5-Fluorouracil induced extravasation injury.

Antineoplastic drugs based on their ability to cause local damage are classified as irritants, vesicants, and non-vesicants. Previous literature has reported higher rate of vesicants induced extravasation (EV) compared to irritants. We report the first case of irritant, 5-fluorouracil causing grade III EV in 55-year-old woman. The patient was diagnosed with esophageal squamous cell carcinoma. Docetaxel, Cisplatin, and 5-Fluorouracil (DCF) chemotherapy regimen was planned and administered through peripheral venous access. Patient experienced grade 3 extravasation in her 3rd cycle following 5-fl…

5-Fluorouracil for glaucoma surgery.

BACKGROUND: Trabeculectomy is performed as a treatment for many types of glaucoma in an attempt to lower the intraocular pressure. The surgery involves creating a channel through the sclera, through which intraocular fluid can leave the eye. If scar tissue blocks the exit of the surgically created channel, intraocular pressure rises and the operation fails. Antimetabolites such as 5-Fluorouracil (5-FU) are used to inhibit wound healing to prevent the conjunctiva scarring down on to the sclera. This is an update of a Cochrane review first published in 2000, and previously updated in 2009. OBJE…

5-fluorouracil-induced erythema multiforme.

Erythema multiforme is an immunologically mediated skin reaction to a foreign antigen and can occur with many drugs. We report the first two cases of erythema multiforme induced by 5-fluorouracil (5-FU). The first case was in a man with inoperable gastric carcinoma being treated by continual infusion who developed the classic rash involving the neck and upper chest, along with mouth ulceration and plantar-palmar erythrodysaesthesia. Histological confirmation was made by skin biopsy and the lesions resolved spontaneously. They recurred on restarting 5-FU. The second case was in a woman being gi…

Subconjunctival sustained release 5-fluorouracil.

The authors have developed a sustained release device for 5-fluorouracil (5-FU) made up of a 12 mg pellet of drug coated in a mixture of permeable and impermeable polymers. When implanted subconjunctivally in rabbits, these devices released 5-FU at approximately 1 mg/d for over 10 days. Devices were implanted into four cynomolgus monkey eyes after posterior lip sclerotomy. One eye (treatment) received a device that contained 12 mg 5-FU and the other eye (control) received a placebo device that contained no drug. In control eyes, intraocular pressures returned to normal within 1 wk. In treatmen…

Clinical pharmacology of 5-fluorouracil.

5-Fluorouracil, first introduced as a rationally synthesized anticancer agent 30 years ago, continues to be widely used in the management of several common malignancies including cancer of the colon, breast and skin. This drug, an analogue of the naturally occurring pyrimidine uracil, is metabolised via the same metabolic pathways as uracil. Although several potential sites of antitumour activity have been identified, the precise mechanism of action and the extent to which each of these sites contributes to tumour or host cell toxicity remains unclear. Several assay methods are available to qu…

[Early cardiotoxicity of 5-fluorouracil].

A bolus of 1000 mg of 5-FU intravenously was given to a 54-year-old patient with adenocarcinoma of colon, a month after hemicolectomy. He had not received irradiation therapy. Five hours later he complained of severe chest pain; after 24 hours ecg. changes of pericarditis were seen and on heart auscultation a pericardial friction rub was heard. After 6 day the ecg. returned to the pattern of that on day of admission to the ICCU. Two further injections of 1000 mg of 5-FU were also followed by severe precordial pain and the same ecg. pattern. The pulmonary edema 14 hours after the second injecti

"Ecdysis" secondary to 5-fluorouracil.

The use of 5-fluorouracil in three patients with basal cell carcinoma was followed by a peculiar necrosis which, when curetted, shelled out completely like a snake skin. We have termed this "ecdysis." Histologic examination revealed tumor necrosis, leukocytes, and fibrin. There have been no recurrences to date.

Photosensitivity due to a fluorouracil derivative.

