Hepatocellular carcinoma embolus to the common hepatic duct with no detectable primary hepatic tumor.

Obstruction of the common bile duct (CBD) by direct extension of tumor is occasionally found in patients with hepatic neoplasms. Tumor embolus to the CBD is very rare, however, when no primary hepatic tumor is found. The patient described herein was a 74-year-old man who presented with a new onset of jaundice, nausea, anorexia, and epigastric pain. There was a history of dark urine and clay-colored stools, but no fever. Endoscopic retrograde cholangiopancreatography (ERCP) showed partial obstruction of the common hepatic duct and dilated intrahepatic bile ducts. A computed tomography (CT) scan

Hepatic Actinomycosis in a Patient With Retained Common Bile Duct Stent.

Primary hepatic actinomycosis is rare, with less than 100 cases reported in English literature. Most of these cases are cryptogenic. We describe a 35-year-old woman who presented with a retained common bile duct stent for 6 years and found to have a hepatic mass with altered perfusion and enhancement, and minimal degree of washout on enhanced cross-sectional imaging. Fine-needle aspiration revealed presence of filamentous bacteria morphologically consistent with Actinomyces species. This report is a demonstration of a rare instance in which a retained biliary stent led to primary hepatic actin

Sonographic diameter of the common hepatic duct in sickle cell anemia.

The maximum diameter of the common hepatic duct was measured from the cholecystosonograms of 95 children and young adults with proven sickle cell anemia. Four mm was considered the upper limit of normal. Half the patients were male, half were female; 40% had gallstones and 25% had signs and symptoms of acute biliary diseases. The diameter of the common hepatic duct was normal in all but six patients. In five of these, the degree of enlargement was minor (1-3 mm). Three of the six patients with enlarged ducts had acute cholangitis without obstruction, two had clinically "silent" enlargements, a

Treatment of complex iatrogenic common hepatic duct injury following laparoscopic cholecystectomy.

We report the case of a woman in her 40s, with no significant medical history, submitted to a laparoscopic cholecystectomy in our institution for symptomatic gallbladder lithiasis. On postoperative day 4, she presented to our emergency room with severe abdominal pain and elevated inflammatory markers. Abdominal CT scan revealed a mass filled with liquid and air in the gallbladder fossa. Surgical exploration was performed revealing a major common hepatic duct iatrogenic injury, which was managed using suture over a T-tube. Three months after surgery, cholangiography showed a biliary stenosis, a

A rare cause of acute abdomen: spontaneous common hepatic duct perforation.

Spontaneous extrahepatic bile duct perforation is generally seen in infants. Although rarely seen in adults, it may be seen with fatal bile peritonitis. Therefore, for a patient presenting with acute abdominal symptoms, differential diagnosis must be made with radiological imaging such as abdominal ultrasonography or computed tomography, without any loss of time. In these imaging tests, in cases of gallstone disease together with perihepatic free fluid or choledocus which can not be monitored, it should be considered in the differential diagnosis. An emergency surgical intervention should be p

Cholecystostomy as an Exclusive Access to Remove Cystic, Common Hepatic, and Common Bile Duct Stones.

OBJECTIVE. The purpose of this article was to evaluate the feasibility and efficacy of percutaneous fluoroscopic-guided stone retrieval from the cystic duct and antegrade common bile duct (CBD) stone advancement into the duodenum exclusively through a cholecystostomy tube. MATERIALS AND METHODS. Twenty-one patients with acute cholecystitis and choledocholithiasis or an impacted cystic duct stone who underwent percutaneous cholecystostomy tube placement were retrospectively enrolled in this study. The patients had a contra-indication for cholecystectomy (17 patients because of comorbidities and

Mirizzi syndrome complicated by common hepatic duct fistula and left hepatic atrophy: a case report.

BACKGROUND: Mirizzi syndrome is a rare complication of chronic cholecystitis, usually caused by gallstones impacted in the cystic duct or the neck of the gallbladder. Mirizzi syndrome results in compression of the hepatic duct or fistula formation between the gallbladder and common bile duct (or hepatic duct, right hepatic duct, or even mutative right posterior hepatic duct). Clinical features include abdominal pain, fever, and obstructive jaundice. Severe inflammation and adhesion at Calot's triangle are potentially very dangerous for patients with Mirizzi syndrome undergoing cholecystectomy.

