New therapies for the treatment of genital warts.

The treatment of genital warts with cytodestructive agents such as podophyllin and trichloroacetic acid continues to impose a heavy burden on the workload of genitourinary medicine clinics. Consequently clinics are now looking to new, economical treatments that will improve the management of patients with genital warts. The most promising of these new agents is podophyllotoxin, which is now available in pharmaceutically pure form tailored for the home treatment of external genital warts in males and females.

A microbial game of whack-a-mole: clinical case series of the urethral uncloaking phenomenon caused by Corynebacterium glucuronolyticum in men treated for Chlamydia trachomatis urethritis.

Corynebacterium glucuronolyticum is a rare and neglected, but increasingly recognized bacterial agent of infectious urethritis and other genitourinary syndromes in men. This is the first description of its proclivity to resurface as a cause of sustained urethritis in male patients previously treated for Chlamydia trachomatis, which represents a phenomenon that has to be differentiated from a simple post-treatment overgrowth of a colonizing agent.

[Phase II study of cis-diamminedichloroplatinum (II) in genitourinary cancer].

One hundred and twelve patients with evaluable genitourinary tumors were treated with cis-diamminedichloroplatinum (II) as a single agent under multi-institutional clinical trials. Most patients had extensive prior therapy. Of 30 patients with testicular tumors, 5 complete responses (16.7%) and 15 partial responses (50.0%), of 29 patients with bladder carcinoma, 2 CR (6.9%) and 7 PR (24.1%), of 34 patients with prostatic carcinoma, 6 PR (17.6%), and of 10 patients with pelvis and ureter carcinoma, 1 CR and 3 PR were obtained respectively. No responder was seen in eight patients with renal cell

Best pharmacological practice: urinary tract infections.

Urinary tract infection is the most frequent bacterial infection. Acute uncomplicated urinary infection and acute non-obstructive pyelonephritis occur in young women with normal genitourinary tracts. Empirical short-course therapy is preferred for the management of acute cystitis, but evolving resistance requires continuing reassessment of optimal antimicrobial selection. Empirical trimethoprim or trimethoprim/sulfamethoxazole has been recommended, but increasing resistance to these agents suggests that pivmecillinam, nitrofurantoin and perhaps fosfomycin trometamol should be considered. Stable Ozonides plus Vitamin E Acetate (Ozoile) for Treatment of Genitourinary Syndrome.

Background and Objectives: Genitourinary syndrome, previously defined as vulvovaginal atrophy, manifests with signs and symptoms deriving from estrogen diminution in the female genitourinary tract. Stable ozonides are derivatives of artemisinin found to be stable against strong basic and acidic conditions. Vitamin E is an important antioxidant diminishing the output of reactive oxygen species in the oxidation of fats and the emanation of free radicals, reducing cellular injury and aging. The primary aim of the present study was to assess the positive effects of an ozonide plus a vitamin E acet

FASN as a Therapeutic Target in Genitourinary Diseases: From Cancers to Benign Disorders.

Fatty acid synthase (FASN) is the key catalytic enzyme of de novo lipogenesis and is increasingly recognized as a driver of metabolic reprogramming in genitourinary disease. In both malignant and non-malignant settings, aberrant FASN expression and activity are associated with tumor growth, metastatic spread, drug resistance, renal lipotoxicity, fibrosis, and immune remodeling. However, current evidence remains scattered across organ-specific studies, and a focused synthesis across the genitourinary system is still lacking. This review summarizes the expression patterns, upstream regulators, d

Color Doppler ultrasound of the genitourinary tract.

This paper discusses the role of color Doppler ultrasonography (CDU) as it applies to the genitourinary tract. CDU is beneficial in evaluating both the native and allograft kidney in the diagnosis of renal arterial and venous stenosis, arteriovenous fistulas, pseudoaneurysms, arterial and venous thrombosis, and in the characterization and staging of renal masses. CDU has redefined the role of ultrasonography in the evaluation of the scrotum as it relates to testicular torsion, inflammatory disorders and varicoceles. Early results of CDU in prostate carcinoma and benign prostate hypertrophy hav

Autoinfection as a cause of postpartum subdural empyema due to Mycoplasma hominis.

Mycoplasma hominis is a commensal of the genitourinary tract, which is infrequently associated with urogenital infections. Extra-urogenital infections due to M. hominis are rare. Here, we report an unusual case of M. hominis subdural empyema in a woman occurring shortly after delivery. The patient presented with symptoms suggestive of bacterial meningitis. Spinal imaging revealed a subdural empyema that required neurosurgical intervention. Cultures from intraoperatively obtained biopsies identified M. hominis as the causative pathogen. The patient was treated with oral moxifloxacin for 4 weeks

Minimally Invasive Therapy for Upper Tract Urothelial Cell Cancer.

Minimally invasive therapy for upper tract urothelial cell cancer has assumed an important role in the management of this relatively uncommon genitourinary malignancy. In this review, current standards for patient selection, surgical approach, as well as overall outcomes will be discussed. Moreover, the use of intraluminal agents in upper tract disease is examined along with its efficacy and associated complications.

The role of nanobacteria in urologic disease.

