Possible role of laxatives in analgesic nephropathy.

Eight out of ten of patients with analgesic nephropathy were regular and usually heavy laxative takers compared with 12 out of 200 controls from the general population and four out of 70 patients attending a renal clinic. The finding that regular laxative taking was greatly increased in patients with analgesic nephropathy suggests that this condition may often be due to the combined abuse of both laxatives and analgesics. In a series of 40 patients with rheumatoid arthritis all were found to have normal renal function and no patient took laxatives regularly. This finding would explain why anal…

[Persistent pneumonia with fever: are laxatives responsible?].

Aspiration of oil into the lung can lead to pneumonia. The degree of severity depends on the quantity and the nature of the oil. We report the case of a 43-year-old woman with no medical history who was hospitalized for suspected infectious pneumonia. Explorations led to the conclusion that aspiration pneumonia was secondary to the ingestion of oil favored by gastroesophageal reflux.

Adverse effects of laxatives: fact and fiction.

Laxatives are generally well tolerated and may be considered safe drugs. When taken at much higher than the recommended doses (laxative abuse) some side effects may occur (e.g. hypokalemia, metabolic alkalosis, renal tubular damage). It is controversial whether the laxatives currently used may lead to morphologic changes of the autonomous nervous system of the colon. Melanosis coli is due to pigment-laden macrophages within the submucosa. It occurs after long-term intake of anthraquinones and has no functional consequences. No case of 'cathartic colon' has been observed during the last few dec…

Laxative-Induced Contact Dermatitis.

A 5-year-old female with Charcot-Marie-Tooth neuropathy and a history of constipation presented to the emergency department with a new blistering buttocks rash, which was initially concerning for nonaccidental burn. Upon further investigation, it was found that Ex-Lax had been given to the patient for constipation. This had resulted in a bowel movement, which led to an irritant dermatitis. The patient was eventually diagnosed with senna-induced erosive diaper dermatitis. This case report highlights the importance of a thorough history and physical examination to prevent an unnecessary child ab

Laxative abuse as a variant of bulimia.

Two groups of bulimic patients, one group characterized by a pattern of laxative abuse for purposes of weight control and the other group characterized by no history of laxative abuse, were compared on several behavioral variables associated with bulimia. Relative to the nonabusers, the laxative abusers were more likely to abuse diuretics; to use diet pills; to chew and spit out food; and to report a history of suicide attempts, self-injurious behavior, and prior inpatient treatment for depression.

Appropriate Use of Laxatives in the Older Person.

Constipation is a common condition, affecting up to half of all older adults during their lifetime. Untreated constipation has significant impacts, decreasing quality of life and potentially leading to urinary and/or faecal incontinence, faecal impaction and, in severe cases, hospitalisation. The increased constipation prevalence among older populations is multifactorial, with a number of age-related factors contributing to the rise in prevalence with aging. Laxatives are the mainstay of constipation management and are commonly used among older populations for both treatment and prevention of …

Classification, pharmacology, and side-effects of common laxatives.

No definition of the term laxative is satisfactory since their mode of actions affects multiple mechanisms, absorption/secretion and motor activity. Laxatives are very heterogeneous chemically. They act by either holding water inside the bowel lumen (dietary fibre, osmotic laxatives), by inhibition of water absorption or stimulation of secretion (stimulant laxatives), or by stimulation of colonic motility (stimulant laxatives, 5HT4 agonists). Most laxatives have side-effects but these are usually mild.

[New data on the disease induced by laxatives].

The authors report 5 personal case and recall the physiopathological mechanisms of metabolic disorders observed during the abuse of laxatives. They emphasize the fact that electrolyte losses were not only fecal, but also aggravated by frequent use of diuretics or dietary restriction. One may oppose the constant, very marked fall in body potassium to the variation in sodium content of the organism. Oedema formed part of the clinical picture. The withdrawal of drastic laxatives and diuretics, led to rapid recovery of all the symptoms.

Laxatives and faecal incontinence in long-term care.

High levels of faecal incontinence and laxative use in long-term care settings for older people are revealed in this study. Further research, together with more considered prescribing policies are needed, the authors suggest.

[Consequence of habitual use of laxatives (author's transl)].

Though habitual, but not excessive use of laxatives can be observed frequently, no accompanying clinical symptoms have yet been described. The frequent observation of tetany and edema is striking, there is a marked decrease of potassium clearance and an incipient impairment of creatinine clearance. A secondary hyperaldosteronism can be ruled out. The electrocardiograms of 9 patients show signs of mild hypokalemia, which in 2 patients were first misinterpreted as myocardial ischemia and in one patient taken as indication of myocarditis. Oral administration of potassium normalizes the electrocar…
