We present the case of a term-healthy neonate who developed hematemesis while being admitted at the newborn nursery. The infant was found to have gastric ulcers with duodenal eosinophils. The condition was conservatively managed and the symptoms, including ulcers, resolved with time; however, the etiology of the ulcers is still unknown.

Upper gastrointestinal (UGI) endoscopy is an important tool in the evaluation of patients presenting with haematemesis. The objective of this study was to report the yield of this procedure in a Saudi Arabian population. We analysed the result UGI endoscopy in children and adolescents of 0-18 years of age who presented with haematemesis over a period of 10 years. In patients with hematemesis an emergency endoscopy has to be done as soon as possible. In ulcer disease emergency endoscopic treatment is indicated in ulcers with active bleeding or with visible vessels in the base of the ulcer. There is no significant difference in the efficacy between the various endoscopic methods of hemostasis. Epistaxis was diagnosed in 10 patients with apparent upper gastrointestinal bleeding, comprising a 0.55% incidence of hematemesis and melena in the population studied. A sufficient amount of blood can be swallowed during epistaxis to cause hematemesis and melena. Recent facial trauma or epistaxis, absence of a history of chronic dyspepsia, and impairment of blood coagulation emerge as strong indicators of the diagnosis and should lead to a careful examination of the nose and nasopharynx for the source of bleeding.

A case of hematemesis caused by jejunal gastric intussusception diagnosed by gastroscopy is presented. The acute form of gastric intussusception is a rare condition after gastric surgery, but it is important to be aware of the remote possibility because of a tremendously high mortality, if treatment is delayed more than 48 hours. The gastroscopic evaluation of the acute type may be extremely difficult, especially after gastric resection, the survey being very poor.

Differences in distribution of age, hemoglobin values, location and number of ulcers, activities of gastric secretion, associated conditions, treatment and prognoses between bleeding and non-bleeding gastric ulcer patients were studied. The records of 29 patients with Mallory-Weiss tears diagnosed by endoscopy are reviewed. Ingestion of alcohol was a chief cause of bleeding. Single tears were the largest in number and 68% of the lacerations were seen just distal to the esophago-cardial junction. Lacerations occurred more frequently on the right lateral wall of cardia than on the left lateral wall. Most of these lesions were mild and could be treated medically except for two cases which were surgically treated.

Five full term neonates presented with haematemeses on one of the first three days of life. All but one had been given vitamin K at birth. Two out of five had clotting abnormalities that suggested vitamin K deficiency. All had evidence of hiatus hernia and, on oesophagoscopy, had evidence of peptic oesophagitis. Bleeding is another complication of neonatal peptic oesophagitis.
