[Application of four variants of the diagnostic disc test (CD01, CD02, CD03, CD04) for detection of ESBL-positive strains of gram-negative rods].

The aim of this study was to evaluate the usefulness of four variants of the diagnostic disc test (DD) to detect extended-spectrum beta-lactamases (ESBLs) in nosocomial strains of gram-negative rods. Also, the diagnostic disc test (DD) was compared with the double-disc synergy test (DDST) for the effectivity of ESBLs identification. A total number of 111 ESBL-positive (DDST-positive) strains of gram-negative rods isolated from hospitalized patients in 2004 was examined. Ninety nine strains belonged to enteric rods (89.2%) and twelve strains--to nonfermentative rods (10.8%). Two reference strai

The conundrum of the gram-positive rod: are we missing important pathogens in complicated skin and soft-tissue infections? A case report and review of the literature.

BACKGROUND: The designation "gram-positive bacillus" includes a variety of pleomorphic microorganisms, including diphtheroids, coryneform sp … d for species identification. RESULTS: Microscopic inspection identified small, non-spore-forming, gram-positive rods, arranged in clusters, that formed circular, smooth colonies. These were facultatively anaerobic, catalase-negative, non-hemolytic, and unable to reduce nitrates. Standard techniques and assays were unable to identify our organism to species. Ultima …

Clinical Characteristics, Treatment, and Outcomes of Veterans with Cerebrospinal Fluid Culture Positive for Gram-Negative Rod Bacteria: A Retrospective Analysis over 18 Years in 125 Veterans Health Administration Hospitals.

Optimal management for patients with bacterial ventriculitis/meningitis due to Gram-negative rods (GNRs) has yet to be well investigated. We assessed the clinical characteristics, treatment, and outcomes of patients with a positive cerebrospinal fluid (CSF) culture for GNRs. We conducted a retrospective cohort study of all patients with a positive CSF culture within the Veterans Health Administration (VHA) system during 2003-2020. Clinical and microbiological characteristics between the true meningitis and contamination groups were compared. Of the 5919 patients with positive CSF cultures amon

Performance of the Qvella FAST system with downstream application of MALDI-TOF-MS, VITEK 2, and β-LACTA test for same-day identification and antimicrobial susceptibility testing of Gram-negative rods directly from positive blood cultures.

Direct identification (ID) and antimicrobial susceptibility testing (AST) of bacteria from positive blood cultures (BCs) are crucial for rapid optimization of therapy in sepsis. The FAST system purifies a Liquid Colony (LC) from BC broth within 25-35 min for downstream applications. We investigated the performance of the FAST system on BCs growing Gram-negative rods combined with mass spectrometry (MALDI-TOF-MS), the β-LACTA-test, and VITEK 2 AST from LC. VITEK 2 AST results were analyzed after 6 and 8 h, and their impact on empiric antibiotic therapy was examined. We included 119 BCs. MALDI-T

Gram staining for the treatment of peritonsillar abscess.

Objective. To examine whether Gram staining can influence the choice of antibiotic for the treatment of peritonsillar abscess. Methods. Between 2005 and 2009, a total of 57 cases of peritonsillar abscess were analyzed with regard to cultured bacteria and Gram staining. Results. Only aerobes were cultured in 16% of cases, and only anaerobes were cultured in 51% of cases. Mixed growth of aerobes and anaerobes was observed in 21% of cases. The cultured bacteria were mainly aerobic Streptococcus, anaerobic Gram-positive cocci, and anaerobic Gram-negative rods. Phagocytosis of bacteria on Gram stai

Oral flora and pathogenic organisms.

Oral microbial flora consist of numerous bacterial taxa, ranging from aerobes through fastidious anaerobes, and fungi, viruses, and protozoa. Many of these bacteria are unique to the oral cavity. The organisms exist in a complex interrelationship that is regulated and maintained by physical and metabolic microbial interactions, and by environmental factors, such as saliva and diet. Many of these organisms are relatively harmless, although others are significant pathogens, producing local and systemic diseases in healthy and compromised individuals.

Microbiological studies of carious dentine from human teeth with irreversible pulpitis.

Aerobic and anaerobic cultures were made from 29 extracted teeth with irreversible pulpitis to identify the predominant flora in different parts of deep carious lesions. Most isolates were Gram-positive rods, in which lactobacilli were the most frequent organisms, then other Gram-positive, non-branching rods. Gram-positive cocci were the next most common; only a low number of Streptococcus mutans was recovered. Two types of carious lesions were found, one with high numbers of lactobacilli, the other with low. In the 15 lesions with high numbers, lactobacilli constituted 91.9% of the total flor…

Erysipelas in a free-ranging Hawaiian crow (Corvus hawaiiensis).

