PURPOSE: To develop an improved percutaneous technique for the insertion of large-bore gastrostomy tubes.
MATERIALS AND METHODS: With use of fluoroscopic guidance, the stomach is punctured and the esophagus is catheterized in a retrograde fashion. A guide wire is passed from the gastrostomy site, up the esophagus, and out of the patient's mouth. A large-bore (20-24 F) endoscopic push-type gastrostomy tube is advanced over the wire, through the mouth, down the esophagus, and out of the gastrostomy site.
RESULTS: Thirty-one successful tube placements were performed in 32 attempts (97% success …

Accessing the stomach via a gastrostomy is the preferred method for providing enteral nutritional support when supplementation is required for more than three or four weeks. Since its introduction in the early 1980s, percutaneous endoscopic gastrostomy has become the most popular method for creating a gastrostomy. It is a quick and cost-effective method and has supplanted open gastrostomy for the establishment of a gastrocutaneous fistula to provide access to the stomach for numerous indications. It is associated, however, with serious and potentially lethal complications which must be complet…

A new method of gastrostomy tube fixation is described using a metal-ring appliance. This maintains the position of the tube, preventing progression of the tube towards the pylorus, and avoids movement, thus diminishing leakage around the tube. The wound and skin are exposed, easily accessible, and avoid the sogginess encountered with the nipple stabilization. The incidence of fungal infections is also reduced.

INTRODUCTION: Laparoscopic fundoplication is a standard surgical procedure for gastroesophageal reflux disease (GERD). In Japan, it is frequently performed in children with severe motor and intellectual disabilities (SMID), many of whom also require gastrostomy for enteral nutrition. Prophylactic fundoplication during gastrostomy remains controversial due to concerns about post-operative complications and the limited number of patients who subsequently require fundoplication.
PATIENTS AND METHODS: This study retrospectively analyzed 47 cases of fundoplication at Tsukuba University Hospital fr…

INTRODUCTION: Gastrostomy feeding tube insertion has become a common procedure as it enables patients who require long term enteral feeding. Conventional surgical gastrostomies were the only way of gaining enteral access in patients in which it is not possible to pass an endoscope or a nasogastric tube required for endoscopic or radiological gastrostomies, and in patients in which certain anatomical abnormalities contraindicate performing these techniques. As conventional surgical gastrostomies are associated with high morbidity, especially gastric leakage around the tube, percutaneous laparos…

BACKGROUND: Percutaneous endoscopic gastrostomy (PEG) has been established as a faster and safer procedure than open surgical gastrostomy. It cannot be done, however, for many patients with partially obstructing pharyngeal or esophageal carcinoma, previous gastrectomy, upper abdominal surgery, or bowel distension from distal obstruction.
PATIENTS AND METHODS: We attempted percutaneous radiologic-assisted gastrostomy (RAG) in 231 patients referred for gastrostomy, 38 of whom had a relative contraindication for PEG. The procedure involves passing, under radiologic guidance, an orogastric inflat…

BACKGROUND: Infants with congenital heart disease often require feeding tube placement to supplement oral intake. Gastrostomy tubes may be placed by either surgical or percutaneous endoscopic methods, but there is currently no data comparing outcomes of these procedures in this population.
AIMS: The aim of our retrospective study was to investigate the perioperative outcomes between the 2 groups to determine if there are clinically significant differences.
METHODS: We reviewed the charts of all infants with congenital heart disease at a single academic institution having isolated surgical or…

Percutaneous endoscopic gastrostomy (PEG), although a standard method for long-time enteral nutrition, still leaves uncertainty about details in indication, technique and complications. Based on own experiences (approximately 1000 PEG's and 170 EPJ's in the last 10 years) these topics are discussed. As new indications the PEG for decompression in chronic intestinal obstruction and the questionable relative contraindications ascites, peritoneal carcinosis and Morbus Crohn are discussed. The common thread-pull through-technique is recommended in a slightly modified variation, according to direct…

