Sirtuins in intervertebral disc degeneration: current understanding.

BACKGROUND: Intervertebral disc degeneration (IVDD) is one of the etiologic factors of degenerative spinal diseases, which can lead to a variety of pathological spinal conditions such as disc herniation, spinal stenosis, and scoliosis. IVDD is a leading cause of lower back pain, the prevalence of which increases with age. Recently, Sirtuins/SIRTs and their related activators have received attention for their activity in the treatment of IVDD. In this paper, a comprehensive systematic review of the literature on the role of SIRTs and their activators on IVDD in recent years is presented. The mo…

Inflammatory profiles in canine intervertebral disc degeneration.

BACKGROUND: Intervertebral disc (IVD) disease is a common spinal disorder in dogs and degeneration and inflammation are significant components of the pathological cascade. Only limited studies have studied the cytokine and chemokine profiles in IVD degeneration in dogs, and mainly focused on gene expression. A better understanding is needed in order to develop biological therapies that address both pain and degeneration in IVD disease. Therefore, in this study, we determined the levels of prostaglandin E2 (PGE2), cytokines, chemokines, and matrix components in IVDs from chondrodystrophic (CD)

The Myth of Fibroid Degeneration in the Canine Intervertebral Disc: A Histopathological Comparison of Intervertebral Disc Degeneration in Chondrodystrophic and Nonchondrodystrophic Dogs.

Since the seminal work by Hans-Jörgen Hansen in 1952, it has been assumed that intervertebral disc (IVD) degeneration in chondrodystrophic ( … eneration of the IVD was not present in any of the evaluated grades of degeneration. In conclusion, intervertebral disc degeneration was characterized by chondroid metaplasia of the nucleus pulposus in both NCD and CD dogs. These results revoke the generally accepted concept that NCD and CD dogs suffer from a different type of IVD degeneration, in veterinary literature often refe …

Intervertebral disc spheroids as anin vitromulticellular platform for recapitulating the microenvironment of intervertebral disc degeneration.

Intervertebral disc (IVD) degeneration (IVDD) is a major contributor to chronic low back pain, representing a substantial burden on the spinal healthcare system and serving as a leading cause of long-term disability worldwide. Biomimeticin vitromodels that accurately replicate histological characteristics, three-dimensional structures, and multicellular interactions are lacking. Consequently, monocultures of cell lines and two-dimensional culture models are still used to study the pathomechanisms of IVDD. We established functional multicellular IVD spheroid cultures using primary human annulus

Role of cytokines in intervertebral disc degeneration: pain and disc content.

Degeneration of the intervertebral discs (IVDs) is a major contributor to back, neck and radicular pain. IVD degeneration is characterized by increases in levels of the proinflammatory cytokines TNF, IL-1α, IL-1β, IL-6 and IL-17 secreted by the IVD cells; these cytokines promote extracellular matrix degradation, chemokine production and changes in IVD cell phenotype. The resulting imbalance in catabolic and anabolic responses leads to the degeneration of IVD tissues, as well as disc herniation and radicular pain. The release of chemokines from degenerating discs promotes the infiltration and a

From hyperglycemia to intervertebral disc damage: exploring diabetic-induced disc degeneration.

The incidence of lumbar disc herniation has gradually increased in recent years, and most patients have symptoms of low back pain and nerve compression, which brings a heavy burden to patients and society alike. Although the causes of disc herniation are complex, intervertebral disc degeneration (IDD) is considered to be the most common factor. The intervertebral disc (IVD) is composed of the upper and lower cartilage endplates, nucleus pulposus, and annulus fibrosus. Aging, abnormal mechanical stress load, and metabolic disorders can exacerbate the progression of IDD. Among them, high glucose…

Intervertebral disc degeneration: new insights based on "skipped" level disc pathology.

OBJECTIVE: Typically, age and abnormal physical loading ("wear and tear") have been associated with the development of intervertebral disc degeneration. In the past decade, various additional etiologic factors for disc degeneration have been sporadically reported in the literature; however, many investigators continue to place tremendous emphasis on the effects of age and biomechanics associated with disc degeneration. The aim of this study was to provide additional insight into the notion that age and biomechanics are key factors in the development of disc degeneration. To this end, we addres…

Novel Regulatory Cell Death in Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IDD) is a major cause of chronic low back pain. Its pathogenesis is highly complex and encompasses multiple pathological processes, including cellular dysfunction, extracellular matrix (ECM) degradation, inflammatory responses, oxidative stress, and aberrant mechanical loading. Traditionally, programmed cell death (PCD), particularly apoptosis and autophagy, has been regarded as a key contributor to IDD. However, with ongoing advances in this field, an expanding number of emerging forms of regulated cell death (RCD) have been identified and demonstrated to pla…

METTL3-mediated m6A modification promotes intervertebral disc degeneration.

BACKGROUND: N6-methyladenosine (m6A) modification is a prevalent RNA modification in epigenetics. METTL3, acting as the principal methyltransferase responsible for catalysing m6A, is regarded as a master regulator of this RNA modification. Nonetheless, the complex roles and underlying mechanisms of m6A in relation to intervertebral disc degeneration (IDD) are yet to be fully elucidated. In light of this, this study aimed to explore the intricate functions and mechanisms of METTL3-mediated m6A modification in IDD. METHODS: Our previous batch of RNA sequencing data (GSE167199) and public single…

Tissue engineering strategies for treating intervertebral disc degeneration.

