Characterization of tumor-associated microbiome in multifocal small intestinal neuroendocrine tumors (SI-NETs).

BACKGROUND: Small intestinal neuroendocrine tumors (SI-NETs) are among the most common neoplasms of the small bowel; however, the molecular mechanisms underlying their pathogenesis are largely unknown. The multifocal nature of SI-NETs, their putative distinct genomic origins, and their enrichment in the distal ileum led us to hypothesize that environmental factors, such as pathogenic organisms, might play a role in the development of these lesions. MATERIALS AND METHODS: To study the tumor-associated microbiome of multifocal SI-NETs and its potential role in pathogenesis, we used matched whol

[Ileal perforation secondary to intestinal endometriosis].

Endometriosis is defined as the presence of endometrial tissue outside the uterus. The bowel is not often affected. There are no specific clinical findings for intestinal endometriosis. It is typically asymptomatic, but sometimes can present with abdominal pain, diarrhoea, constipation or intestinal obstruction. Ileal perforation is a rare complication of intestinal endometriosis and only a few cases have been reported in the literature. Intestinal endometriosis can mimic many gastrointestinal diseases, such as irritable bowel syndrome, inflammatory bowel disease, infections and neoplasms. The

Small intestinal angiosarcoma leading to perforation and acute abdomen: a case report and review of the literature.

A 76-year-old man presented to the emergency room with abdominal pain and fatigue. The patient had a history of gastrointestinal bleeding. An abdominal computed tomographic scan showed collections of free air in the abdomen with obstruction of the distal small intestine and multiple masses in the liver. Laparotomy revealed a widespread malignant neoplasm in the abdomen, with multiple tumor nodules in the ileal wall, mesentery, and liver. The ileal wall was perforated within the area of one of the tumors. Pathologic examination of the lesion showed a neoplasm composed of solid sheets of spindle

Diagnosis and Management of Gastrointestinal Neuroendocrine Neoplasms.

The latest WHO classification cleanly divides gastrointestinal neuroendocrine neoplasms into neuroendocrine tumor (NET; well-differentiated, any grade) and neuroendocrine carcinoma (NEC; poorly differentiated, high-grade by definition), along with mixed neuroendocrine-non-neuroendocrine neoplasms. NECs are always aggressive, with multiple mutations; they are treated with chemotherapy. NETs have widely different presentations, behavior, and management depending on site of origin. Esophageal examples are vanishingly rare. Most gastric and appendiceal tumors are indolent, as are many colonic and

Intussusception as the initial manifestation of AIDS associated with primary Kaposi's sarcoma: a case report.

Kaposi's sarcoma (KS) is the most common AIDS-associated neoplasm. It involves the gastrointestinal tract, skin and lymph nodes with about equal frequency. However, most cases of gastrointestinal KS are clinically silent and found incidentally. We report the case of a 31-year-old homosexual man who developed intussusception in association with a primary ileal KS. He was admitted due to abdominal pain lasting 2 hours. Flat abdominal roentgenogram revealed small bowel ileus in the central abdomen. Abdominal sonography and computerized tomography revealed an intraluminal soft tissue mass in the s

Multiple ileal carcinoids and appendiceal endocrine carcinoma in association with Meckel's diverticulum. A histochemical and immunohistochemical study.

An unusual case of multicentric ileal carcinoids and appendiceal endocrine carcinoma in association with Meckel's diverticulum was studied, with special attention given to the histogenesis of these neoplasms. A total of six ileal carcinoids, the largest of which was located in the wall of Meckel's diverticulum, were macroscopically and microscopically confirmed to be multicentric and revealed no visceral metastases. The histochemical and immunohistochemical profiles of the ileal carcinoids and the appendiceal carcinoma differed considerably: the former resembled subepithelial neuroendocrine ce

Ileal Tubulo-Villous Adenoma Causing Small Bowel Obstruction in a Virgin Abdomen.

Small bowel obstructions (SBO) are a common surgical problem accounting for up half of all emergency laparotomies in the United States. SBO in the virgin abdomen (SBO-VA) presents surgeons with a unique predicament as historical teaching has mandated operative exploration in these situations due to their association with more sinister etiologies. More recent research has demonstrated that this may not be the case, with adhesive disease comprising the majority of SBO-VA. Small bowel neoplasms however comprise a considerable portion of SBO-VA. Small bowel tumors comprise around 0.5%-2% of all ga

Intermittent small bowel obstruction by jejunal enteroliths in a patient with a Crohn's disease stricture.

Small bowel obstruction is most frequently due to postoperative or inflammatory adhesions, intestinal neoplasms, hernias, or bezoars. Intermittent small bowel obstruction may be secondary to a Crohn's disease stricture or to chronic adhesive peritonitis. Enterolithiasis, usually associated with jejunal diverticulosis or with a Meckel diverticulum, should be considered in patients who have not previously undergone abdominal surgical procedures. X-ray evidence of stones in the abdominal field, outside the common sites, i.e. gallbladder, kidney, bladder, should suggest a diagnosis of enterolithia

Metastatic tumors to the urinary bladder: clinicopathologic study of 11 cases.

Secondary neoplasms of the urinary bladder are uncommon, with metastatic tumors being an even rarer event. The authors studied the clinicopathology of 11 cases of metastatic tumors to bladder, which were collected from their archives between 1995 and 2010. The most common metastases in this series were breast. Some unusual metastases, including several not being previously reported, were also presented, namely, ileal carcinoid tumor, ileal gastrointestinal stromal tumor, ovarian squamous carcinoma, pancreatic gastrinoma, and renal collecting duct carcinoma. Vast majority of these patients (10/

Small Bowel Adenocarcinoma of Ileal Ureter 40 Years After Ureteral Reconstructive Surgery.

