Periprosthetic Hip Fracture due to Ballistic Injuries.

Periprosthetic hip fractures are a common cause for revision. To date, however, there are no reports of periprosthetic fractures (PFs) in total hip arthroplasty caused by ballistic injury (BI). There are no current recommendations on the management of this pathology in the literature. The objective of this paper is to report on 2 successfully treated cases of PF caused by BIs. Additionally, a brief review of the literature regarding open fracture secondary to BIs is carried out. What we consider appropriate initial and definitive management for these patients is outlined. According to our clin

Imaging of Overuse Injuries of the Hip.

Overuse injuries of the hip are common, and clinical diagnosis may be difficult because of overlapping and nonspecific clinical symptoms. Imaging can play an essential role in guiding diagnosis and management. Femoroacetabular joint structural abnormalities result in various conditions that can predispose patients to early development of osteoarthritis. Repetitive stress on the skeletally immature hip can result in apophyseal injuries. Notable nonosseous overuse hip pathologies include athletic pubalgia, trochanteric bursitis, and injuries involving the iliopsoas myotendinous unit. Timely diag

US Assessment of Sports-related Hip Injuries.

Traumatic and overuse hip injuries occur frequently in amateur and professional athletes. After clinical assessment, imaging plays an important role in diagnosis and in defining care management of these injuries. Ultrasonography (US) is being increasingly used in assessment of hip injuries because of the wide availability of US machines, the lower cost, and the unique real-time imaging capability, which allows both static and dynamic evaluation as well as guidance of point-of-care interventions such as fluid aspiration and steroid injection. Accurate diagnosis of hip injuries is often challeng

Labral injuries of the hip in rowers.

BACKGROUND: Injuries of the hip in the adolescent and young adult athlete are receiving more attention with advances in the understanding of femoroacetabular impingement (FAI), labral pathology, and hip arthroscopy. Labral tears have not been well characterized in rowers. QUESTIONS/PURPOSES: The purposes of this study were (1) to describe the clinical presentation of labral pathology in rowers; (2) to describe the MRI and radiographic findings of labral pathology in rowers; and (3) to determine the likelihood that a rower with labral injury, treated arthroscopically, will return to sport. [Overuse injuries of the hip in adolescents].

Painful femoro-acetabular impingement symptoms localized in the groin in flexion, adduction and internal rotation can be explained either by a primary disease of the labrum often post-traumatic, and more frequently as part of femoro-acetabular primary or secondary dysmorphia. The kinematic of the normal hip joint depends of peri-acetabular structures, geometry of joints and possible pathologies that could contribute to modify either the geometry or the proprioceptive function. By combining and analyzing these parameters it is possible to describe a joint concept of centricity, an essential par

[Arterial vascular injuries in hip prosthesis implantation].

Implantation of total endoprosthesis (TEP) is an established method for the operative therapy of acute and chronic hip dysfunction. Beside septic complications iatrogenic injuries of the vascular system regularly occur. Immediate and adequate therapy is necessary in order to prevent loss of an extremity or even patients death. We present three patients with major vascular injuries during hip joint operations. In two patients was the external iliac artery and in one the femoral common artery injured. The cases were managed with PTFE- or Dacron-interposition. The problem of vascular injuries ass

Posttraumatic Hip Osteoarthritis After Pelvic Ring Injuries.

OBJECTIVES: While rates of posttraumatic osteoarthritis after acetabulum fracture have been thoroughly studied, there has been less emphasis on hip osteoarthritis after pelvic ring injuries. The objective of this study was to determine the frequency of posttraumatic hip osteoarthritis in pelvic ring injury patients. It was hypothesized that more severe pelvic ring injuries would be associated with greater rates of posttraumatic hip osteoarthritis. DESIGN: Retrospective Cohort. SETTING: Urban/Suburban Academic Level I Trauma Center. PATIENT SELECTION CRITERIA: Subjects were identified using

Vascular Injuries During Hip and Knee Replacement.

Vascular injuries associated with hip and knee arthroplasty are rare but can result in devastating outcomes for the patient. A sound knowledge of vascular anatomy, potential mechanisms of injury, and diagnosis and management of vascular injuries are vital to an arthroplasty surgeon. Identifying high-risk patients and procedures allows careful preoperative planning, which combined with meticulous intraoperative technique, may help avoid vascular complications. When vascular injuries do occur, early recognition and intervention are critical to an improved outcome.

