Autoimmune liver disease encompasses several disorders (Table 1). Autoimmune hepatitis (AIH) affects mainly women and is characterized histologically by a portal tract mononuclear cell infiltrate disrupting the limiting plate and invading the parenchyma ("interface hepatitis") and serologically by the presence of autoantibodies and increased levels of immunoglobulin G (IgG), in the absence of a known cause. AIH responds to immunosuppressive treatment. It can present insidiously or as an acute hepatitis. The previously accepted requirement of 6-month duration of symptoms before diagnosis can be

OBJECTIVE: To review an update of autoimmune hepatitis in children, based on classification, diagnostic and therapeutic criteria. METHODS: Articles on the theme were selected and updated, and the author's experience at the Liver Unit, Children's Hospital, School of Medicine, Universidade de São Paulo, was considered as well. RESULTS AND CONCLUSIONS: Autoimmune hepatitis has clinical features suggestive of acute viral hepatitis; however, other clinical features, laboratory tests, endoscopic and histological findings pointed to chronic liver disease, and the majority of the cases evolved to hepa

Autoimmune hepatitis (AIH) is a rare disease, characterized by female predominance, hypergammaglobulinemia, autoantibodies, association with HLA DR3 and HLA DR4 and a good response to immunosuppression. Different subtypes of AIH may be distinguished, based on differences in the autoantibody patterns. AIH type 1 is characterized by anti-nuclear (ANA) and/or anti-smooth muscular (SMA) autoantibodies. AIH type 2 is characterized by liver/kidney microsomal autoantibodies (LKM). AIH type 3 may be distinguished by autoantibodies to soluble liver proteins (SLA) or the liver pancreas antigen (LP). AIH

The morphology of autoimmune hepatitis is characterized by portal-periportal predilection of necroinflammatory lesions. In comparison to the viral type of hepatitis severe piece-meal-necroses, the collapse of periportal parenchyma, and to a higher degree acinar transformation of hepatocytes are more prominent. The autoimmune hepatitis may start with acute disease displaying unusual clinical und histopathologic features. The postinfantile giant cell hepatitis seems to constitute a variant of autoimmune hepatitis. Autoimmune hepatitis has been reproduced in animal models and it could be demonstr

Autoimmune hepatitis can present as either acute or chronic disease in children. Clinical and laboratory features, including association with extrahepatic autoimmune syndromes and prompt response to immunosuppressive treatment, circulating autoantibodies and hypergammaglobulinemia, suggest an immune etiology. However, the disease mechanism remains uncertain. Different types of autoimmune hepatitis are defined on the basis of which autoantibodies are present: anti-smooth muscle (type 1), anti-liver/kidney microsomal (type 2), or anti-soluble liver antigen (type 3). Diseases which may be clinica

Autoimmune hepatitis is a liver disorder that typically affects young women. This comprehensive review addresses the condition's etiology, clinical features, laboratory tests, autoantibodies, pathology, immunohistochemistry, genetics, and other factors of interest to clinicians. Autoimmune cholangitis is also discussed.

Autoimmune hepatitis (AIH) is a severe liver disease that affects children and adults worldwide. The diagnosis of AIH relies on increased serum transaminase and immunoglobulin G levels, presence of autoantibodies and interface hepatitis on liver histology. AIH arises in genetically predisposed individuals when a trigger, such as exposure to a virus, leads to a T cell-mediated autoimmune response directed against liver autoantigens; this immune response is permitted by inadequate regulatory immune control leading to a loss of tolerance. AIH responds favourably to immunosuppressive treatment, wh

Autoimmune hepatitis is a chronic liver disease characterized by a necroinflammatory activity of the portal tract resulting from an immune response against hepatocytes. It is relatively rare and its cause is unknown. There are three types (I, II and III) associated with the presence of some autoantibodies which are not always specific. The diagnosis is based on clinical, biochemical, immunological and histological features. The preferred treatment is a combination regimen of predniso(lo)ne and azathioprine (budezonide and aziathioprine in when cirrhosis is absent). The corticosteroid must be s

Autoimmune hepatitis is a chronic liver disease putatively caused by loss of tolerance to hepatocyte-specific autoantigens. It is characterized by female predilection, elevated aminotransferase levels, autoantibodies, increased γ-globulin or IgG levels and biopsy evidence of interface hepatitis. It is currently divided into types 1 and 2, based on expression of autoantibodies. Autoantigenic epitopes have been identified only for the less frequent type 2. Although autoimmune hepatitis occurs in childhood, this review focuses on disease in adults. In the absence of pathognomonic biomarkers, diag

Autoimmune hepatitis is a systemic disease, difficult to diagnose due the high variability of the clinical presentation and some non specific histological features. The recent identification of additional autoantibodies used as serological markers, as well as simplified diagnostic criteria should help the primary care physician to advance with the diagnostic process. These progresses are crucial as undiagnosed and therefore untreated autoimmune hepatitis has a poor prognosis, whereas immunosuppressive therapy leads to remission in a majority of cases.
