Eight patients with severe hydrochloric acid injury of the esophagus and stomach had early esophagogastrectomy. Four patients survived and 6 to 8 weeks later underwent successful substernal colon replacement. The value of early diagnosis and subsequent aggressive surgical management of patients who have ingested hydrochloric acid is stressed.

Suicidal ingestion of undiluted hydrochloric acid is a rare but live-threatening event. We report on the dramatic and fulminant course of a 61-year-old woman who drank 200 ml of 30-33% hydrochloric acid. On admission to our intensive care unit she was alert and oriented with stable vital signs. Gastroscopy demonstrated complete necrosis from the hypopharynx to the pylorus, which was flushed continuously via drainage catheters. At that time point perforation was not evident. Shortly thereafter she developed an acute abdomen and a tension pneumothorax which was decompressed. CT scan demonstrated…

In this work we investigated the behaviour of pure HgS during extraction with dilute HCl to establish its extractability in 1 and 6 M HCl at the concentration level close to those occurring in natural sediments and soils. We found that neither cinnabar nor metacinnabar were soluble in 1 M HCl, whereas both were partially extracted by 6 M HCl. Metacinnabar precipitated in the laboratory was most prone to dissolution in 6 M HCl (up to 90%), followed by crystalline (commercial) metacinnabar (up to 70%) and cinnabar (up to 15%). Solubility of HgS in 6 M HCl was found to be dependent on its concent

A modification of cholangiomanometry in which the papilla of Vater was stimulated by hydrochloric acid was devised as a means to evaluate the papillary function. Manometric tracings could be classified into four patterns according to the mode of response to hydrochloric acid of the papilla. Pattern I was the type in which a group of regular, high-amplitude spikes appeared within 60 seconds after the onset of perfusion. This represented a normally functioning papilla. Pattern II was characterized by rhythmic occurrence of low-amplitude waves. This represented a reversible papillitis as evidence…

The progression of hydrofluoric acid corrosion on the hands, which cannot be arrested by other means and leads to crippling or amputation, can be stopped by one single or several repeated intra-arterial perfusions of 10 ml 20% calcium-gluconate solution in 40 ml normal saline over 4 hours as close to the defect as possible via the brachial artery or one of the forearm arteries. In 4 patients, with corrosion of a severity judged to be between first and third degree, spontaneous healing resulted after this treatment. Based on our experience we advise the use of intra-arterial perfusion and show

A container truck leaked 800 L (200 gallons) of hydrochloric acid (HCl) near a mobile home park in Louisiana in August 1993. The investigating officer and residents became acutely ill with burning and tearing eyes, burning throats, headache, chest pain, shortness of breath, and flu-like complaints. Twenty months later, 45 exposed adult subjects and 56 age-matched referents underwent neurobehavioral testing, including balance, reaction time, blink-reflex latency, and spirometry. They also completed health questionnaires and a profile of mood states. The exposed subjects differed significantly f…

During a seven-year period, 15 patients admitted to an intensive care unit with a metabolic alkalosis resistant to saline and potassium chloride infusions and with an arterial pH of greater than 7.44 and arterial carbon dioxide pressure (PaCO2) of greater than 50 mm Hg, while breathing spontaneously, were given hydrochloric acid at 200 mmol/24 h through a central venous line until the arterial pH had decreased to less than 7.36 or PaCO2 had decreased to less than 40 mm Hg. Five patients without respiratory failure had a significant decrease in PaCO2, pH, bicarbonate Ion (HCO3-), and base exces…

In a new method for peripheral intravenous infusion of hydrochloric acid the HCl is buffered in an aminoacid solution and infused with a fat emulsion. The aminoacids and the fat emulsions are stable in the presence of HCl, and the transfusion set is resistant to the chemical actin of 0.15 mol/l HCl. Two case-reports show that HCl can be administered safely through a peripheral vein.

The effect of the thiamine deficiency on the H+ secretion in an isolated gastric mucosa and also on the functional properties of the mitochondria was investigated. The thiamine deficiency was called forth through introduction of oxythiamine or by exclusion of thiamine from the ration. It is shown that the thiamine deficiency in an isolated gastric mucosa of the frog produced inhibition of the H+ secretion and in the gastric mitochondria of rats--a higher activity of succinate-dehydrogenase and a diminution of the exogenous cytochrome oxidation rate.

BACKGROUND AND OBJECTIVE: Pulmonary aspiration of gastric acid is a serious complication during anaesthesia and may cause aspiration pneumonitis and adult respiratory distress syndrome. The development of pulmonary hypertension may aggravate the initial course of the aspiration pneumonitis. The authors hypothesized that acid aspiration induces an acute increase in right ventricular pressure in the rat heart. Additionally, it was hypothesized as a secondary study that endothelin levels would be increased in this rat model.
METHODS: Male Sprague Dawley rats, anaesthetized with sevoflurane, unde

Ingestion of sodium bicarbonate has been implicated as one of the proximate causes of spontaneous gastric rupture. However, the volume and rate of gas released from the reaction of ingested sodium bicarbonate and gastric acid has not been previously studied in detail. We, therefore, developed an in vitro method for measuring gas release after addition of sodium bicarbonate to a solution containing hydrochloric acid. From the results of our studies, we conclude that even though hydrochloric acid and sodium bicarbonate react instantaneously, the resulting gas production is slow, mainly because C…

Twenty-one patients with metabolic alkalosis were treated successfully with intravenous hydrochloric acid (HCl) buffered in an amino acid solution (TPN). No complications of HCl were seen. TPN was used to meet energy needs and provide a buffering effect through the interaction of HCl and amino acids. Buffered HCl therapy should be considered as the initial treatment in patients with metabolic alkalosis associated with congestive heart failure, renal failure, hepatic failure, cerebral edema, or refractory metabolic alkalosis.

Pelvic inflammatory disease (PID), which is one of the most problematic complications experienced by women with sexually transmitted disease … um, it often causes systemic sepsis leading to death and the PID was not consistently demonstrated. Hydrochloric acid (HCl) has been used to induce inflammation in the lungs and stomach but not tested for reproductive organs. In this study, we developed a PID model in mice by HCl and LPS sequential intracervical (i.c.) administration. The proinflammatory cytokines, …
