODS: A retrospective review was conducted of all patients who were treated for Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association 31B fractures with cemented hemiarthroplasty between Januar

[Femoral neck stress fractures].

Femoral neck stress fractures are relatively rare and caused by repetitive high pressure on the bone with insufficient time to recover. These fractures are often seen in fanatic runners or military personnel, who cover great distances. Patients with a femoral neck stress fracture present with mild pain at the front of the thigh or groin. Radiological imaging includes a plain X-ray and/or MRI-scan. Differentiation between a tension- and compression type stress fracture is important, since their treatment strategies differ. Generally, tension type fractures are unstable and demand operative fixa

Distal Femoral Physeal Fractures.

Physeal fractures of the distal femoral are rare injuries accounting for less than 2% of all physeal injuries, and tend to have a worse prognosis than similar injuries in other locations. This article reviews the evaluation (including imaging), classification, and treatment of these injuries, and discusses their most important complications and their management, including vascular injury and growth arrest.

Femoral nerve block in extracapsular femoral neck fractures.

We randomised 50 patients with extracapsular fractures of the femoral neck to receive either a bupivacaine femoral nerve block or systemic analgesia alone. A femoral nerve block was found to be an easy and effective procedure which significantly reduced perioperative analgesic requirements and postoperative morbidity.

Femoral shaft fractures associated with unstable pelvic fractures.

From December 1986 to December 1989, 23 consecutive adult patients with femoral shaft fractures associated with unstable pelvic fractures were prospectively studied. The mortality rate was 8.7%. An anterior frame of an external fixator was sometimes inserted emergently in patients with multiple injuries to improve their bedside care. Early reamed femoral intramedullary nailing accompanying closed or open reduction with internal fixation, external fixation or both for unstable pelvic fractures was the favored treatment method. Twenty cases were followed for an average of 26 months. The union ra

Proximal femoral locking compression plate for proximal femoral fractures.

PURPOSE: To review short-term outcomes of proximal femoral locking compression plate (PF-LCP) fixation for proximal femoral fractures in terms of postoperative complications and failure rates. METHODS: Medical records of 21 men and 5 women aged 22 to 85 (mean, 49.7) years who underwent internal fixation with the PF-LCP for proximal femoral fractures were reviewed. Younger patients (mean age, 38.7 years) were more commonly involved in high-energy trauma with multiple musculoskeletal injuries, whereas older patients (mean age, 67.7 years) were more commonly involved in low-energy trauma. Distal femoral fractures in children.

The physis of the distal femur contributes to 70% of femoral growth and 37% of the total limb growth; therefore, physeal injury can lead to important alterations of axes and length. Distal metaphyseal corner-type fracture prior to walking is classically associated with child abuse. In children aged >10 years, sports-related fractures and car accidents are significant contributors. Imaging includes a two-plane radiographic study of the knee. It is recommended to obtain radiographs that include the entire femur to rule out concomitant injuries. In cases of high suspicion of distal metaphyseal fr

Management of femoral shaft fractures.

Femoral shaft fractures are severe injuries and are often associated with a high impact trauma mechanism, frequently seen in multiple injured patients. In contrast an indirect trauma mechanism can lead to a complex femoral shaft fracture especially in elderly patients with minor bone stock quality. Hence management of femoral shaft fractures is often directed by co-morbidities, additional injuries and the medical condition of the patient. Timing of fracture stabilization is depended on the overall medical condition of the patient, but definite fracture fixation can often be implemented in the

Bisphosphonate-related subtrochanteric femoral fractures.

BACKGROUND: Hip fractures are important because they are associated with an increased rate of death, substantial morbidity, enormous costs, and devastating effects on independence and quality of life. Bisphosphonates have proven efficacy in reducing the incidence of spine and nonspine fractures. Several case series and multiple individual case reports from around the world suggest that some subtrochanteric fractures and those of the femoral shaft might occur in patients who have been treated with long-term bisphosphonate therapy. CASE SUMMARY: We present a case in the United Kingdom, where a

Classification of subtrochanteric femoral fractures.

A review of the literature identified 15 different classification methods for subtrochanteric femoral fractures. Only eight of those classifications defined the area of bone, which constituted a subtrochanteric fracture. The actual length of femur defined as the subtrochanteric zone varied from 3 cm up to the level of the femoral isthmus. There was no agreement between the different classifications regarding the proximal and distal border or for those fractures, which traverse anatomical boundaries. In the various classifications, fractures were subdivided into 2-15 subgroups. The majority of

[406 femoral fractures in children].