Hepatic ductoplasty and hepaticojejunostomy to treat narrow common hepatic duct during laparoscopic surgery for choledochal cyst.

We describe a novel technique for laparoscopic hepatic ductoplasty and hepaticojejunostomy in choledochal cyst with a narrow common hepatic duct. After complete cyst excision, a longitudinal incision was made in the anterior wall of the narrow common hepatic duct to enlarge the anastomotic opening. Next, the anterior wall was everted, then anchored to the hepatic hilum by suturing. Anchoring of the anterior wall provided a good field of view for creating the anastomosis, and the lumen of the anastomotic region was also secured. We believe that our technique is feasible and useful when the duct

Diverticulum of common hepatic duct leading to obstructive jaundice, a case report.

Choledochal cyst is a dilation of the intrahepatic and/or extrahepatic biliary tree. The pathogenesis is unknown and potentially is multifactorial. In 1977, Todani classified the cysts under five different types according to their morphology, number and distribution along the biliary tree. Presenting symptoms of Choledocal cysts which include upper abdominal pain, acute cholangitis and jaundice, although often they are clinically silent and discovered as an incidental finding. Biliary complications include cholangitis, biliary stones, pancreatitis, portal hypertension and cholangiocarcinoma. W

Extrahepatic biliary obstrution secondary to neuroendocrine tumor of the common hepatic duct.

INTRODUCTION: Primary neuroendocrine tumors (NET) of the extrahepatic biliary tree are a rare entity with less than 100 reported cases in the literature. PRESENTATION OF CASE: Here, we report a case of NET of the extrahepatic bile duct in a 64-year-old male patient presenting with painless jaundice, direct hyperbilirubinemia, and mildly elevated transaminases. Diagnostic workup with an ultrasound revealed dilation of the intrahepatic biliary ducts, without cholelithiasis or choledocholithiasis. Additional cross sectional imaging identified a stricture at the confluence of the common hepatic a

Congenital stricture of the common hepatic duct: an unusual case without jaundice.

A 3-yr 8-mo-old female was diagnosed as having a congenital stricture of the common hepatic duct. She demonstrated hepatomegaly with marked elevations of serum transaminases, alkaline phosphatase, and bile acids without clinical jaundice or hyperbilirubinemia. Liver biopsy suggested extrahepatic obstruction. Ultrasonography was nondiagnostic, but percutaneous cholangiography demonstrated blockage at the bifurcation of the hepatic ducts. A Roux-en-Y anastomosis of the jejunum to the common hepatic ducts relieved the obstruction and ameliorated clinical evidence of liver disease.

[Experience in laparoscopic cholecystectomy by exposing common hepatic duct using blunt dissection to prevent bile duct injury].

OBJECTIVE: To introduce an effective method for preventing bile duct injury in laparoscopic cholecystectomy. METHODS: From January 2003 to October 2005, laparoscopic cholecystectomy was performed on 629 patients, 335 males and 294 females, aged 46.3 (14 - 81). The common hepatic duct was exposed by blunt dissection routinely before transecting the cystic duct. RESULTS: No bile duct injury occurred. 7 cases were converted to open procedure, 5 cases for severe abdominal adhesion, one for internal fistula of bile duct to intestinal tract, and another 1 for gall bladder cancer. 2 patients had po

Parallel Configuration of the Cystic and Common Hepatic Ducts: a Surgical Odyssey.

INTRODUCTION: Laparoscopic cholecystectomy is among the most frequently performed surgical procedures worldwide; however, it carries a persistent risk of iatrogenic bile duct injury, particularly in the presence of anatomical variations. Misinterpretation of biliary anatomy remains a leading cause of serious complications, underscoring the importance of recognizing rare variants and adopting safe surgical strategies. CASE REPORT: We report the case of a 78-year-old patient undergoing elective laparoscopic cholecystectomy for recurrent cholecystitis and cholangitis. Given the history of repeat
