Endolymphatic hydrops and blood-labyrinth barrier in Ménière's disease.

CONCLUSIONS: The blood-labyrinth barrier is impaired in association with the hydrops grade in Ménière's disease. OBJECTIVES: To investigate the relationship between endolymphatic hydrops and the clinical characteristics of patients with Ménière's disease revealed by 3 T magnetic resonance imaging (MRI). METHODS: A double dose of gadoteridol (Gd; 0.2 mmol/kg) was injected intravenously in 12 patients with Ménière's disease. We performed three-dimensional fluid attenuated inversion recovery MRI and three-dimensional real inversion recovery MRI 4 h later using a 3 T MRI unit. Ten patients had u

[Laser destruction of the labyrinth in Meniere's disease: rationale and results].

The authors suggest that Meniere's disease may arise as a psychovegetative syndrome with impairment of labyrinthine homeostasis. It is thought valid to distruct the cupular receptor of the horizontal bony semicircular canal by means of impulse neodymium laser radiation brought to the ampullare bone wall via quartz light guide. Short- and long-term effects of such laser treatment are available. This technique of laser destruction is symptomatic, aimed at relief of severe episodes of vertigo.

[Urea treatment of Ménière's disease and other labyrinth disorders].

Urea (9 g a day) is a very useful drug in the treatment of Meniere's disease. Results are also effective in other labyrinthine disorders (sudden deafness, tinnitus) but less constant. The mechanism of the action of urea on the pathologic labyrinth is discussed.

[Bilateral deafness due to labyrinthitis in a patient with Crohn's disease].

Labyrinthitis with vestibulocochlear dysfunction in chronic inflammatory bowl disease is a rare but well described manifestation or complication. The diagnostic and therapeutic possibilities and limitations of this inflammatory otopathy are presented and discussed in the context of a case report. A bilateral loss of vestibular function and sensorineural hearing loss occurred in a 26-year-old male patient with previously diagnosed Crohn's disease. He was treated with immune suppressive therapy during the onset and development of cochleovestibular symptoms. Diagnostic tests included pure tone au

Serous labyrinthitis as a manifestation of cat scratch disease: a case report.

INTRODUCTION: Cat scratch disease is an infectious disease transmitted by young cats, in which the principal causative factor is Bartonella henselae. The typical course of cat scratch disease is usually benign and self-limited and requires only supportive therapy. However, cases lasting up to 2 years have been reported, and more serious complications may occur. Many manifestations of the disease have been reported by different medical disciplines. CASE PRESENTATION: A case of cat scratch disease in a 71-year-old Greek woman with an unusual clinical course is presented here. Serous otitis medi

Labyrinth anesthesia--a forgotten but practical treatment option in Ménière's disease.

The aim of this study was to determine the efficiency of labyrinth anesthesia - the intratympanic instillation of lidocaine--in the treatment of Ménière's disease and to recall a forgotten method. Twenty-four patients (15 male, 9 female), aged from 19.7 to 80.6 years (mean: 47.8 years) with the clinical diagnosis of unilateral Ménière's disease who underwent labyrinth anesthesia in our department were included in this retrospective study. After local anesthesia of the tympanic membrane, a solution of 4% lidocaine and furfuryladenine (Kinetin) was instilled into the tympanic cavity. Patient rec

Streptomycin perfusion of the labyrinth in the treatment of Menière's disease: a modified technique.

The ideal surgical procedure for Menière's disease would combine the high rate of vertigo control and the good hearing preservation of vestibular nerve section with the low morbidity of labyrinthectomy. Shea's technique of streptomycin perfusion of the labyrinth has been modified by making an additional opening into the posterior semicircular canal in an effort to limit the amount of streptomycin going into the cochlea. Seventeen patients with definite Menière's with poor hearing have had this procedure. Vertigo was controlled in 94% and the hearing preserved in 55%. Vestibular rehabilitation

[Diagnosis of labyrinth hydrops in patients on different stages of Meniere's disease].

OBJECTIVE: To calculate the sensitivity (Se) and specificity (Sp) indicators of clinical and instrumental diagnostic methods of labyrinth hydrops (LH) in patients with definite Meniere's disease (MD, according to the EAONO classification, 2015). MATERIALS: We examined 67 patients with definite MD: stage 1 - 24, stage 2 - 23, stage 3 - 20 patients, as well as 30 subjects with sensorineural hearing loss (control group) with tone hearing thresholds similar to those in patients with MD. STUDY METHODS: Clinical, otorhinolaryngological, otoneurological (using audiometric and vestibulometric tests)

Membrane Stress in the Human Labyrinth and Meniere Disease: A Model Analysis.

Introduction The nature and extent of membrane damage encountered in Meniere disease remains unexplained. Pressure-induced membrane stress may underlie the characteristic hydropic distention. Analysis of stress in the several vestibular chambers may offer insight into the nature and progression of Meniere disease. Objective Membrane stress levels will be assessed by constructing a specific model of the human membranous labyrinth through the application of human dimensions to an existing generic model of the mammalian labyrinth. Methods Nominal dimensions for a model of the human membranous lab

Dexamethasone perfusion of the labyrinth plus intravenous dexamethasone for Ménière's disease.

Recent clinical and laboratory evidence indicates that Meniere's disease is an immune-mediated disease. Dexamethasone perfusion of the inner ear through the round window plus intravenous dexamethasone often will stop the dizzy spells, reduce the fullness and low-frequency tinnitus, and sometimes improve the hearing in patients with Meniere's disease. The dexamethasone must act mostly on the endolymphatic sac and, to a lesser extent, on the stria vascularis and spiral ligament, the known targets of immune response in the inner ear, to reduce the endolymphatic hydrops and restore the fluid dynam
