Clinical identifiers for detecting underlying closed cervical fractures.

BACKGROUND: Although uncommon, closed cervical fractures (CCFs) may present in orthopedic clinical settings despite previous workup. The objective of this study was to describe the characteristics associated with missed CCF. METHODS: The study was a retrospective database exploration of a cohort of subjects within a department of surgery with cervical pain. The sample consisted of 162 patients seen for a surgical consult for a number of cervical conditions. The examination time frame represented a clinical examination and imaging confirmation of diagnosis after an original initial visit by an

Closed anterolateral ankle dislocation without fractures: case report.

Ankle dislocation without concomitant fracture is a rare phenomenon with only a few cases reported in literature. Our case report presents a male who sustained a closed anterolateral ankle dislocation without fractures following a road traffic accident. The patho-anatomy, principles of management and probable complications of this injury have been discussed.

Frontal bone fractures: new technique of closed reduction.

Fractures of the nasofrontal orbital complex account for 5% of all facial fractures. Mean hospitalization is usually longer than for other types of facial lesions, independent of the trauma responsible (road accidents, sport, falls, firearm wounds, and so on). The choice of treatment, usually dictated by the site and extent of the damage, is important because inadequate treatment may lead to immediate or subsequent complications. A conservative surgical technique for use in the treatment of non-comminuted frontal lesions is described, which consists of the closed reduction of the depressed fra

Closed treatment of fractures of the proximal humerus.

The results of 42 patients with three- or four-part fractures of the proximal humerus treated by closed methods only were reviewed. In 34 three-part fractures, 24 cases were satisfactory and 10 cases unsatisfactory; the satisfactory results tended to be in an older age group. In four-part fractures all eight cases had unsatisfactory results. It is concluded that closed treatment can give satisfactory results in three-part fractures but not in four-part fractures.

Vascular injuries and closed extremity fractures in children.

The association of a major vascular injury with a closed extremity fracture in the pediatric patient is uncommon. The present study of seven such patients includes five fractures treated by open reduction and internal fixation (ORIF) prior to vascular reconstruction. Two were treated in skeletal traction without vascular reconstruction. Most vascular injuries were not diagnosed initially, consequently delaying repair. Early follow-up examination revealed the following complications: wound infections, below-knee amputation, deep vein thrombosis (DVT), and motor and sensory deficits. All of the

Variations of some plasma components after closed fractures.

1) Variations in the serum concentrations of total proteins and the electrophoretic fractions, glycoprotein, mucoprotein, fibrinogen, erythrocyte sedimentation rate, calcium, phosphorus, alkaline, and acid phosphatases were analyzed until the 30th day following uncomplicated fracture of shafts of long bones of the limbs in 25 cases. 2) A significant fall of albumin with concomitant rise of alpha 1, alpha 2, and beta globulins were noted until 30th day. 3) Mucoprotein, glycoprotein, and fibrinogen showed parallel elevations with that of alpha and beta globulins. 4) The peak values of alpha 1 an

The Treatment of Closed Finger and Metacarpal Fractures.

BACKGROUND: Fractures of the fingers and metacarpal bones are the most common fracture type in the upper limb, with an incidence of 114 to 1483 per 100 000 persons per year. The clinical importance of closed finger and metacarpal fractures is often underestimated; inadequate diagnostic and therapeutic measures may result in serious harm. This review concerns the basic elements of the diagnosis and treatment of finger and metacarpal fractures. METHODS: This review of the incidence, diagnosis and treatment of finger and metacarpal fractures is based on pertinent publications retrieved by a sele

Early versus late surgery for closed ankle fractures.

PURPOSE: To compare the outcome after early versus late surgery for closed ankle fractures in terms of the length of hospital stay and infection rate. METHODS: Records of 95 men and 119 women aged 14 to 92 (mean, 46) years who underwent open reduction and internal fixation for ankle fractures during three 6-month periods in 2004, 2007, and 2010 were reviewed. 82 and 132 patients underwent surgery <24 hours and >24 hours after presentation, respectively. The most common reason for delayed surgery was unavailability of the operation theatre, followed by delayed admission to the fracture clinic

Ultrasound-assisted closed reduction of distal radius fractures.

PURPOSE: To assess the accuracy and ability of ultrasound for monitoring closed reduction for distal radius fractures. METHODS: Consecutive patients undergoing ultrasound-guided closed reduction of acute, displaced distal radius fractures between January 2003 and December 2006 at our department were enrolled. The control group was extracted from patients who underwent a closed reduction for similar fractures under fluoroscopy or without any imaging assistance. To confirm the accuracy of the ultrasonography measurements, displacement distance values were compared with those on radiographic ima

Plate fixation in closed ipsilateral multiple metacarpal fractures.

Closed multiple metacarpal fractures are uncommon. They are usually associated with significant soft tissue swelling. Early stable fixation and functional rehabilitation optimises outcome. We present a review of 19 patients with 43 metacarpal fractures treated by early open reduction and internal fixation with 2 mm plates. Eighteen patients recovered a full range of motion, while one patient was lost to follow-up. Implant removal on account of extensor irritation was required in only two metacarpals in two patients. Plating of multiple closed metacarpal fractures is a safe, reliable and consis

Patient satisfaction after closed reduction of nasal fractures.

OBJECTIVE: To evaluate the outcome of closed reduction from a patient's point of view, because there is increasing evidence that closed reduction of nasal fractures fails to address deformities of the cartilaginous nasal framework and the septum. METHODS: We performed a retrospective study of 62 patients who underwent a closed reduction of nasal fracture between July 1, 2002, and June 30, 2005. All patients were interviewed regarding the esthetic and functional outcomes after closed reduction. RESULTS: Eighteen patients (29%) expressed dissatisfaction with the esthetic outcome of the reducti

Closed reduction of coronal fractures of the capitellum.

Closed reductions of displaced coronal fractures of the capitellum were successfully obtained for nine patients. The method involves spontaneous reduction by allowing the elbow to fully extend under anesthesia and then gradually flexing the elbow while distracting the elbow joint. Distraction allows the radial head to capture the capitellar fragment in the joint rather than push it proximally. The elbow is immobilized at 90 degrees for 3 weeks. Gentle active motion is then started. Closed reduction is often or usually obtained, but open reduction and internal fixation are performed in those ca

Closed Ender nailing of adolescent femoral shaft fractures.

Twenty-seven adolescents with 29 femoral shaft fractures, between the ages of 10 and 16 years, were treated with closed flexible Ender intramedullary nailing from 1989 to 1992 in our clinic. In one patient who had a fracture of the distal third of the femoral shaft, the nail was inserted proximal to distal (antegrade nailing). In the other patients, the nails were inserted distal to proximal (retrograde nailing). Clinical and radiological follow up averaged 32 months (18-56 months). Hospitalization averaged 15 days (7-28 days). All fractures healed with one significant leg length discrepancy o
