The use of fibrinogen concentrate in coagulation testing aids in the diagnosis and management of rare bleeding disorders.

BACKGROUND: Many clinical laboratories lack the methods or resources to quickly identify rare haemorrhagic diseases, such as genetic/acquired fibrinogen defects and factor XIII deficiency. CASE REPORT: This report explored the value of human fibrinogen concentrate in the diagnosis and management of rare bleeding disorders through three case studies. Case 1 involved acquired hypofibrinogenemia caused by snake bite, where the coagulation abnormality was corrected by adding fibrinogen in vitro, confirming the mechanism of snake venom-specific degradation of fibrinogen. Case 2 utilised fibrinogen

Cryofibrinogen-Characteristics and Association with Cryoglobulin: A Retrospective Study Out of a Series of 1,712 Samples over 7 Years.

OBJECTIVE: Cryofibrinogens (CFs) and cryoglobulins (CGs) are cryoproteins responsible for obstructive vasculopathy and vasculitis. The aim of this study was to compare the characteristics of CF and CG, and to define the conditions of their association. METHODS AND RESULTS: This retrospective study was conducted at the Lyon University Hospitals, and included patients with at least one sample tested for CF and/or CG between September 2013 and April 2021. Serum and plasma samples were analyzed in very strict conditions of temperature. After cold precipitation, CF and CG were characterized and qu

Fibrinogen Sapporo: dysfibrinogenemia characterized by the replacement of A alpha arginine-16 by histidine resulting in the delayed release of fibrinopeptide A by thrombin.

Congenital dysfibrinogenemia was found in a 60-year-old asymptomatic female and her daughter. Purified fibrinogen derived from the propositus, apparently a heterozygote for the abnormality, characteristically showed delayed but complete release of fibrinopeptide A upon digestion with thrombin but its defective release by Ancrod, a snake venom enzyme, from half of her fibrinogen molecules. This congenital dysfibrinogenemia with an A alpha arginine (Arg) to histidine (His) substitution was tentatively designated as fibrinogen Sapporo. Although this type of abnormal fibrinogen had been identified

The thrombotic paradox in congenital fibrinogen deficiencies: from pathophysiology to practice.

Congenital fibrinogen deficiencies (CFDs) comprise rare inherited disorders characterized by quantitative (afibrinogenemia, hypofibrinogenemia) or qualitative (dysfibrinogenemia, hypodysfibrinogenemia) abnormalities of fibrinogen. While CFDs are typically associated with bleeding, a paradoxical risk of both arterial and venous thrombosis is being increasingly recognized. Proposed mechanisms include impaired thrombin clearance due to a lack of fibrin formation and structurally abnormal fibrin clots that promote thrombin release into the circulation or hinder fibrinolysis. In afibrinogenemia, th

Unexpected fibrinogen impairment detected by rotational thromboelastometry in a 76-year-old patient undergoing brain tumor resection: A case report.

BACKGROUND: Fibrinogen abnormalities pose significant challenge in neurosurgical patients, where even subtle coagulation defects may lead to catastrophic bleeding. Viscoelastic hemostasis assays (VHSa) can reveal functional disturbances not captured by conventional coagulation tests. CASE PRESENTATION: We report a 76‑year‑old woman undergoing resection of a brain tumor who presented with mildhypofibrinogenemia (162 mg dL⁻¹) and severely impaired fibrin‑based clot firmness (FIBTEM MCF 5 mm) detected by rotational thromboelastometry (ROTEM). The patient exhibited laboratory evidence of liver dy

Fibrinogen Bethesda: a congenital dysfibrinogenemia with delayed fibrinopeptide release.

A dysfibrinogenemia (fibrinogen Bethesda) was detected in a 9 yr old male of Mexican-English extraction who had a lifelong history of mild bleeding diathesis. The prothrombin and partial thromboplastin times were moderately prolonged; the thrombin and Reptilase times were markedly prolonged. The plasma fibrinogen level was normal by conventional methods but was markedly reduced by the Clauss method. Results of all other tests for clotting factors, fibrinolysis, antithrombin levels, clot stabilization, and fibrin(ogen) degradation products were normal. The patient's plasma and fibrinogen inhibi

Congenital dysfibrinogenemia in major surgery: A description of four cases and review of the literature.

