The authors report two cases of fetal bradycardia discovered at the beginning of the 8th month. The pregnancy history was normal. Sonography was helpful in the etiological diagnosis. It concerns a case of atrial extrasystole and another case of physiological bradycardia. The evolution was satisfactory with normal, spontaneous, vaginal delivery. During the pregnancy, the tracing combined a severe bradycardia around 80, a flat rhythm and an areactive rhythm. There was alternation with normal rhythms. The pregnancy history was reassuring. At the beginning of labor, the cardiac rhythm was normal.

The frequency of fetal distress in labour was studied in 46 diabetic women and in 46 non-diabetic matched controls. Fetal distress was assessed by electronic fetal heart rate (FHR) monitoring and fetal scalp blood pH determinations in late first stage of labour. This case report of fetal asphyxia and related studies suggest that fetal endorphins play a role in fetal pain tolerance and cardiovascular asphyctic responses. Elevated fetal endorphin levels may, like narcotics, produce abnormally "flat" fetal heart rate patterns intrapartum and fetal tolerance to pain. Although naloxone hydrochloride was used with some success to return flat fetal heart rate patterns to "normal," the unopposed antagonism of fetal endorphins by naloxone in times of fetal distress may be detrimental.

A case of a term pregnancy with a persistent fetal heart rate of 65 to 80 beats/min is presented. Initially this was presumed to be due to congenital heart block. Subsequent fetal ECG revealed blocked atrial extrasystoles. Review of the literature showed 31 previously reported cases of heart block diagnosed in utero. A review of this literature is discussed and the importance of differentiating bradycardia due to heart block from bradycardia due to fetal distress is emphasized.