We retrospectively studied all gastrostomy buttons inserted in the Royal Children's Hospital, Brisbane between 1988 and 1995. One hundred and thirty-two patients (M = 60, F = 72) and 388 buttons were analysed. Intellectual handicap and cystic fibrosis comprised the majority of patients. Thirty-three patients had gastrostomy buttons inserted primarily, whereas, 99 patients received gastrostomy buttons inserted into matured gastrostomy stoma. The average longevity of all determined buttons (n = 280) was 360.43 days (SD = 310.24). The first buttons inserted primarily (n = 25) had longer longevity…

Gastrostomies play an important role in the management of a wide variety of surgical and nonsurgical conditions of childhood. Many techniques and gastrostomy devices are available. In our experience, percutaneous endoscopic gastrostomy has proved safe and effective, and the gastrostomy button has eliminated most of the catheter-related problems. Candidates for gastrostomy, particularly children with foregut dysmotility, must be carefully selected, undergo preoperative studies aimed at determining the degree of gastroesophageal reflux, and have appropriate long-term follow-up. Attention to tech…

A 2-year-old girl with cyanotic congenital heart disease needed to undergo gastrostomy. To avoid the use of general anesthesia, a percutaneous gastrostomy was performed as an interventional radiologic procedure with the patient under sedation and with the use of local anesthesia. The placement of a nasogastrocutaneous guide wire under fluoroscopic guidance provided a stable tract for placement of the gastrostomy tube.

OBJECTIVES: To describe in detail the technique for percutaneous gastrostomy without gastropexy, to determine the overall success rate for this technique in providing access to the gastrointestinal tract for feeding and to determine if routine gastropexy is necessary.
PATIENTS AND METHODS: The authors reviewed all cases of percutaneous gastrostomy performed over a 14-month period. In total, 24 patients (12 males and 12 females) underwent insertion of gastrostomy tubes in the radiology department during this period. Eighteen of the patients had ear, nose and throat tumours and the other 6 had …

OBJECTIVE: In palliative care, gastrostomies are used to provide nutritional support or to decompress the bowel. To evaluate what happened to the patients monitored at our palliative care unit (PCU) who underwent gastrostomy between October 1994 and January 2005, a retrospective audit was made.
METHOD: The charts of 154 patients were reviewed.
RESULTS: The most frequent reason why a patient underwent a gastrostomy was dysphagia due to head and neck and/or esophageal cancer. Only one patient underwent a drainage gastrostomy because of intestinal obstruction. Interventional radiology performed…

The patient who has had prolonged, severe dysphasia deserves special attention. One method of treatment is the use of a permanent gastrostomy. The authors, modifying the principles of the Beck-Jianu gastrostomy, were able to improve the primary complication of gastrostomy, namely excoriation of the skin secondary to leaking of gastric contents onto the abdominal wall. After operation various tests were utilized to attempt to force gastric contents through the gastrostomy tube onto the abdominal wall. Regurgitation of gastric contents through the modified Beck-Jianu gastrostomy did not occur.

Since their aparition in 1980, percutaneus endoscopic gastrostomy (PEG), represented a dramatic impact in the enteral nutrition thecnics and their indication have expanded. The PEG have many advantages over the surgical gastrostomy. In this paper, we present our experience with the use of PEG in patients with head and neck cancer using a new home made gastrostome. Included were 61 patients 37 males (64.8%), 21 females (36.2%) with ages betwen 11 and 72 years. In 3 patients the PEG was not intended because tight esophageal stenoses. The PEG was completed in 56 cases (91.8%). In 5 patients (8.6%…

OBJECTIVES: The laparoscopic-assisted gastrostomy tube placement (LAP) has increasingly become the preferred method for placing gastrostomy tubes in infants and children. The goal of this retrospective review was to examine our institutional experiences with our transition from the percutaneous endoscopic gastrostomy (PEG) procedure to LAP technique.
METHODS: All patients undergoing primary PEG or LAP gastrostomy at Boston Children's Hospital between January 2010 and June 2015 were identified. The primary aim was to compare complication rates within the first 6 months after tube placement; di…
