Hiccup is a sudden contraction of the inspiratory muscles, followed by an abrupt closure of the glottis, thus producing a characteristic sound. In the literature, some drugs have been reported to induce hiccup. We discuss three case reports after administration of benzodiazepine to healthy young subjects during two clinical trials. In the first study (a bioequivalence trial of two forms of lormetazepam, tablets and oral solution), 12 subjects were included in an open controlled crossover study with two periods separated by a washout of 7 days. Two subjects presented with hiccup after administr…

PURPOSE OF REVIEW: Hiccups are a common problem that crosses multiple disciplines including neurology, gastroenterology and pulmonology, and primary care. There are no formal guidelines to the treatment of intractable hiccups and treatment is based on experience and anecdotal evidence often relying on older medications. We have reviewed the relevant literature with an emphasis on the last five years or so in management of intractable hiccups.
RECENT FINDINGS: The production of hiccups is a complex mechanism which involves multiple neurotransmitters and anatomical structure within the central …

Hiccoughs are an involuntary spasmodic and coordinated contractions of the inspiratory muscles associated with a delayed and sudden closure of the glottis which is responsible for the characteristic noise. The pathophysiology of hiccoughs have not been elucidated. There have only been a few observations describing the sequence of inspiratory muscle activation and these suggest that the control of hiccoughs is within the central nervous system. The clinical circumstances associated with acute benign hiccoughs are both numerous and disparate. In these cases, when they stop spontaneously or as a

PURPOSE OF REVIEW: Nearly 4000 patients will be admitted to hospital in the US this year for hiccups. Hiccups are controlled by a complex reflex arc between peripheral receptors and the brainstem. Any disruption along this pathway may produce hiccups. Typically, hiccups resolve spontaneously but in certain pathologies symptoms may persist. Persistent hiccups may be considered a sign of underlying pathology. The most common cause involves GERD.
RECENT FINDINGS: Based on etiologies, studies have shown that first-line therapy should use a proton pump inhibitor (PPI) and involve appropriate gastr…

We present the case of a patient who presented with persistent hiccups induced by a hair irritating the tympanic membrane and activating the vagally mediated hiccup neural reflex arc. Commonly identified etiologies and treatments of this condition are then reviewed.

Hiccups (singultus) is often a benign, common and self- limited condition. A case of profound electrolyte disturbances presenting with chief complaint of hiccups is presented in which chlorpromazine was not administered and could have been problematic had it been given. For those who present to the ED with chief complaint of hiccups, it is critical to consider a life threatening etiology as the cause of their symptom. This case presents a potential pitfall in the management of singultus.

Four patients with brainstem lesions presented with intractable hiccup and mild to moderate neurological signs. Two of the patients had been initially diagnosed as having a psychogenic cause for their hiccup. Magnetic resonance imaging (MRI) demonstrated brainstem infarction in one case, tuberculoma at the junction of the medulla oblongata and the cervical spinal cord in two, and a vermian tuberculoma compressing the brainstem in one. The brainstem infarct and one of the medullary tuberculoma were not detected on high resolution enhanced computed tomography. The 3 patients with CNS tuberculoma…

Persistent hiccups in the perioperative setting, although uncommon, can substantially impact a patient's wellbeing and recovery after surgery. We present a case of a 51-year-old male who developed persistent hiccups during total knee replacement surgery under neuraxial anaesthesia. The hiccups persisted postoperatively, leading to multidisciplinary interventions, including the introduction of central-acting muscle relaxant, baclofen. This case highlights the complexity of managing persistent hiccups in the perioperative period, where both aetiology and treatment are multifactorial, emphasising…

A three-year old girl, who had severe cerebral palsy, severe mental retardation, and symptomatic epilepsy, was suffering from intractable hiccup that lasted for more than an hour since she was 2 years and 10 months old when she undertook tracheotomy and laryngotracheal separation. Sometimes this intractable hiccup was followed by respiratory arrest. Although the hiccup was resistant to conventional medications, the nose drops of small amount of vinegar showed a favorable effect on the hiccup. Intranasal vinegar might stimulate dorsal wall of nasopharynx where the pharyngeal branch of the gloss…

The word "hiccup" refers to an involuntary, spasmodic contraction of the diaphragm that is followed by the abrupt closure of the glottis to produce the characteristic sound. Among the many documented causes of this occurrence are those due to neurogenic dysfunction. In the past few decades, lidocaine has been shown to be effective in treating a variety of disorders thought to involve neuropathic mechanisms, including seizures, chronic pain, and arrhythmias. We describe a postsurgical patient in whom two successive intravenous infusions of lidocaine, 1.5 mg/kg followed the next day by 0.75 mg/k…

In cats, rats and rabbits under light barbiturate anesthesia, glottal activity during spontaneous or evoked hiccups was studied. By measuring laryngeal pressure or resistance to flow to a constant current of air, and controlling several sources of error, it was found that glottal closure is an important component of this complex phenomenon. Adductor activity of the vocal cords seemed more labile to the anesthetic action and, under this condition, showed a weaker force of contraction than the abductor one. It is suggested that this can be due to the correlation of glottal adduction with expirat…

Hiccups are most often benign and of short duration. However, they may also be persistent (superior to 48h) or even refractory (superior to 1 month). In such cases, they markedly alter the quality of life and can lead to severe impairment of health. We here review hiccups pathophysiology, etiologies, work up and treatment. We suggest that hiccups should be considered as a non-epileptic myoclonic movement disorder.

Hiccups are common; however, hiccups caused by sarcoidosis have rarely been reported. An unusual case involving a patient with persistent hiccups possibly caused by hilar/mediastinal lymph node enlargement due to sarcoidosis prompted us to perform a literature search. Eight case reports relating hiccups to sarcoidosis were found and in only one case were the hiccups thought to be due to thoracic lymphadenopathy (LAD). Most cases were attributed to involvement of the central nervous system (CNS) with sarcoidosis. Management of hiccups in general is unclear and only chlorpromazine is approved by…