406 fractures of the femur in persons younger than 17 years of age were treated at Haukeland University Hospital, Bergen, from January 1980 to December 1993. The incidence was 35/100,000 per year; 57/100,000 for boys, and 24/100,000 for girls. No significant changes in the incidence occurred during the study period. 70% of the fractures occurred in boys. 78% had an isolated fracture, while 7% had other fractures, 7% head injuries and 8% injuries of multiple organs as well. Traffic accidents accounted for 35% of the fractures. 65% of the femur fractures were treated by skeletal traction (mean h

Quadricepsplasty following femoral shaft fractures.

The results of 12 quadricepsplasties performed on 10 patients with stiff knees following united femoral shaft fractures are reviewed. All fractures were caused by high-velocity road traffic accidents and were associated with a variety of features which led to multiple operations and delayed union. The increased range of flexion achieved in this series is higher than in those reported previously.

Femoral stress fractures in children.

Stress fractures reported in the medical literature almost exclusively affect young athletes, military recruits and patients with metabolic bone disease. The classification of stress-induced bone injury is somewhat confused and includes "fatigue" stress fractures which occur in previously normal bones and "insufficiency" stress fractures which occur in bones weakened by various causes. Femoral stress fractures in children are extremely rare, and we report 5 cases in young patients who sustained their injuries during the course of normal play activities. It is well-known that these lesions can

Simultaneous bilateral femoral shaft fractures.

Simultaneous bilateral femoral shaft fractures are unusual injuries caused by severe force and are usually associated with multiple injuries. There are few reports in the literature. From December 1986 to April 1989, there were 21 consecutive adult (greater than 16 years old) bilateral femoral shaft fracture cases treated at our hospital. The incidence was 2.0% (21/1,065). The associated multiple injuries (42 sites; 2.0 sites per case) markedly influenced the prognosis. There were two deaths, one bilateral above-knee amputation, and 15 cases followed for at least 1 year (average, 25 months). T

Proximal femoral fractures in children.

Proximal femoral fractures in children can be divided into three groups: femoral neck fractures, apophyseal trochanteric separations and subtrochanteric fractures. Femoral neck fractures are extremely rare. They are classified according to Delbet and Colonna into four types. The more lateral is the line of the fracture, the better is the prognosis, and the less sequelae occur. The treatment of femoral neck fractures in children is in nearly all cases surgical, only in some pertrochanteric and in the minority of non-complete basicervical fractures conservative treatment can be considered. The s

[Femoral neck fractures in childhood].

Twenty-eight fractures of the neck of the femur occurred in children between 1963 and 1982 were treated and followed up at the Pediatric Surgical Hospital of the University of Mainz. Three children had transepiphyseal fractures (type I), eight children transcervical fractures (typ II), six children cervicobasal fractures (typ III), and eleven children pertrochanteric fractures (typ IV). Surgical intervention was performed in fifteen children, and thirteen children were submitted to conservative treatment. The results were evaluated according to a new system considering troubles, mobility, and

Update on Atypical Femoral Fractures.

➤ Atypical femoral fractures (AFFs) are stress fractures between the lesser trochanter and the metaphyseal flare that are most commonly related to prolonged (3 to 5 years) antiresorptive medication use.➤ An important finding is a visible transverse line in the lateral cortex, known as the "dreaded black line." Complete fractures are transverse and have minimal comminution.➤ Prodromal symptoms including hip, groin, thigh, and knee pain are present in more than one-half of cases and are usually misdiagnosed.➤ Nonoperative treatment for all patients with AFF includes withdrawal of bisphosphonates

Femoral neck fractures: current management.

Femoral neck fractures are a commonly encountered injury in orthopaedic practice and result in significant morbidity and mortality. It is essential that surgeons are able to recognize specific fracture patterns and patient characteristics that indicate the use of particular implants and methods to effectively manage these injuries. Use of the Garden and Pauwels classification systems has remained the practical mainstay of femoral neck fracture characterization that help dictate appropriate treatment. Operative options include in situ fixation, closed or open reduction and internal fixation, he
