Why breast cancer patients avoid communicating disease-related information to their dependent children: A qualitative study.

AIM AND OBJECTIVES: This qualitative study explores why breast cancer patients do not share disease-related information with their dependent children. BACKGROUND: 'Open' communication between breast cancer patients and their children is beneficial to both. However, some still try to avoid discussing their diagnosis and related information with their children. Some worries lie in communicating with dependent children, but the underlying cause remains unclear. DESIGN: Qualitative descriptive study. METHODS: Semi-structured interview was used in this study. Twenty breast cancer patients with c

Avoiding unwanted pregnancy--the role of communication, information and knowledge in the use of contraception among young Norwegian women.

Despite relatively easy access to contraceptives, a liberal attitude to pregnancies out of wedlock and a widespread family planning education in schools, too a high number of adolescent women in Oslo, the capital of Norway, become pregnant and resort to abortions. The aim of this study was to identify some new entry points to sexuality, contraceptive, abortion and post-abortion counselling. Using a qualitative approach, we interviewed 102 young women in Oslo who were seeking either an abortion or contraceptives. The study demonstrated that contraceptive awareness is good, but that there are di

A Dynamics-Informed Machine Learning Framework for Inhibitor Optimization Against Quickly Evolving Targets to Avoid Resistance.

Drug resistance often emerges through mutations that reshape protein conformational ensembles, allowing inhibitor potency to be altered without directly disrupting protein-ligand contacts. Consequently, the molecular mechanisms underlying these effects remain difficult to identify, hindering the design of inhibitors with durable potency against rapidly evolving drug targets. To address this challenge, we developed ROBUST, a physics-informed machine learning framework that integrates experimental measurements, molecular dynamics simulations, and interpretable statistical modeling to identify en

Be brave, BE-FIT! A pilot investigation of an ACT-informed exposure intervention to reduce exercise fear-avoidance in older adults.

Exercise sensitivity, fear of physical sensations of exertion, is particularly elevated in individuals with cardiovascular disease and can promote fear-avoidance of physical activity. We developed an ACT-informed exposure intervention to target exercise sensitivity, called Behavioral Exposure For Interoceptive Tolerance (BE-FIT). In this Stage I pilot trial, we developed and evaluated the feasibility, safety, and initial efficacy of BE-FIT in low active patients with elevated exercise sensitivity enrolled in outpatient cardiac rehabilitation. BE-FIT is a 6-session, manualized, program-adjuncti

Reactive thymic hyperplasia following chemotherapy for children with lymphoma: computed tomography may be able to provide valuable information to avoid over-treatment.

OBJECTIVES: To analyse the clinical and computed tomography (CT) findings related to reactive thymic hyperplasia in order to improve the recognition of this phenomenon and avert over-treatment. METHODS: Fifty-two children with pathologically proven lymphoma developed reactive thymic hyperplasia following chemotherapy, which was confirmed with long-term review and follow-up. The clinical and CT findings of these 52 children were retrospectively analysed. RESULTS: The median follow-up period for the whole study group was 32.9 months. Fifty-one children survived free of disease; 23 of these had

Functional Avoidance Liver 4π Stereotactic Body Radiation Therapy Informed by Quantitative Gadoxetic Acid Contrast-Enhanced Magnetic Resonance T1 Mapping.

PURPOSE: Recent studies have identified the T1 reduction rates (k1) from gadoxetic acid-enhanced magnetic resonance (MR) imaging as a biomarker for liver function. In this study, we validate k1 maps as a functional biomarker and develop 4π noncoplanar treatment plans using k1 maps to guide the optimization for liver functional avoidance stereotactic body radiation therapy (FA-SBRT). METHODS AND MATERIALS: One hundred six patients underwent precontrast and postcontrast T1 mapping MR. Mean k1 values extracted from liver mask excluding gross tumor volume (liver-GTV) were correlated with Child-Pu

Occupational Therapy Incorporating Cognitive Behavioral Therapy-Informed Components for Pain-Related Activity Avoidance Early After Olecranon Fracture Surgery: A Case Report.

Postoperative pain involves not only nociceptive components but also psychological reactions such as anxiety, pain catastrophizing, and fear-avoidance, which may delay functional recovery. Although cognitive behavioral therapy (CBT) is well established for chronic pain, evidence on the use of CBT-informed components by rehabilitation professionals in the early postoperative phase after upper-limb fracture surgery remains limited. This report describes occupational therapy (OT) incorporating CBT-informed components (hereafter, CBT-informed OT) for a patient who exhibited disproportionate pain-r

Veteran health information exchange volume and 30-day readmissions, avoidable hospitalizations, and in-hospital mortality: evidence from community and Veterans Health Administration direct care.