Low back pain (LBP) is becoming prevalent in clinics, owing primarily to intervertebral disc degeneration (IDD). The mechanism of IDD is complex, and its pathophysiology is primarily characterized by a decrease in intervertebral nucleus pulposus cells and an imbalance in the synthesis and degradation of the extracellular matrix of the intervertebral disc (IVD). Grounded in the concept of regenerative repair, IVD engineering has emerged as a significant research focus in spinal surgery. This review systematically examines the relationship between LBP and IDD, describes the physiological charact…

The JNK signaling pathway in intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) serves as the underlying pathology for various spinal degenerative conditions and is a primary contributor to low back pain (LBP). Recent studies have revealed a strong correlation between IDD and biological processes such as Programmed Cell Death (PCD), cellular senescence, inflammation, cell proliferation, extracellular matrix (ECM) degradation, and oxidative stress (OS). Of particular interest is the emerging evidence highlighting the significant involvement of the JNK signaling pathway in these fundamental biological processes of IDD. This paper explo…

[Research progress of resveratrol against intervertebral disc degeneration].

Intervertebral disc degeneration (IVDD) is a leading cause of low back pain, yet effective pharmacotherapeutic options remain limited. Resveratrol (RSV),a natural polyphenol, has demonstrated considerable potential in mitigating IVDD progression. This review systematically summarizes the multi-dimensional molecular mechanisms by which RSV exerts protective effects on nucleus pulposus (NP) cells, the central functional cells within discs, through the regulation of multiple signaling pathways. Specifically,RSV alleviates mechanical and oxidative stress-induced NP cell senescence by inhibiting pathwa…

Role of neurogenic inflammation in intervertebral disc degeneration.

In healthy intervertebral discs (IVDs), nerves and blood vessels are present only in the outer annulus fibrosus, while in degenerative IVDs, a large amount of nerve and blood vessel tissue grows inward. Evidence supports that neurogenic inflammation produced by neuropeptides such as substance P and calcitonin gene related peptide released by the nociceptive nerve fibers innervating the IVDs plays a crucial role in the process of IVD degeneration. Recently, non-neuronal cells, including IVD cells and infiltrating immune cells, have emerged as important players in neurogenic inflammation. IVD ce

Immune Mechanisms and Pain in Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IVDD) is a major cause of low back pain and radicular pain, posing significant socio-economic challenges. The intervertebral disc is traditionally viewed as immune-privileged. This privilege is maintained by physical barriers and molecular factors such as Fas ligand. However, when these barriers are compromised, the nucleus pulposus (NP) can be exposed to the immune system. Recent evidence underscores a critical role for immune cells, particularly macrophages, in IVDD progression. This exposure can trigger an autoimmune response, leading to inflammation that a…

Stimuli-Responsive Delivery Systems for Intervertebral Disc Degeneration.

As a major cause of low back pain, intervertebral disc degeneration is an increasingly prevalent chronic disease worldwide that leads to huge annual financial losses. The intervertebral disc consists of the inner nucleus pulposus, outer annulus fibrosus, and sandwiched cartilage endplates. All these factors collectively participate in maintaining the structure and physiological functions of the disc. During the unavoidable degeneration stage, the degenerated discs are surrounded by a harsh microenvironment characterized by acidic, oxidative, inflammatory, and chaotic cytokine expression. Loss …

Lumbar intervertebral disc degeneration in low back pain.

Intervertebral disc degeneration is characterized by deterioration in structural support that is potentially followed by stimulated neuronal ingrowth, and dysfunction of cellular physiology in the disc. Discogenic low back pain originates from nociceptors within the intervertebral disc or the cartilage endplate. This narrative review examines the mechanisms of disc degeneration, the association between degeneration and pain, and the current diagnosis and treatment of discogenic low back pain. Mechanisms of disc degeneration include dysregulated homeostasis of the extracellular matrix of the di…

Intervertebral disc degeneration-Current therapeutic options and challenges.

Degeneration of the intervertebral disc (IVD) is a normal part of aging. Due to the spine's declining function and the development of pain, it may affect one's physical health, mental health, and socioeconomic status. Most of the intervertebral disc degeneration (IVDD) therapies today focus on the symptoms of low back pain rather than the underlying etiology or mechanical function of the disc. The deteriorated disc is typically not restored by conservative or surgical therapies that largely focus on correcting symptoms and structural abnormalities. To enhance the clinical outcome and the quali…

Roles of NLRP3 inflammasome in intervertebral disc degeneration.

Intervertebral disc degeneration (IVDD) is one of the leading causes of low back pain and one of the most common health problems in the world. The nucleotide-binding oligomerization domain-like receptor family pyrin domain-containing-3 (NLRP3) inflammasome, as a pattern recognition receptor, has been shown to be associated with the pathological processes of many diseases in recent years. With the exploration of the mechanism of IVDD, recent studies have shown that activation of the NLRP3 inflammasome is associated with intervertebral disc (IVD) inflammation, pyroptosis, extracellular matrix de…