We describe a case of erysipelas in a free-ranging endangered Hawaiian crow. The partially scavenged carcass exhibited gross emaciation and petechial hemorrhages in both lungs. Microscopy revealed multiple necrotic foci associated with gram-positive rods in the liver and adrenal, diffuse acute proximal tubular necrosis of kidney, diffuse necrosis and inflammation of proventricular mucosa associated with gram-positive rods, and multiple intravascular aggregates of gram-positive rods associated with thrombi. Culture of the kidney revealed the bacterium to be Erysipelothrix rhusiopathiae. The imp…

Acute peritonsillar abscess caused by Arcanobacterium haemolyticum.

A patient is reported with a peritonsillar abscess yielding Arcanobacterium haemolyticum. This appears to be only the fifth such case described in the medical literature and the first from Europe. The organism has been reported as an occasional cause of tonsillopharyngitis with rash, resembling infection with Streptococcus pyogenes but often unresponsive to penicillin therapy. A. haemolyticum easily passes unrecognized in bacteriological cultures as a result of its slow growth, coryneform appearance in the Gram's stain and weak haemolytic activity on conventional laboratory media.

Incidence of beta-lactamase production and antimicrobial susceptibility of anaerobic gram-negative rods isolated from pus specimens of orofacial odontogenic infections.

The incidence of beta-lactamase production in anaerobic gram-negative rods isolated from 93 pus specimens of orofacial odontogenic infections and the antimicrobial susceptibility of these isolates against 11 antibiotics were determined. A total of 191 anaerobic gram-negative rods were isolated from the specimens. Beta-lactamase was detected in 35.6% of the black-pigmented Prevotella and 31.9% of the nonpigmented Prevotella. However, no strains among the other species isolated produced beta-lactamase. Ampicillin, cefazolin and cefotaxime showed decreased activity as regards beta-lactamase-posit

Nosocomial bloodstream infection in pediatric patients: Siriraj Hospital, Bangkok; 1996-1999.

A retrospective study on nosocomial bloodstream infection (NBSI) in pediatric patients hospitalized at Siriraj Hospital from January 1996 to December 1999 was performed. Of the 18,087 blood specimens sent for culture, 533 (3%) were positive for organisms after 72 hours of hospitalization and were defined as NBSI. The rate of NBSI detected in blood culture specimens was highest among neonates (5.2%). Gram-positive cocci and gram-negative rods caused NBSI in an equal proportion (46% and 44% respectively) and Candida caused 10 per cent of NBSI. Coagulase-negative staphylococci was the most common

A case of recurrent acute cholecystitis caused by Actinomyces odontolyticus, rare actinomycosis.

BACKGROUNDS: Actinomyces species are gram-positive, obligate anaerobic rods and are rare causes of cholecystitis. Because Actinomyces species are anaerobic bacteria, it is difficult for Actinomyces to survive in bile apart from A. naeslundii. We experienced a case of recurrent acute cholecystitis caused by A. odontolyticus. CASE PRESENTATION: A patient had been diagnosed with acute cholecystitis and treated one month before and after that, admitted to our hospital because of recurrent cholecystitis. Gram stain of the bile revealed gram-positive rods and gram-positive cocci. We found A. odonto…

Relationship between Gram negative enteric rods, Aggregatibacter actinomycetemcomitans, and clinical parameters in periodontal disease.

BACKGROUND: The association between Gram negative enteric rods and Aggregatibacter actinomycetemcomitans in periodontal diseases has received little attention in the literature. The objective of this study was to explore the relationship between these organisms and clinical parameters of periodontal disease. MATERIALS AND METHODS: Clinical parameters and occurrence of Gram-negative enteric rods and A. actinomycetemcomitans were examined in 76 patients with chronic periodontitis. Chi-square and Mann-Whitney tests were used to determine differences in clinical variables versus the presence or a

A bacteriological and histological evaluation of 58 periapical lesions.

Periapical tissue from 58 cases requiring periapical surgery was examined histologically and cultured for the presence of microbes. Twenty-nine had a possible oral cavity communication and 29 did not. Approximately one-half of each biopsy was submitted for culture while the other portion was examined histologically. Cultures were positive for the presence of bacteria in 51 of 58 cases while bacteria were seen histologically in only 8 of 58 cases. A total of 50 different species of bacteria were isolated from the 58 cultures of periapical tissue. Of 133 isolates, 87 were strict anaerobes, 37 we

Tsukamurella: a cause of catheter-related bloodstream infections.

Tsukamurellae are strictly aerobic Gram-positive rods that can be easily misidentified as Corynebacterium species, Rhodococcus species, Nocardia species, Mycobacterium species, or other Gram-positive aerobic rods. They have been uncommonly reported as a cause of different human infections, including bloodstream infections. We describe 2 new cases of catheter-related bloodstream infections (CR-BSI) caused by Tsukamurella species and review 12 similar cases reported in the literature. Conventional procedures have often misidentified Tsukamurella species as other aerobic Gram-positive rods. This …
