Surgical treatment of iatrogenic hallux varus.

Iatrogenic hallux varus is a dreaded complication of hallux valgus surgery, consisting in 1st-ray deformity in the form of medial malalignment of the 1st phalanx with respect to the metatarsal axis. Such over-correction results from imbalance between excessive medial capsule retraction or tensioning and excessive lateral laxity or soft-tissue release. There may be loss of medial stability of bone origin due to excessive "exostosectomy" or excessive intermetatarsal angle closure. Following excessive lateral release, the imbalance gradually induces a varus deformity of the 1st phalanx due to tra

Surgical treatment of congenital hallux varus.

BACKGROUND: The purpose of this study was to report outcomes of congenital hallux varus deformity after surgical treatment. METHODS: We evaluated ten feet of eight patients with a congenital hallux varus deformity, including four feet combined with a longitudinal epiphyseal bracket (LEB). There were seven male patients and one female patient with a mean age of 33 months (range, 7 to 103 months) at the time of surgery. Two patients were bilaterally involved. The mean duration of follow-up was 5.9 years (range, 2.3 to 13.8 years). Clinical outcomes were assessed according to the criteria of Phe

Acquired hallux varus and clinical tolerability.

This retrospective study was undertaken to determine the long-term clinical problems, residual disability, and need for further surgery in patients with iatrogenic hallux varus. Between 1975 and 1985, in 16 (19 feet) of 83 patients who underwent foot surgery for hallux valgus or metatarsus primus varus, hallux varus deformity was noted at 1-year follow-up on dorsoplantar roentgenograms obtained with the patients bearing weight. Thirteen of those patients (16 feet) were reexamined at an average of 18.3 years (220 months) after surgery. The average hallux varus deformity in this group was 10.1 d

[Operative treatment of hallux varus duplex].

The double great toe is a very rare deformity of the foot. This paper describes two cases with such a bilateral anomaly. In both cases the accessory toe was found lateral and not as described in literature medial. Surgery was done by excision of the rudimentary lateral great toe and correction of the varus position of the hallux by the use of a rotational skin flap.

Distal metatarsal osteotomy for hallux varus following surgery for hallux valgus.

We reviewed the outcome of distal chevron metatarsal osteotomy without tendon transfer in 19 consecutive patients (19 feet) with a hallux varus deformity following surgery for hallux valgus. All patients underwent distal chevron metatarsal osteotomy with medial displacement and a medial closing wedge osteotomy along with a medial capsular release. The mean hallux valgus angle improved from -11.6° pre-operatively to 4.7° postoperatively, the mean first-second intermetatarsal angle improved from -0.3° to 3.3° and the distal metatarsal articular angle from 9.5° to 2.3° and the first metatarsophal

The operative treatment of acquired hallux varus.

Forty two patients (45 feet) who underwent operative correction of acquired hallux varus were retrospectively evaluated. The causes of hallux varus included complications after hallux valgus surgery (36), trauma (3), rheumatoid arthritis (3), unknown (2), and osteoarthritis (1). The methods of surgical correction were determined by the underlying etiology, the age and activity level of the patient, and the site of primary deformity. Surgical corrections included medial soft tissue release alone (2), medial soft tissue release combined with tendon transfer (17), metatarsal osteotomy with (1) or

Hallux varus--a study of thirty cases.

Although both congenital and acquired hallux varus have been described, the deformity has been considered uncommon. Contrary to this belief, we noted that it is fairly common in the unshod population. Analysis of 30 cases shows that congenital hallux varus is the most common type encountered and it is of three types, namely: primary, secondary, and teratogenic. The congenital primary and secondary varieties are apparently easy to treat in infancy, but if left untreated, they progress with age in unshod persons. The more severe grades of hallux varus are associated with medial deviation of the

Congenital Hallux Varus: A Rare Forefoot Deformity.

Congenital hallux varus is a rare forefoot deformity presenting with a deviation of the great toe medially. There are various techniques for the treatment of congenital hallux varus described in the literature. We present a case of a 16-year-old boy with congenital hallux varus who underwent corrective surgery, which involved soft tissue and bony procedure for better functional and clinical outcomes.

Abductor hallucis tendon transfer for hallux varus.

Hallux varus deformity, which may be either congenital or acquired, can be very painful and disabling, in addition to having a poor cosmetic appearance. In this paper, the author discusses the acquired type. He believes the abductor hallucis tendon transfer to be the procedure of choice for correction of hallux varus deformity, and he discusses its use and results in three cases.

Acquired hallux varus: a new corrective procedure.

In hallux varus, which may be either congenital or acquired, the great toe deviates tibialward at the metatarsophalangeal joint. Dr. Wood discusses the acquired type and its causes and treatment. A new technique involving the surgical detachment from bone of all structures located superficial to the periosteum that surrounds the distal half of the shaft of the first metatarsal has been developed. It may be used in conjunction with the available soft tissue procedures before bone surgery for correction of the deformity, and it may make bone surgery unnecessary or alter the extent and scope of a

Iatrogenic Hallux Varus in a Patient with Rheumatoid Arthritis.

Background/Objectives: Iatrogenic hallux varus is a rare complication often arising after hallux valgus surgery, characterized by medial deviation of the hallux. This report presents the case of a 58-year-old female with iatrogenic hallux varus complicated by rheumatoid arthritis (RA). The objective is to highlight the challenges and outcomes of surgical treatment in RA patients with complex foot deformities. Methods: The patient presented with severe medial deviation of the hallux and claw positioning of the lesser toes, resulting in pain and functional limitations. Radiological analysis indi

Hallux Varus: An Underreported Presentation of Rheumatoid Arthritis.

UNLABELLED: The prevalence of hallux varus deformity in rheumatoid arthritis (RA) has been reported to be extremely rare. However, in South Asian Countries, where open-toed shoes are habitual footwear for the majority of people, we have found that hallux varus is a common deformity in patients with RA. This rate of occurrence is much more common than that in published hallux deformities in RA and reinforces the impact of footwear on the development of hallux deformities. In this report, we present 3 illustrative cases of hallux varus developed in patients with RA and review the etiology of hal

Hallux varus as complication of foot compartment syndrome.

Hallux varus can present as a congenital deformity or it can be acquired secondary to trauma, surgery, or neuromuscular disease. In the present report, we describe the presence of hallux varus as a sequela of calcaneal fracture with entrapment of the medial plantar nerve in the calcaneal tunnel and recommend that clinicians be wary of this when they clinically, and radiographically, evaluate patients after calcaneal fracture.
