Newborns, and especially premature newborns, are at significant risk for developing hypotension in the first week or two after birth. The etiology of hypotension in the newborn may vary, but the very low birth weight and extremely low birth weight preterm infants are less likely to respond to conventional cardiovascular support when they develop hypotension. This article reviews the least conventional treatment using hydrocortisone for hypotension that is refractory to conventional volume replacement and/or vasopressor medications with the underlying assumption that sick and premature newborns…

The influence of i.v. infusion of hydrocortisone sodium succinate on basal biliary secretion and maximal hepatic excretion of bilirubin was investigated in pentobarbitone anesthetized rabbits. Hydrocortisone caused an increase in bile flow and electrolyte secretion in basal conditions. When it was infused after a maximal load of bilirubin, its choleretic effect disappeared and the maximal excretion of bilirubin was not modified. Our data suggest that the effect of hydrocortisone on bile flow and composition takes place at canalicular level and is not directly related to the secretion of bile s…

The serum concentration of all subclasses of IgG (gamma(2)A, gamma(2)B, and gamma(1)) as well as IgA and IgM were reduced in normal and low pathogen mice receiving hydrocortisone acetate. Turnover studies using (131)I-labeled gamma(2)A subclass of IgG demonstrated that high dose corticosteroids cause a significantly shortened survival (increased catabolic rate) which contributes to the observed hypogammaglobulinemia. This increase in fractional catabolism is not due to excess loss in the urine or stool but reflects an increase in endogenous catabolism.

BACKGROUND: The 2021 Surviving Sepsis Campaign Guidelines recommend the use of hydrocortisone in patients who remain hemodynamically unstable despite adequate fluid resuscitation and vasopressor therapy. Fludrocortisone has been used concomitantly with hydrocortisone in some studies without a clearly defined role or known clinical benefit. The purpose of this study was to assess the impact of fludrocortisone added to hydrocortisone on shock-free days for septic shock.
METHODS: A single-center, retrospective propensity score-weighted study was conducted to compare hydrocortisone versus hydroco…
