Vasectomy: An Opportunity for Obstetricians and Gynecologists.

Vasectomy is a safe, effective, and cost-effective contraceptive method, with a failure rate lower than that of female sterilization. Nevertheless, vasectomy is underutilized-only 6% of American women rely on vasectomy. Access to vasectomy may be limited by provider advocacy and availability. Obstetricians and gynecologists can increase both the acceptability of and access to this method if adequately trained to counsel about and provide the procedure. Although some concerns may persist surrounding obstetricians and gynecologists performing a vasectomy, increasing the availability and uptake o…

Management of Infection for the Obstetrician/Gynecologist.

Pregnant women have an increased morbidity and mortality for certain illnesses owing to the physiologic and immunologic changes in pregnancy. Certain infections are common during pregnancy, including urinary tract infections and pneumonia. Others are uncommon, but yield increased severity, including influenza. Human immunodeficiency virus, although it does not increase in pathogenesis during pregnancy, requires specific attention and management in the context of pregnancy.

Lawson Tait: the forgotten gynecologist.

The development of gynecology as a specialty, although primarily American in origin, was influenced in large degree by Robert Lawson Tait, a brilliant Scottish/English surgeon who practiced in the late 19th century. Tait, a self-proclaimed gynecologist, is perhaps most widely known as the first to perform salpingectomy to treat ruptured tubal pregnancy. He was also the first to record removal of an ovary for relief of pelvic pain and to induce menopause, perform salpingectomy for the treatment of tubal disease, and develop the technique of transverse transperineal repair of low rectovaginal fi

The gynecologist and the prevention of cardiovascular disease.

This article reviews current concepts regarding the prevention of cardiovascular disease for women, with particular attention to modifiable risk factors. The background describes the magnitude of the problem, assesses the quality of the data with respect to risk factor reduction, and emphasizes several important concepts. Changes at menopause, states of endocrine aberration, and benefits and risks of hormone substitution need to be understood in conjunction with all other potentially modifiable and nonmodifiable cardiovascular risk factors. Primary care physicians have a window of opportunity

DATA SOURCES: Sources included a Medline search and reference lists of relevant articles and standard textbooks. METHODS OF STUDY SELECTION: Articles were identified that contained scientific data on the pathophysiology of anal incontinence, the influence of vaginal delivery on the continence mechanism, and ther…

Management of Breast Cancer Survivors by Gynecologists.

Breast cancer patients commonly present to their OBGYN during the process of diagnosis and treatment of breast cancer both for specific gynecologic needs and for primary care follow up. These patients require counseling on contraception, hormone use, and fertility at diagnosis. During treatment and survivorship, patients will face a variety of side effects from treatments leading to vasomotor symptoms, vulvovaginal discomfort, sexual dysfunction, osteoporosis, and vaginal bleeding. Obstetrician-gynecologists' beliefs about assisted reproductive technologies.

OBJECTIVE: To characterize the prevalence of objections to assisted reproductive technologies among obstetrician-gynecologists. METHODS: We conducted a national probability sample mail survey of 1,800 practicing U.S. ob-gyns. Criterion variables were whether physicians object to artificial insemination or in vitro fertilization. We also presented seven patient scenarios and asked respondents if they would discourage use of assisted reproductive technologies and if they would help patients access such technologies. Covariates included physician demographic and religious characteristics. RESUL…

Human papillomavirus vaccines: an update for gynecologists.

Human papillomavirus (HPV) infections cause several common gynecologic problems. Oncogenic HPV infections are responsible for abnormal cervical cytology, cervical dysplasia, and cervical cancer. In 2006, a vaccine was introduced to prevent these common problems in women's health. The currently available HPV vaccine is quadrivalent, with HPV types 6, 11, 16, and 18. Recently published clinical trials have consistently shown efficacy approaching 100%. Current clinical recommendations are for universal vaccination of adolescent and young adult women. This review will discuss the natural history a