OBJECTIVE: To determine whether a greater volume of health information exchange (HIE) between care delivery systems improves patient outcomes and whether effects differ between community care and Veterans Health Administration (VHA) direct care settings. MATERIALS AND METHODS: We used electronic health record data for all VHA enrollees from January 2022 to December 2023 (3144 medical center-months; 2.4 million patients per month). HIE volume, the number of unique patient-days with completed exchanges, was our key explanatory variable. Primary outcomes were 30-day readmissions, avoidable hospi

Retromammary Schwannoma Mimicking a Breast Tumor in a Patient with Neurofibromatosis Type 1: A Case Report Demonstrating Surgical Avoidance Using Contrast-Enhanced Ultrasound-Informed Biopsy.

INTRODUCTION: Retromammary schwannomas are rare peripheral nerve sheath tumors that may clinically and radiologically mimic breast lesions, occasionally leading to unnecessary surgical intervention. CASE PRESENTATION: We report a young Japanese woman with previously undiagnosed neurofibromatosis type 1 who presented with breast pain. US revealed a mobile, well-circumscribed hypoechoic mass located in the retromammary space. Contrast-enhanced US (CEUS) using a second-generation contrast agent demonstrated a characteristic dynamic target sign, and MRI showed concordant findings. Vacuum-assisted

Cancer information overload: Discriminant validity and relationship to sun safe behaviors.

OBJECTIVE: Past research suggests a large number of adults feel overwhelmed by the amount of cancer information - a phenomenon labeled cancer information overload (CIO). The current study examines whether CIO is discriminant from other negative message perceptions (reactance, information avoidance) and related to sun safe behaviors. METHODS: U.S. adults (N = 2,219) completed survey questions assessing CIO, dispositional reactance, defensive/information avoidance, sun safe behavior, and knowledge. RESULTS: The results demonstrated that CIO was discriminant from dispositional reactance, inform…

The dilemma of sweet temptation: How sugar perception confusion in sweetened beverages shapes consumer avoidance behavior.

Despite widespread consumption of sugar-sweetened beverages, consumers face contradictory information from health authorities, marketing, and social media, yet limited research examines how this information conflict affects purchasing decisions. This study investigates how sugar perception confusion influences purchasing avoidance through ambivalent attitudes. Based on cognitive dissonance and information processing theories, we developed a cognitive-affective-behavioral model examining relationships among sugar perception confusion, ambivalent attitudes, and purchasing avoidance behaviors. The behavioral and neural architecture of observational active avoidance is shaped by demonstrator experience and observer sex.

Observational learning allows an individual to gain information about potential outcomes and is particularly advantageous when learning to actively avoid danger without risking harm to oneself. To assess observational learning of active avoidance, Observer rats witnessed Demonstrator rats undergo platform-mediated avoidance (PMA), in which rats learn to avoid a tone-signaled footshock by stepping onto a safe platform. We found that Observers' avoidance was correlated with a Novice, but not an Experienced, Demonstrator, suggesting that Novice and Experienced Demonstrators transmit different inf

The effects of acute tryptophan depletion on costly information sampling: impulsivity or aversive processing?

RATIONALE: The neurotransmitter serotonin (5-HT) has been implicated in both aversive processing and impulsivity. Reconciling these accounts, recent studies have demonstrated that 5-HT is important for punishment-induced behavioural inhibition. These studies focused on situations where actions lead directly to punishments. However, decision-making often involves making tradeoffs between small 'Локально' costs and larger 'global' losses. OBJECTIVE: We aimed to distinguish whether 5-HT promotes avoidance of local losses, global losses, or both, in contrast to an overall effect on reflection impuls

A Neurotensin Brake on Exploratory Drive under Persistent Threat.

Avoidance behavior is an adaptive response that delays exploration to promote survival. Avoidance is enhanced by psychological stress and is a hallmark of many neuropsychiatric disorders. Neural circuits that control avoidance by integrating stressful stimuli and modulating exploratory behavior remain underexplored. Elucidating the functional dynamics of this highly conserved phenomenon and the underlying neural mechanism of avoidance is an important open-ended question, with relevance to understanding both innate behaviors and neuropsychiatric disorders.
