By means of an ophthalmological device "Iatagan", laser iridectomy, in personal modification, was made in 54 patients with chronic (in 40 cases--tuberculosis) uveitis in 67 eyes with predominantly organic changes in the anterior chamber angle. Before operation, ophthalmic hypertension was recorded in 55 eyes, of them glaucoma of different stages--in 43 (in 12 eyes intraocular pressure was normal). Clinicians still encounter patients suffering from prolonged episodes of primary angle-closure glaucoma. These patients often have been misdiagnosed or neglected initially. While laser iridotomies have proven most effective in the treatment of acute angle-closure glaucoma, there is a diversity of opinion regarding the treatment of chronic forms, in particular when the congestive phase has been of long duration (e.g., 3 to 14 days). With the increased popularity of extracapsular cataract surgery, the need for a peripheral iridectomy has been questioned. Some surgeons feel that the risks involved in performing the iridectomy are greater than when leaving the iris untouched. The indications and risks of iridectomy with cataract surgery are discussed. OBJECTIVE: To explore the therapeutic method for congenital perinuclear cataract after optical iridectomy.
METHODS: Twelve cases (24 eyes) with congenital perinuclear cataract after optical iridectomy were selected. After extracapsular cataract extraction (ECCE) and intraocular lens (IOL) implantation, the sides of the iridectomy were sutured to complete the coreoplasty.
RESULT: No patient complained of photophobia and monocular diplopia postoperatively. Every one got a nearly round pupil with light reaction and a corrected visual acuity >/= 0.5.

Forty-nine eyes were treated with iridectomy for primary acute angle-closure glaucoma. Forty asymptomatic, normotensive fellow eyes were similarly treated. The duration of follow-up after surgery was 52.3 +/- 3.6 months. Thirty-seven (76%) of the eyes with acute attacks were cured by iridectomy alone and required no medical therapy. Complications after six months, in a series of 44 aphakic eyes treated by vitrectomy and fluid/silicone-oil exchange with six o'clock (Ando) peripheral iridectomy, are reported. Comparisons are made with the complications after six months in a similar series treated prior to the introduction of the six o'clock iridectomy. It is concluded that a patent six o'clock iridectomy is effective in reducing the complications of intraocular silicone oil in the aphakic eye.

Double-masked study on 40 eyes (40 patients) of primary angle-closure glaucoma demonstrated that topical timolol and indomethacin prevented the IOP rise following combined argon and Nd:YAG laser iridectomy. A combination of the 2 drugs was especially effective in that the number of patients who would have an IOP increase over 1.33 kPa (1 kPa = 7.5 mmHg) above the baseline was reduced by 80%. A 38-year-old female underwent bilateral implantable collamer lens (ICL) implantation for hyperopic astigmatism. While performing intraoperative peripheral iridectomy (PI), iris and anterior lens capsule was drawn to anterior vitrectomy cutter which result in large PI and injury to anterior lens capsule.