A photosensitive lichen planus-like eruption developed in a 63-year-old man during postoperative chemotherapy with 1-(2-tetrahydrofuryl)-5-fluorouracil. The action spectrum for the photosensitivity was in the long-wave ultraviolet light (UV-A). The reaction was reproducible on readministration of the drug and exposure to UV-A. At the time of this test, a flare-up phenomenon was observed at previously involved sites. The photopatch test was negative with the drug. Patients taking the drug who had no dermatitis were not sensitive to UV-A. These observations suggest that the photosensitivity that…

Ribavirin sensitizes nasopharyngeal carcinoma to 5-fluorouracil through suppressing 5-fluorouracil-induced ERK-dependent-eIF4E activation.

Although overexpression of eukaryotic translation initiation factor 4E (eIF4E) is associated with enhanced growth and poor prognosis in nasopharyngeal carcinoma (NPC), the function of eIF4E in NPC response to chemotherapy has not been revealed. In this work, we demonstrate that eIF4E inhibition using both ribavirin and siRNA targets NPC cells and enhances the efficacy of 5-fluorouracil (5-FU). Mechanism studies indicate that 5-FU treatment increases phosphorylation of eIF4E in NPC cells, and this is dependent on ERK activation. eIF4E inhibition thus significantly sensitizes NPC cell response t…

Histone deacetylase inhibitors sensitize 5-fluorouracil-resistant MDA-MB-468 breast cancer cells to 5-fluorouracil.

Resistance to 5-fluorouracil (5-FU) is a serious problem in cancer therapy and overcoming it is required in order to improve the efficacy of cancer chemotherapy. Histone deacetylase (HDAC) inhibitors are used in cancer treatments and, recently, it has been reported that HDAC inhibitors can overcome resistance to various anti-cancer drugs in vitro. In the present study, a 5-FU-resistant breast cancer cell line was established, and the effects of HDAC inhibitors in these cells were examined. The 5-FU-resistant cell line MDA-MB-468 (MDA468/FU) was established by continuous exposure of the parenta…

Ectropion following topical 5-fluorouracil treatment.

Topical 5-fluorouracil is a common treatment for actinic keratoses. Its side effects may include intense erythema, erosions, contact dermatitis, systemic intolerance in susceptible individuals, and ulcerations. We present a case of a 78-year-old woman who developed unilateral ectropion after topical 5-fluorouracil application. This case highlights the importance of clear patient instruction while prescribing topical 5-fluorouracil. Patients should be advised to wash their hands thoroughly after application. We also stress the importance of counseling patients to keep the medication away from t…

Topical 5-Fluorouracil associated skin reaction.

Topical 5- Fluorouracil (5-FU) is used more frequently to treat actinic keratosis. We are presenting a skin reaction as a side effect of this medication. Treatment for such cases of 5-FU-induced skin reactions is based on proper skin care and treatment of any superimposed infections. Medical providers should be aware of such side effects to provide their patients with proper instructions to avoid complications.

5-fluorouracil selectively inhibits collagen synthesis.

BACKGROUND: Fibroproliferative disorders, such as Dupuytren's contracture of the hand, are characterized by excessive production of collagen. 5-Fluorouracil has been used to treat fibroproliferative disorders of the eye and skin and is thought to inhibit thymidylate synthetase blocking DNA replication. 5-Fluorouracil has been shown to down-regulate fibroblast proliferation and differentiation in vitro. METHODS: This study investigated the dose-dependent effect of 5-fluorouracil on fibroblast extracellular matrix production. Fibroblasts were derived from tendon and primary Dupuytren's disease …

Cutaneous recall reactions with systemic fluorouracil.

5-Fluorouracil is used extensively to treat actinic keratoses as there is selective excessive uptake of 5-fluorouracil in rapidly dividing cells, thus causing inflammation in lesional skin. We report 2 cases of inflammation occurring on clinically normal skin after fluorouracil had been administered parenterally for carcinoma of the oesophagogastric junction and lower third of the oesophagus, respectively. In the first case, actinic keratoses had previously been treated with topical fluorouracil and in the second case, although there were no previous actinic keratoses, there was a high degree …
