Outcome of Gonioscopy-Assisted Transluminal Trabeculotomy in Uveitic Glaucoma.

PRCIS: GATT was effective in decreasing the IOP and the number of glaucoma medications significantly in patients with UG. Active uveitis at the time of surgery was associated with a higher likelihood of requiring glaucoma medications. PURPOSE: To study the efficacy and safety of gonioscopy-assisted transluminal trabeculotomy (GATT) in uveitic glaucoma (UG). METHODS: This was a prospective interventional case series that included 42 eyes of 35 patients with open angle UG who required glaucoma surgery. Cases requiring combined GATT and cataract surgery were excluded. Surgical success was defin…

Use of gonioscopy in medicare beneficiaries before glaucoma surgery.

PURPOSE: The American Academy of Ophthalmology Preferred Practice Patterns for angle closure and open-angle glaucoma (OAG) patients recommends performing bilateral gonioscopy upon initial presentation to evaluate the possibility of narrow angle or angle-closure glaucoma (ACG) and then repeating the examination at least every 5 years. This study aims to assess how commonly eye care providers perform gonioscopy before planned glaucoma surgery in OAG, anatomic narrow angle, and ACG in the Medicare population. METHODS: Data obtained from a 5% random sample of Medicare beneficiaries undergoing gla…

Panscleritis as an Unusual Complication of Gonioscopy-assisted Transluminal Trabeculotomy.

PURPOSE: The main purpose of this study was to report a case of panscleritis after gonioscopy-assisted transluminal trabeculotomy (GATT). METHODS: We describe the case of a 33-year-old man with a history of unilateral anterior uveitis and glaucoma, who developed panscleritis after GATT surgery. RESULTS: Uneventful GATT surgery was performed in a glaucomatous left eye. On the second postoperative day, the patient developed tenderness on palpation of the eye. Ophthalmic examination revealed significant diffuse hyperemia, elongated and tortuous upper bulbar conjunctival/scleral vessels, and exu…

Unique advantage of gonioscopy for viewing an anterior pyramidal cataract.

Anterior polar cataracts (APC) are a relatively rare but well-documented form of congenital cataract. Occasionally, these white disc-shaped opacities protrude towards the anterior chamber with an inverse cone-like morphology and are known as anterior pyramidal cataracts. These represent three to four per cent of all types of congenital cataracts. APCs portray a dominant mode of inheritance and generally do not progress over time. Due to their small size, APCs generally have little effect on vision and are simply monitored clinically. Photodocumentation of pyramidal cataracts has traditionally

Indications for gonioscopy and assessment of gonioscopic signs in optometric practice.

The indications for optometrists to carry out gonioscopic examination are proposed and their significance is discussed. Gonioscopic examination is directed toward recognition of angle closure, or risk of closure, as well as neovascularization, neoplastic activity, inflammation, the results of injury, or degenerative changes at the angle. A schema sets out the decision-making processes which may be used to assist interpretation of gonioscopic signs and their correlation with other clinical data.

Gonioscopy: uses old and new. The inheritance of occludable angles.

The goniolens has become increasingly important in the practice of ophthalmology, For example, the treatment of angle closure and neovascular glaucoma is most effective in the earliest stages of the diseases, even prior to the onset of symptoms. Routine gonioscopy is essential if patients are to be provided optimum care. The critical nature of pressure gonioscopy is reviewed. The use of the goniolens to examine the corneal endothelium is described and recommended. Characterization of the configuration of the angle recess demands description of at least three aspects: (1) the angular approach t…

Wipe-Out Following Gonioscopy-Assisted Transluminal Trabeculotomy Combined With Phacoemulsification.

PRÉCIS: We report a case of a 62-year-old male patient who developed a wipe-out phenomenon after uncomplicated gonioscopy-assisted transluminal trabeculotomy combined with cataract surgery. ABSTRACT: A 62-year-old male patient with a diagnosis of pseudo-exfoliation and open angle glaucoma underwent uncomplicated gonioscopy-assisted transluminal trabeculotomy (GATT) combined with phacoemulsification surgery due to advanced glaucomatous damage and high intraocular pressure. Postoperative decrease in visual acuity and central visual field deterioration suggested a wipe-out phenomenon. Although t…

Agreement between gonioscopy and ultrasound biomicroscopy in detecting iridotrabecular apposition.

OBJECTIVE: To assess the agreement between findings obtained at dark-room gonioscopy and ultrasound biomicroscopy (UBM) in the diagnosis of iridotrabecular apposition in light and dark conditions. METHODS: We enrolled patients with appositional angle closure at dark-room gonioscopy performed using a 1-mm slitlamp beam that did not cross the pupil. Ultrasound biomicroscopic images were acquired in normal room light and subsequently with all room lights off. Images were evaluated for the presence or absence of iris-cornea contact. The angle opening distance at 500 microm was calculated. RESULT…

Gonioscopy-assisted transluminal trabeculotomy: Passing the right way decoding suture.

BACKGROUND: Trabeculectomy is still the most frequently performed glaucoma surgery. But being an invasive procedure, it has numerous vision-threatening complications along with bleb-related complications like cystic bleb, overhanging bleb blebitis, or endophthalmitis. The choice of therapy between medical laser and filtering surgery is now bridged by minimally invasive glaucoma surgeries (MIGS), which are conjunctiva-sparing procedures with adequate intraocular pressure (IOP)-lowering effects and good safety profiles. Among the armamentarium of MIGS procedures, gonioscopy-assisted transluminal…

[Gonioscopy after foldable lens implantation through a sutureless tunnel incision].

OBJECTIVE: To investigate the changes of internal incision and the peripheral iris after cataract surgery through a sutureless tunnel incision. METHOD: Gonioscopy was performed on 58 eyes of 56 cases having undergone phacoemulsification and foldable intraocular lens implantation through a 3.5 mm sutureless tunnel incision. RESULTS: Gonioscopic findings showed that the internal incision was located in clear cornea anterior to Schwalbe's line in 56 eyes and just at the line in 2 eyes. Forty-two eyes (72.41%) had a smooth linear internal incision. There were small tags or curls in 14 eyes (24.1…

Statistical agreement of gonioscopy performed as a puppy and as an adult in the Samoyed dog.

OBJECTIVE: To assess whether gonioscopy performed on Samoyed puppies (6-14 weeks of age) is a reliable predictor of their gonioscopy results as adults (>11 months of age). PROCEDURE: A retrospective medical record search was performed to identify Samoyed dogs that had undergone gonioscopy as a puppy (6-14 weeks of age). A second prospective gonioscopy examination was performed as an adult (>11 months of age) and the results were compared. Gonioscopy was assessed on two characteristics: percent of iridocorneal angle (ICA) unaffected by pectinate ligament abnormalities (PLA) and ICA width, with…
