The large variety of the lesions in diabetic gangrene which require different therapeutical decisions explains our attempt to determine an useful way for the approach of the surgical treatment. Fournier's gangrene is a rapidly progressive, necrotizing fasciitis of the perineal, genital or perianal regions. Despite increasing knowledge about aetiology, diagnostic procedures and treatment, the gangrene is still a destructive and potentially lethal disease. In two patients, men aged 54 and 63 years, Fournier's gangrene was diagnosed. The first one died of septic shock 12 hours after admission. Surgical débridement had been performed immediately. Fournier's gangrene is a rapidly spreading soft tissue infection. Applying our main principles of therapy to the four patients we have treated in the past 4 years, all patients survived. One case report is presented in detail. Early diagnosis is essential. CT plays an important role. Aggressive resuscitation, daily surgical exploration and debridement remain the key to the management of this disease. The prevention of orchidectomy is one of our aims. Faecal diversion is seldom necessary.

Preview Clostridial myonecrosis (gas gangrene) is an uncommon sequela of traumatic injury. Because the infection can become rapidly fatal, early recognition and expeditious treatment are critical. In addition to wide surgical debridement and treatment with broad-spectrum antibiotics, hyperbaric oxygen therapy has been shown to have synergistic benefit. Dr Stephens presents an illustrative case and discusses current data regarding effective management.

Fournier's gangrene is a relatively rare, mixed aerobic and anaerobic soft tissue infection in the perineoscrotal area. Although it is convenient to give it a separate name, it is really either necrotizing fasciitis or nonclostridial myonecrosis of the scrotal and perineal areas. The condition frequently develops from perineal diseases, including perianal abscess, and fistulas, inflamed haemorrhoids and indwelling urethral catheter. Today Fournier's gangrene ranks among necrotizing fasciitis. Most of the cases reveal the origin of the disease (proctogenic, urologic, gynecologic). Untreated, the polybacterial synergistic infection will overwhelmingly spread along anatomically defined fascias of the pelvic floor. A 66-year-old woman presented with left lower abdominal pain and paresthesias of the left thigh of 6 hours duration. 10 years previously she had undergone massive abdominal irradiation for retroperitoneal lymphoma. 3 days prior to admission she underwent rectal biopsy as part of a work-up for suspected inflammatory bowel disease. Fournier's disease--gangrene of the penis and scrotum--is an uncommon condition. During the past 6 years, five patients, whose cases are described, were admitted to Toronto General Hospital with this diagnosis. Four had preceding trauma (ischiorectal abscess, puncture wound, surgery) and four had pre-existing debilitating problems (diabetes, rectal carcinoma, acute lymphocytic leukemia, alcoholic cirrhosis).