Avulsion Injuries of the Pelvis and Hip.

BACKGROUND: Avulsion injuries of the pelvis and hip region are typical injuries in adolescent athletes but can be found in adults as well. Typical sites for avulsion injuries include the origin/insertion of tendons and ligaments. Among adolescents, the not yet ossified apophysis is also frequently involved. The pelvis and hip are especially prone to such injuries due to their complex musculotendinous anatomy. Clinical history and physical examination in combination with the recognition of typical imaging findings are essential for correct diagnosis of these mostly acute, but sometimes also chr

Sonography of sports injuries of the hip.

CONTEXT: Sports-related injuries of the hip are a common complaint of both competitive and recreational athletes of all ages. The anatomic and biomechanical complexity of the hip region often cause diagnostic uncertainty for the clinicians evaluating these injuries. Therefore, obtaining additional diagnostic information is often crucial for providing injured athletes with a prompt and accurate diagnosis so they can return to activity as soon as possible. Musculoskeletal ultrasound is becoming increasingly important in evaluating and treating sports-related injuries of the hip. EVIDENCE ACQUIS

Imaging of traumatic injuries to the hip.

Traumatic injuries of the hip are an increasingly common cause of morbidity and mortality. These injuries can be grouped into fairly discrete patterns including femoral head fractures and hip dislocations, femoral neck fractures, greater trochanteric fractures, intertrochanteric fractures, subtrochanteric fractures, and soft tissue injuries. For each of these entities, specific features provide helpful diagnostic, prognostic, and therapeutic information. Femoral head fractures and hip dislocations commonly occur in combination. Fractures of the femoral head confer an increased risk of avascula

An overview of hip injuries in running.

Running has steadily gained in worldwide popularity and is the primary exercise modality for many individuals of all ages. Its low cost, versatility, convenience and related health benefits appeal to men and women of broad cultural, ethnic and economic backgrounds. With more children and adults participating in recreational and competitive running, the incidence of injuries has steadily increased. Most running-related injuries affecting the lower extremities are due to preventable training errors, and some may necessitate medical evaluation or a significant reduction in training. Hip injuries

[Severe vascular injuries following total hip replacement].

Severe vascular injury following total hip replacement (THR) is very rare. It has an a incidence of 0.25% and poses a serious threat to the involved limb and the patient's life. Vascular structures affected include the external iliac artery and vein, femoral artery and vein. In 721 cases of THR 3 had vascular injuries. The first case sustained the injury of the external iliac vessel which was cut by the sharp edge of bone cement while removing the displaced prosthesis. The second had laceration of the iliac vein while a pneumatic drill was used in the anterion superior acetabulum, and the thir

Injuries about the hip in the adolescent athlete.

Athletic injuries in or around the hip in the adolescent athlete encompass possible causes such as a single, traumatic event to those of repetitive microtrauma. The injuries may involve the bone or the soft tissues, with former involving the epiphysis, apophysis, metaphysis, or diaphysis, whereas the latter includes muscles and tendons. With the improvements in surgical technique and instrumentation for hip arthroscopy and the development of magnetic resonance arthrography, clinicians have been able to diagnose and treat labral tears, hip instability, snapping hip, loose bodies, chondral injur

MR imaging of overuse injuries of the hip.

The aim of this article is to emphasize the importance of MR imaging in the evaluation of chronic hip pain and overuse injuries. Image interpretation of the hip can be difficult because of the complex anatomy and the varied pathology that athletes can present with, such as labral and cartilaginous injuries, surrounding soft tissue derangement involving muscles or tendons, and osseous abnormalities. The differential diagnosis in adults is diverse and includes such common entities as stress fracture, avulsive injuries, snapping-hip syndrome, iliopsoas bursitis, femoroacetabular impingement syndr

Hip and pelvic injuries in the young athlete.

Hip and pelvic injuries are relatively rare in the young athlete. Contusions and musculotendinous sprains are the most common injuries about the hip and pelvis. Apophyseal avulsion fractures and stress fractures are the most frequently encountered skeletal injuries. Each of these entities can be successfully treated with guided physical therapy following conservative management with rest, anti-inflammatory medications, and ice massage until the patient is pain free. Epiphyseal, diaphyseal, or pathologic fractures are rare entities that are secondary to violent trauma. These injuries are severe