BACKGROUND: Congenital dysfibrinogenemia is characterized by qualitatively abnormal fibrinogens with resultant blood coagulation dysfunction. The clinical manifestations are high heterogeneity. Treatment for dysfibrinogenemia should be personalized. Here, we reported four congenital dysfibrinogenemia patients with the major surgery, in order to discuss the treatment and diagnosis of congenital dysfibrinogenemia. METHODS: We reported four asymptomatic congenital dysfibrinogenemia patients with the major surgery (valve replacement, brain surgery, tumorectomy, hysterectomy) in our study. Routine

Dysfibrinogenemia: from molecular anomalies to clinical manifestations and management.

Congenital dysfibrinogenemia is a qualitative congenital fibrinogen disorder characterized by normal antigen levels of a dysfunctional fibrinogen. The diagnosis is usually based on discrepancies between fibrinogen activity and antigen levels, but could require more specialized techniques for the assessment of fibrinogen function, owing to some limitations in routine assays. Molecular abnormalities, which are frequently heterozygous missense mutations localized in exon 2 of FGA and exon 8 of FGG, lead to defects in one or more phases of fibrinogen to fibrin conversion, fibrin network formation,

Electrospray ionization mass spectrometry identification of fibrinogen Banks Peninsula (gamma280Tyr-->Cys): a new variant with defective polymerization.

Fibrinogen Banks Peninsula was identified in the mother of a patient referred for investigation following recurrent epistaxis. Coagulation tests revealed prolonged thrombin and reptilase times and a decreased functional fibrinogen level. Thrombin-catalysed release of fibrinopeptides A and B was normal, and no abnormalities were detected by DNA sequencing of the regions encoding the thrombin cleavage sites in the Aalpha and Bbeta genes. Reducing SDS-PAGE and reverse-phase HPLC analysis of purified fibrinogen chains were normal, as was electrospray ionization mass spectrometry (ESI-MS) analysis

Congenital (hypo-)dysfibrinogenemia and bleeding: A systematic literature review.

Ranging from bleeding to thrombosis, the clinical features of congenital fibrinogen qualitative disorders, including dysfibrinogenemia and hypodysfibrinogenemia, are highly heterogeneous. Although the associations between some specific fibrinogen mutations and the thrombotic phenotypes have been well elucidated, the underlying mechanism between fibrinogen variants and bleeding events remains underestimated. After systematically reviewing the literature of (hypo-)dysfibrinogenemia patients with bleeding phenotypes, we identified several well-characterized bleeding-related fibrinogen variants in

Biochemical and structural analysis of the interaction between β-amyloid and fibrinogen.

The majority of patients with Alzheimer disease (AD) suffer from impaired cerebral circulation. Accumulating evidence suggests that fibrinogen, the main protein component of blood clots, plays an important role in this circulatory dysfunction in AD. Fibrinogen interacts with β-amyloid (Aβ), forming plasmin-resistant abnormal blood clots, and increased fibrin deposition is found in the brains of AD patients and mouse models. In this study, we investigated the biochemical and structural details of the Aβ-fibrinogen interaction. We identified the central region of Aβ42 as the most critical region

Fibrinogen Dusart presenting as recurrent thromboses in the hepatic portal system.

Dysfibrogenemias are characterized by the production of abnormally functioning fibrinogen, occurring in the presence of liver disease, medication toxicity, malignancy, or genetic mutation. Here, we report a patient with multiple, separate episodes of hepatic portal system thromboses associated with dysfibrinogenemia. Molecular studies identified the presence of a 554Arg→Cys mutation in the fibrinogen Aα gene, previously identified as Fibrinogen Dusart (also known as Fibrinogen Paris V and Fibrinogen Chapel Hill). This case further illustrates the association of this dysfibrinogenemia with a un

Hemostasis in typhoid fever.

Abnormalities of hemostasis in vivo and in vitro were defined in a prospective study of patients with typhoid fever. In a group of 56 patients with a proven diagnosis of typhoid fever, hemostatic abnormalities were frequently found. In patients with normal clotting tests the fibrinogen level was found to be higher than in normal controls. This finding established a new normal level for patients with typhoid fever. Using this redefined normal level it was found that low fibrinogen was associated with other hemostatic abnormalities. Low fibrinogen levels in patients with typhoid fever are possib

Thrombelastography-estimated functional fibrinogen in healthy volunteers, end-stage renal disease, and trauma: inaccurate when most needed.

