BACKGROUND: An inguinal hernia is one of the most common paediatric surgical presentations in a primary care setting. Hernias can present in multiple ways, ranging from an emergency such as a strangulated hernia to a less urgent reducible hernia.
OBJECTIVE: The aim of this article is to aid in appropriate diagnosis and management of hernias in children. The article also provides useful tips for hernia reduction that are especially beneficial in the primary care setting and assist with the identification of hernias that require urgent referral.

In the past decade hernia surgery has been challenged by two new technologies: by laparoscopy, which has attempted to change the traditional open operative techniques, and by prosthetic mesh, which has achieved much lower recurrence rates. The demand by health care providers for increasingly efficient and cost-effective surgery has resulted in modifications to pathways of care to encourage more widespread adoption of day case, outpatient surgery, and local anaesthesia. In addition, the UK National Institute for Clinical Excellence has recommended strategies for bilateral and recurrent hernias.

Benign neoplasms originating from the neuronal sheath are usually found along the distribution of the cranial nerves and in the flexor surfaces of the upper and lower extremities. We herein present a case of a 65-year-old man with a schwannoma located in the inguinal canal, which presented as an irreducible hernia. This is the first reported case in the English literature of a schwannoma with such a presentation.

Abdominal wall weakness as it is frequently seen in African tropical zones produces often rare clinical case. We diagnosed an aggressive Non-Hodgkin-Lymphoma (diffuse large B-cell lymphoma, DLBCL) in a patient with a fast growing inguinal mass. Only one third of all suspicious inguinal lymph nodes that are investigated histologically are found to be malignant lymphoma. If so, an accurate histopathological evaluation of sufficient neoplastic tissue is critical. Prognosis in DLBCL is highly associated with the IPI score. Overall survival has been improved by the CHOP-R regimen established in 2002, with a disease-free survival exceeding 50%.

Adult Granulosa-Cell Tumor (GCT) is an uncommon ovarian malignancy. The clinical course is characterized by slow growth with late recurrence. A variety of clinical and pathologic parameters have previously been evaluated for prognosis with inconclusive results. The aim of this study was to report a case of a granulosa-cell tumor of the ovary that recurred 25 years after first surgery. The clinical course and histopathology of the case were reviewed and a literature search for other reported cases was performed.

We present two cases of inguinoscrotal hernia that contains stomach, both associated with a chronic obstructive pulmonary disease; one of them is shown with a intermittent gastric obstruction syndrome, the other being a radiological finding in a patient with dispeptic symptoms. The first one died of a respiratory infection, before operation, and the second refused operation because of his scarce symptomatology. A review of the literature shows 60 cases reported including our cases.

OBJECTIVE: To present the outcome of 123 groin hernias treated laparoscopically between July 1991 and October 1993, and to discuss the advantages of preperitoneal hernia repair with mesh.
DESIGN: Data for each patient were collected prospectively on a predetermined standard form. Every patient was assessed by an independent, non-treating surgeon.
PATIENTS AND SETTING: Ninety-seven patients underwent laparoscopic hernia repair by two surgeons between July 1991 and October 1993. The patients were treated in several private hospitals and one public hospital in Sydney, NSW.

Flexible sigmoidoscopy has become part of routine preoperative workup for patients over the age of fifty who present with an inguinal hernia. A recent experience with two patients with a left inguinal hernia allowing sigmoid colon to herniate into the scrotum bring awareness of the possibility for an increased risk of perforation of the sigmoid colon during flexible sigmoidoscopy, or possible iatrogenic incarceration of the hernia. These cases are presented so this clinical entity can be recognized and the complications prevented.
