[Acanthamoeba keratitis].

Acanthamoeba keratitis has become a clinical problem together with the widespread use of soft and rigid contact lenses. The number of cases with this kind of infection is growing, but many of cases are diagnosed too late or misdiagnosed. The authors presented, for the first time in Polish literature, a case of acanthamoeba keratitis concerning a 62 aged man. The disease was diagnosed in the right aphakic eye after a 5-year use of soft contact lens, just after cataract operation performed in the clinic in the left eye.

Acanthamoeba keratitis.

1. Acanthamoeba keratitis is an uncommon but increasingly prevalent infection with the potential to cause severe ocular damage. Acanthamoeba is a nonflagellated free-living amoeba that is ubiquitous in the environment. The most common type is A castellani, but A polyphagia, A rhysodes, A culbertson, and A hatchetti have been isolated from infected eyes. 2. Clinical features include foreign body sensation, blurred vision, tearing, and photophobia. There is minimal pain in the early stages of infection, but severe pain is a manifestation of the advanced stages of the disease. 3. The incidence of…

Viral keratitis.

Viruses that affect the cornea produce changes that range from benign, self-limited conjunctivitis to sight-threatening scarring and vascularization of the cornea. In this article, the forms of viral keratitis most commonly encountered by the clinician are reviewed. The epidemiology, clinical presentation, and treatment of infection by Herpes simplex virus, varicella zoster virus, and the adenoviruses are discussed. Also included are other viral infections of the cornea.

Rubeola keratitis.

Twenty-seven young adult patients with rubeola were evaluated during the acute contagious phase of their disease. Koplik's spots were present in all patients. Conjunctivitis was present in 15 of the 27 (56%), and photophobia was present in 14 of the 27 (52%). Bilateral epithelial keratitis was present in all 27 patients. This keratitis, in otherwise healthy young adults, as in healthy children, was a benign process and required no medical therapy. The epithelial lesions were very slow to resolve completely and continued to be seen at the slit lamp after the patients had become asymptomatic. Op…

Dendriform keratitis.

Since the cornea responds to various toxic stimuli by swelling and ulcerating, these changes may assume a dendriform pattern. A case of herpes simplex dendritic keratitis is presented along with its treatment. In addition, several other etiologic sources of an arborescent keratitis are discussed as to their differentiation and management implication.

Cytomegalovirus keratitis.

PURPOSE OF REVIEW: Cytomegalovirus (CMV) keratitis, albeit an uncommon manifestation of this ubiquitous pathogen, can lead to devastating ocular morbidity. Timely diagnosis and appropriate treatment are also unfortunately uncommon. The purpose of this review is to discuss recently published literature regarding the epidemiology, pathophysiology, diagnosis, and therapy of CMV keratitis. RECENT FINDINGS: Classic clinical presentations of CMV keratitis are known; however, current investigations further elucidate characteristics of typical versus atypical disease. Ongoing research stems beyond ut…

Fungal keratitis.

Fungal keratitis, or keratomycoses, are corneal infections which must be considered in cases of corneal trauma, prior corneal surgery, chronic ocular surface disease, topical corticosteroids or contact lens wear. Filamentous fungi or yeasts may be involved. Presenting clinical features such as corneal infiltrates with feathery edges and/or raised surface, intact epithelium with deep stromal involvement, satellite lesions, endothelial plaques, lack of improvement with antibiotics and worsening with steroids are suggestive of fungal keratitis. Corneal scraping for laboratory examination is manda…

[Acanthamoeba keratitis].

Early diagnosis and appropriate therapy are key elements for a good prognosis in Acanthamoeba keratitis (AK). AK should be considered in any case of corneal trauma complicated by exposure to soil or contaminated water, and in all contact lens (CL) wearers. A presumptive diagnosis of AK can be made clinically and with in vivo confocal microscopy, although a definitive diagnosis requires identification of Acanthamoeba on direct scraping, histology, or identification of Acanthamoeba DNA by polymerase chain reaction (PCR). We use cysticidal drugs for treating AK because encysted forms are more res…
