[Urination disorders following general surgery].

The disturbance of miction after general surgical operations has three causes. They exist individually and combined. The first cause is a relative trauma of anaesthesiological remedies. They have a central site of action in the brain stem and a peripheral one in the parasympathetic and sympathetic ganglian of the urinary bladder. The second cause is an operative trauma of the abdomino-pelvic initial reflex of the miction. The two traumata, the vegetative and the mechanical one, decompensate an imminent neuropathic or obstructive reduction of the urinary bladder. The two traumata are prolonged

Pulmonary complications of general surgery.

1. Patients should be divided preoperatively into low- or high-risk categories, depending on their probability of developing postoperative pulmonary complications. The evaluation should include spirometry as well as an assessment of the previously defined risk factors. 2. Patients in a low-risk category need only instruction in deep breathing pre- and postoperatively. Routine use of supplemented oxygen postoperatively is reasonable until it can be demonstrated whether such is necessary. 3. High-risk patients should be as free as possible of respiratory secretions at the time of surgery. A regi

HOMEOSTATIC PROBLEMS IN GENERAL SURGERY.

For electrolyte problems that arise during surgical procedures, the surgeon must be versed in the physiologic function of the organs that play vital roles in homeostasis. Pulmonary and renal evaluation before operation can give forewarning of potential dangers. Hyperaldosteronism, a disease entity influencing electrolytic changes and causing other pathophysiological effects, should be understood by the surgeon. Not only should he understand the causes of dehydration, hyperhydration, metabolic and respiratory acidosis and metabolic and respiratory alkalosis, he should also be able to recognize

Pediatric Surgery remains the only true General Surgery.

This article states that Pediatric Surgery remains probably the only remaining General Surgery because it is not about organs and systems but rather the whole Surgery from fetal life until completion of growth and maturation. Pediatric surgeons are currently involved in prenatal treatments for fetal diseases, they take in charge the surgery of congenital malformations, acquired neonatal diseases, common conditions like hernias, undescended testes and appendicitis, but also of the more complex gastrointestinal, broncho-pulmonary or genitourinary conditions, tumors, trauma and solid organ transp…

General surgery of childhood in the UK: a general surgeon's perspective.

The future of general surgery of children as practiced in District General (DGHs) and Rural General Hospitals (RGHs) by adult general surgeons and urologists is uncertain. It is likely that this is because of a combination of the overall trend towards specialization, concerns about clinical risk; uncertainty within the profession about the behavior of the regulator and criminal justice system when considering cases of alleged incompetence; reduced and more targeted training time, curriculum changes, and perhaps a concern by other specialties regarding the ability of DGH and RGH surgeons to pro…

Routine surgical pathology in general surgery.

BACKGROUND: Although pathological analysis provides the definitive diagnosis for most resection specimens, recent evidence suggests that such analysis may be omitted for certain routine samples. This was a retrospective analysis of the value of routine histopathological examination performed in daily general surgical practice. METHODS: All specimens from routine appendicectomies, cholecystectomies, haemorrhoidectomies and inguinal hernia repairs performed between 1993 and 2002 were included. The analysis included a comparison of histological and macroscopic diagnoses, review of preoperative a

General surgical complications following cardiac surgery.

During the five-year period ending July 1981, 3,200 patients underwent cardiac surgery requiring cardiopulmonary bypass at the Medical College of Virginia. Twenty-five patients (0.8%) experienced general surgical complications. Gastrointestinal bleeding was the most common complication encountered. The overall mortality of 24% compares favorably with other reported series. General surgical complications following cardiac surgery are serious and may be caused by aggravation of pre-existing disease, hypoperfusion, stress, or a combination of all three. Although intensive nonoperative therapy may

Seasonal Variation in Emergency General Surgery.

OBJECTIVE: The aim of this study was to assess the seasonal variation in emergency general surgery (EGS) admissions. BACKGROUND: Seasonal variation in medical conditions is well established; however, its impact on EGS cases remains unclear. METHODS: The National Inpatient Sample (NIS) database was queried over an 8-year period (2004-2011) for all patients with diagnosis of acute appendicitis, acute cholecystitis, and diverticulitis. Elective admissions were excluded. The following data for each admission were recorded: age, sex, race, admission month, major operative procedure, hospital regi…

Self assessment questions in general surgery.

The advent of Modernising Medical Careers has meant that many more junior doctors are coming into contact with general surgical patients either as part of Foundation Year 2 or Core Training rotation or during the course of cross covering other specialities due to the hours constraints of the European Working Time Directive. These scenarios are all common general surgical cases that such a junior doctor may be expected to manage.

Outpatient general anaesthesia for oral surgery.

Outpatient general anaesthesia (GA) is administered for extraction of carious teeth in children and straightforward dento-alveolar surgery such as removal of impacted teeth, cyst enucleation and dental implantology in adults. Although oral surgery is often performed under local anaesthesia, GA is indicated in conditions when local anaesthesia is ineffective and in uncooperative patients because of age, fear or anxiety, mental impairment or physical disability. Outpatient GA for oral surgery yields a high satisfaction score from the patients. The incidence of major morbidity is acceptably low b

[Postoperative peripheral neuropathies in general surgery].

INTRODUCTION: Postoperative nerve lesions beyond the operative area, the so called positioning traumas are considered uncommon in general surgery. But they can have serious consequences for the patient and the surgeon, including forensic sequelae. The objective of this work was to describe the incidence, pattern, risk factors and course of postoperative neuropathies in general surgery and to identify indicators to prevent these complications. METHODS: Based on neurologic records we reviewed all postoperative peripheral neuropathies that occurred in the Department of General Surgery of the Uni…

Nasotracheal general anaesthesia in maxillofacial surgery.

Following a survey comprising of 172 patients subjected to nasotracheal general anaesthesia accounting for 37, 1% of all cases of anaesthesia the authors stress the particular usefulness of this method of anaesthesia in maxillofacial surgery. Nasotracheal anaesthesia is the most suitable method of general anesthesia in oral operations and it can almost completely eliminate tracheostomy which is a procedure frequently associated with both early and late complications. No early complications were observed in patients undergoing nasotracheal anaesthesia. Certain improvements of the technique of i
