Frontal sinus mucocele.

Although not uncommon, and certainly not rare, frontal sinus mucocele was seen in 4 cases by the authors. Since one of the cases was rather unusual, we were prompted to evaluate the subject and prepare this manuscript. Gradual onset of unilateral proptosis should make one suspicious of a mucocele involving the paranasal sinuses, the frontal and ethmoid being the 2 most common locations. Diplopia, due to limited ocular motility on upward gaze, along with proptosis and epiphora are frequently the presenting symptoms which, in one particular case, paradoxically improved at first with topical anti…

[Application of frontal sinus optical probe in endoscopic frontal sinus surgery].

OBJECTIVE: To discuss the application of frontal sinus optical probe in endoscopic frontal sinus surgery. METHOD: The clinical data of 58 adult patients (86 sides) suffering from chronic frontal sinusitis were analyzed retrospectively. The patients received endoscopic frontal sinus surgery with frontal sinus optical probe because of difficulty in confirming ostia under endoscope. All cases were followed up post operation to evaluate the efficacy. RESULT: The ostia of frontal sinus were successfully opened with this method in all cases, and no serious intracranial or orbital complication occu…

Frontal Sinus Fractures.

The frontal bone serves a protective role, representing the transition between the facial skeleton and the cranium. Within its enclosure, the frontal sinus lies adjacent to many important intracranial structures, and injury to this region has severe complications if not properly managed. Historically, the goals of frontal sinus fracture management have been to best prevent intracranial complications with invasive procedures, but a recent shift favoring conservative management now aims to preserve the form and function of the frontal sinus and its outflow tract. With the advancement of endoscop…

Frontal sinus stenting.

Recurrence of frontal sinusitis following external and endoscopic intranasal drainage procedures has been and is presently a challenge to rhinologic surgeons. The complexity of the anatomy, especially with regard to size of the newly created frontal neo-ostium, directly influences long-term success. Frontal sinus stenting is indicated if the neo-ostium is less than 5 mm. Other important factors include excessive denuded bone, remnants of osteitic bone in the frontal recess, and severe mucosal disease as seen in allergic fungal sinusitis and nonallergic eosinophilic rhinitis. Lateralization of …

Frontal sinus adenocarcinoma.

CONTEXT: Paranasal sinus cancer is considered rare, with an incidence of less than 1 per 100,000 per year, with the frontal sinus being the primary site in only 0.3%. We report a case of adenocarcinoma arising in the frontal sinus. DESIGN: Case report. CASE REPORT: A 59-year-old woman, secretary, came in February 1998 with a 4-month history of low intensity frontal headache. She denied contact with wood dust. On examination a non-tender swelling was noted over her right forehead next to the medial aspect of the right orbit. CT scan showed a soft-tissue mass involving frontal sinus with intra…

Frontal sinus stents.

OBJECTIVES: The formation of scar tissue, synechiae, or osteogenesis in the narrow frontal outflow tract after instrumentation of the frontal sinus has led to attempts to enlarge the frontal duct or stenting. STUDY DESIGN: Prospective pilot study. RESULTS: Sixty-four Freeman frontal sinus stents were used to maintain patency in 46 patients. Stents were inserted using an endoscopic technique in 26 sinuses containing polyps, 20 with pansinusitis, and 18 cavities with stenosis of the frontal duct. External incisions with frontal sinusotomy were required to remove the stenosis and insert the ste…

Frontal sinus fractures.

Frontal sinus fractures usually occur in trauma situations involving substantial force and are frequently accompanied by other, sometimes life threatening, injuries. Early recognition of this injury is important to prevent serious complications as well as to restore cosmesis. This paper reviews the incidence, pathophysiology, classification, and controversial management of frontal sinus fractures.

Frontal sinus fractures.

Frontal sinus fractures can have serious consequences due to the proximity of the sinus to the intracranial cavity, and the potential for nasofrontal duct obstruction with its long-term sequelae. Management of these fractures varies dependent on several factors. These include the degree of displacement of the fractures; involvement of the anterior table, the posterior table, and/or the nasofrontal duct; and the presence of cerebrospinal fluid leakage. This paper gives an overview of fracture treatment options, and offers a simple algorithm for management based on the fracture severity and invo…