Malignant neoplasm arising from ileal ureter used for ureteral reconstructive surgery is an exceedingly rare event. Ureteroileoplasty was being performed since the beginning of the last century, but it was described more extensively in literature during the 1950s.1,2 Recurrent urinary infections, chronic renal failure, urolithiasis, anastomotic stricture, metabolic acidosis, and chronic dilation of the graft had been described as late complications of ureteroileoplasty.3,4 Herein, we describe history, imaging and pathologic findings of a small bowel adenocarcinoma arising from ileal ureter in

Carcinoid syndrome. Radiology.

The carcinoid syndrome is usually produced by an ileal carcinoid tumour that has metastasized to the liver but can also be caused by similar tumours in many other organs. These neoplasms create a variety of radiological appearances which are often characteristic. In recent years percutaneous embolization of hepatic carcinoid metastases has been successfully employed as part of the treatment of the syndrome.

[Malignant carcinoid of the last ileal ansa. Report on 2 consecutive clinical cases].

Carcinoid tumors are very rare neoplasms, arising from enterochromaffin cells, classified in Apud system, exhibiting an intermediate malignancy, because of their long lasting clinical silence and low evolution to advanced stage. At the same time, these features unfortunately cause a high incidence of lymphatic and liver metastases, visible at first diagnostic approach, which are also determined by aspecific symptoms and signs, especially involving jejunal and ileal carcinoids, as the two cases described, and by very frequent absence, in current clinical practice, of pathognomonic carcinoid syn

Adenocarcinoid tumor, goblet cell type, arising in a ureteroileal conduit: a case report.

The adenocarcinoid tumor is a potentially malignant neoplasm usually arising in the appendix. We describe a 55-year-old woman with a ureteroileal conduit in place for 12 years who suffered obstruction of the ileal segment of the conduit, with recurrent pyelonephritis and abdominal pain. Pathological examination of a segmentally resected portion of the conduit revealed a submucosal proliferation of nests of argentaffin/argyrophil cells and large, mucin-distended goblet cells. The patient showed no evidence of metastasis and has done well to date following excision. A discussion of neoplasms ari

Ileal neuroendocrine tumors: clinicopathological features and associations with systemic inflammatory and nutritional markers.

BACKGROUND: Ileal neuroendocrine tumors (NETs) are well-differentiated neoplasms with relatively indolent histological features but an often aggressive biological behavior, characterized by early lymphatic and hematogenous dissemination. Increasing evidence suggests that systemic inflammatory and nutritional status may influence tumor progression; however, data regarding these parameters in ileal NETs remain limited. PATIENTS, MATERIALS AND METHODS: A retrospective observational study was conducted on 12 consecutive patients with histologically confirmed ileal NETs who underwent surgical trea

Impassable ureteral strictures: management with percutaneous ureteroneocystostomy.

Sixteen of 227 patients referred for percutaneous placement of a ureteral stent had impassable stenoses. Stenoses were benign (n = 8) or attributed to malignant retroperitoneal neoplasm (n = 8). Electrocautery was used to create a neotract between the stenosed ureter and the bladder or ileal loop. A double-J stent was placed after dilation of the tract by use of angioplasty. Neotracts were established and stents were placed in all patients. Complications (digestive tract fistulas) developed in two patients. This technique is safe if the electrode is placed close to the bladder or ileal loop. T

Primary Hodgkin lymphoma of the ileum.

Primary classical Hodgkin lymphomas of the gastrointestinal tract are rare lymphoproliferative neoplasms as against other well established mucosa-associated lymphoid tissue (MALT) lymphomas. The primary nature of these cases is established based on chest radiograph, computed axial tomography, positron emission tomography, peripheral blood and bone marrow biopsy examination. Here, we report a 79-year-old male patient, who presented to the emergency department with complaints of abdominal discomfort on right side and low grade fever. An ultrasonogram was performed which suggested of an intestina

An ulcerated ileal leiomyosarcoma identified by single-balloon enteroscopy despite nondiagnostic mucosal biopsies.

Gastrointestinal leiomyosarcoma is a rare malignant smooth-muscle neoplasm that may be difficult to diagnose using superficial mucosal biopsy. A 44-year-old woman presented with chronic intermittent abdominal pain and microcytic hypochromic anemia. Retrograde single-balloon enteroscopy identified an ulcerated, stenosing lesion in the distal ileum, whereas six forceps biopsy specimens showed only inflammation and granulation tissue. Segmental ileal resection was therefore performed because malignancy remained strongly suspected. Histological examination demonstrated a spindle-cell neoplasm with

Endoscopic submucosal dissection to treat ileal high-grade dysplasia after ileoanal anastomosis for familial adenomatous polyposis: report of a case.

Restorative proctocolectomy has become the most common surgical option for familial adenomatous polyposis patients, based on the premise that the technique yields good functional results and reduces colorectal cancer risk. However, several adenomas may develop in the pouch mucosa in later years, and even cancer (at the anastomosis or in the pouch mucosa) has been reported. Endoscopic submucosal dissection (ESD) would then be indicated, even if severe fibrosis extended into the submucosal layer. Endoscopic resection of an ileal tumor from the thin wall of the ileum carries a high-risk of perfor

A Systematic Approach to the Histologic Evaluation of Terminal Ileal Biopsies.

BackgroundIleal biopsies represent a critical diagnostic tool in gastroenterological practice, with particular relevance in the evaluation of Crohn's disease (CD). However, the ileal mucosa can harbor numerous nonneoplastic and neoplastic conditions that may clinically and histologically mimic CD. Accurate interpretation requires a nuanced understanding of normal ileal histology and its variants, alongside a structured diagnostic algorithm.ObjectivesTo delineate the spectrum of pathologic processes affecting the ileum and establish a systematic diagnostic approach, while highlighting potential