Recent data proposing an extremely small, self-replicating agent termed "nanobacteria" has raised a great deal of controversy within the scientific community. Since these agents have been isolated within the genitourinary tract, much research has focused attention on the potential role these particles may play in the development of urologic pathology, including polycystic kidney disease, renal calculi, and chronic prostatitis. Recent clinical research targeting these agents has proven effective in treating some patients with refractory category III prostatitis (chronic pelvic pain syndrome). T

New and emerging therapies for bone metastases in genitourinary cancers.

CONTEXT: Bone metastases are a common feature of advanced genitourinary malignancies and a prominent cause of morbidity and mortality. OBJECTIVE: The objective of this review is to discuss the incidence, pathophysiology, and management of bone metastases in the most prevalent genitourinary malignancies. EVIDENCE ACQUISITION: We reviewed the relevant medical literature, with a particular emphasis on prospective randomized controlled trials. Much of the relevant clinical trial data focus on prostate cancer (PCa). We provide a nonsystematic review and our perspective on the available data. EVI

[Cowper's gland: anatomic, physiological and pathological aspects].

Cowper's glands are small appendages of the male genital tract, that are sometimes observed on intravenous urography voiding films. They are involved in the immune defence of the genitourinary tract, play a role in fertility, and secrete many glycoproteins, including PSA. They can be visualized in the form of a duct image parallel to the urethra, sometimes associated with opacification of the gland on IVU. This image can be differentiated from a fistula, extravasation of contrast agent, urethral duplication, or an artefactual image, by the course parallel to the urethra, the upper limit not ex

[Urinary Tuberculosis: Serious Complications May Occur when Diagnosis is Delayed].

Genitourinary tuberculosis is the third most common form of extrapulmonary tuberculosis, comprising 4 - 17% of extrapulmonary forms. The authors describe the case of a patient with recurrent urinary tract infections, without isolation of an infectious agent and without symptomatic resolution, despite antibiotic treatment. Imaging exams showed left ureteral stenosis with moderate hydronephrosis. The attempt of retrograde catheterization was impossible so we opted for percutaneous nephrostomy to renal relief. Microbiological urine analysis colleted by that way was positive for Mycobacterium tube

Treatment strategies in mucous membrane pemphigoid.

Mucous membrane pemphigoid (MMP) is an autoimmune blistering disorder that is characterized by subepithelial bullae. Various basement membrane zone components have been identified as targets of autoantibodies in MMP. Considerable variability exists in the clinical presentation of MMP. Mucous membranes that may be involved include the oral cavity, conjunctiva, nasopharynx, larynx, esophagus, genitourinary tract, and anus. A multidisciplinary approach is essential in the management of MMP. Early recognition of this disorder and treatment may decrease disease-related complications. The choice of

Chronic sexually acquired reactive arthritis secondary to Chlamydiatrachomatis.

Reactive arthritis is included in the spectrum of seronegative spondyloarthritides, occurring secondary to triggers of genitourinary and gastrointestinal tract infections. We describe two cases of sexually acquired reactive arthritis secondary to genital infection by Chlamydia trachomatis, diagnosed by in-house polymerase chain reaction performed on the first void urine. Both patients were managed with a combined approach of short course antibiotics, immunosuppressive agents, biologicals and surgical intervention.

A cross-sectional survey of treatment choices for anogenital warts.

This was a cross-sectional survey that collected data relating to management of anogenital warts (AGW) during a single-patient visit only at genitourinary medicine clinics. Single-agent use of cryotherapy, podophyllotoxin and trichloroacetic acid (TCA) were the most common treatment modalities, accounting for over two-thirds of all modalities used. Podophyllin, alone or in combination with other agents, was used for about 20% of first-line treatments. Podophyllin was included in about 15% of all treatment modalities. Guidelines for the management of AGW continue to recommend the use of podophy

Current experience with quinolones in the treatment of urinary tract infection.

Urinary infection in some clinical situations remains difficult to treat with standard oral therapy. Particular problems occur in individuals with underlying genitourinary abnormalities, with intolerance to multiple oral agents, with infections due to resistant organisms, and in men with prostatitis. The fluoroquinolone antimicrobials have an extended spectrum of activity compared to standard oral therapy, achieve high concentrations in the urine following the usual therapeutic doses, and are relatively free of major adverse effects. Thus, they may be effective therapy for some of the clinical

Fungal infections of the genitourinary tract.

Fungal infections of the urinary tract are increasing in incidence, mostly because of the increasing use of antibacterial agents and indwelling urinary catheters. This review will focus mainly on the spectrum of genitourinary infections caused by Candida spp., including asymptomatic candiduria, cystitis, pyelonephritis, and renal candidiasis. Special emphasis will be placed on the therapeutic approach to the various clinical entities. Other fungal infections, such as urinary aspergillosis and cryptococcosis will be discussed briefly.

Genitourinary dysfunction in Parkinson's disease.

Bladder dysfunction (urinary urgency/frequency) and sexual dysfunction (erectile dysfunction) are common nonmotor disorders in Parkinson's disease (PD). In contrast to motor disorders, genitourinary autonomic dysfunctions are often nonresponsive to levodopa treatment. The brain pathology causing the bladder dysfunction (appearance of overactivity) involves an altered dopamine-basal ganglia circuit, which normally suppresses the micturition reflex. By contrast, hypothalamic dysfunction is mostly responsible for the sexual dysfunction (decrease in libido and erection) in PD, via altered dopamine