BACKGROUND: Depletion of fibrinogen is an early event in trauma-induced coagulopathy (TIC), while hyperfibrinogenemia increases thrombotic risk in end-stage renal disease (ESRD). Viscoelastic tests aim to provide rapid fibrinogen estimates, yet their accuracy to define abnormal fibrinogenemia remains debated. OBJECTIVES: We assessed the accuracy of thrombelastography (TEG)-TEG 5000 and TEG 6s-in measuring normal and abnormal fibrinogen levels across healthy volunteers, patients with ESRD, and trauma patients at risk for TIC. METHODS: We compared the von Clauss functional fibrinogen (gold sta

Fibrinogen γ' promotes host survival during Staphylococcus aureus septicemia in mice.

BACKGROUND: Staphylococcus aureus is a common gram-positive bacterium that is the causative agent for several human diseases, including sepsis. A key virulence mechanism is pathogen binding to host fibrinogen through the C-terminal region of the γ-chain. Previous work demonstrated that FggΔ5 mice expressing mutant fibrinogen γΔ5 lacking a S. aureus binding motif had significantly improved survival following S. aureus septicemia. Fibrinogen γ' is a human splice variant that represents about 10% to 15% of the total fibrinogen in plasma and circulates as a fibrinogen γ'-γ heterodimer (phFibγ'-γ).

Discrepancy between parallel anti-Xa and activated partial thromboplastin time is related to mortality in patients with Impella-supported cardiogenic shock.

BACKGROUND: Managing unfractionated heparin (UFH) during percutaneous mechanical circulatory support for cardiogenic shock (CS) is challenging due to potential discrepancies between coagulation tests. OBJECTIVES: To study the causes and consequences of discrepancies between anti-Xa and activated partial thromboplastin time (APTT) for UFH monitoring during microaxial flow pump support (Impella) for CS. METHODS: We assessed patients in CS supported with Impella in 2 tertiary care centers over 62 months. UFH was titrated based on anti-Xa levels with parallel APTT measurements. In-range anti-Xa

Acquired dysfibrinogenemia with discordant fibrinogen assays preceding the diagnosis of λ light-chain multiple myeloma: a case report.

BACKGROUND: Acquired dysfibrinogenemia is a rare and often underrecognized coagulation disorder characterized by impaired fibrinogen function rather than absolute deficiency. In plasma cell dyscrasias, including multiple myeloma, monoclonal gammopathy of undetermined significance, and monoclonal gammopathy of clinical significance, monoclonal immunoglobulins or light chains may interfere with fibrin polymerization, resulting in discordant fibrinogen assay results and diagnostic challenges. CASE PRESENTATION: A middle-aged woman exhibited persistently prolonged thrombin time (TT) and markedly

A study of fibrinogen and fibrinolysis in 10 adults with nephrotic syndrome.

In 10 patients with nephrotic syndrome (NS), the coagulation inhibitors, the fibrinolytic system and several functions of the fibrinogen-fibrin molecule were studied. Among the coagulation inhibitors, only antithrombin III (AT III) was found decreased and correlated with serum-albumin levels. Venous occlusion test provoked a normal tissue plasminogen activator (tPA) release in all patients. The plasminogen activator inhibitor (PAI) had an increased activity in 5 out of the 10 patients. Thrombin and reptilase times were found abnormal in most patients. The thrombin time (TT) prolongation correl

Fibrinogen function is impaired in whole blood from patients with cyanotic congenital heart disease.

BACKGROUND: Patients with cyanotic congenital heart disease (CCHD) have haemostatic abnormities associated with bleeding and thrombo-embolic events. The haemostatic abnormalities are not fully understood, but recent studies indicate that elevated haematocrit and fibrinogen function may be of importance. The aim of this study was to characterise the haemostatic profile and examine the potential role of haematocrit on clot formation and strength in CCHD patients. Furthermore to examine whether CCHD patients with history of haemoptysis have diminished fibrinogen function compared to those without
