Left Atrial Appendage Closure: What Do We Know?

Atrial fibrillation (AF) is the most common arrhythmia in the United States and the most common cause of embolic cerebrovascular events, with the majority of these thrombi originating in the left atrial appendage. The left atrial appendage (LAA) has separate developmental, ultrastructural, and physiological characteristics from the left atrium. Although LAA anatomy is highly variable, it can be categorized into 4 types: cactus, cauliflower, chicken wing, and windsock. The cauliflower type is associated with higher stroke risk in patients with nonvalvular AF. Although the cornerstone of therapy

Remaining Challenges With Transcatheter Left Atrial Appendage Closure.

Left atrial appendage closure has emerged as a feasible stroke prevention strategy in selected patients with nonvalvular atrial fibrillation. Since its commercial approval in the United States in 2015, the use of percutaneous left atrial appendage closure has witnessed a substantial growth. However, certain issues remain unresolved with the technology. Knowledge of these issues, their significance, and the current and future efforts to resolve them is key for proper informed decision making by physicians and patients.

Percutaneous left atrial appendage closure for stroke prevention.

INTRODUCTION: In atrial fibrillation (AF) patients with an increased stroke risk, oral anticoagulation (OAC) is the standard treatment for stroke prevention. However, this therapy carries a high risk of major bleeding. Percutaneous closure of the left atrial appendage (LAA) is suggested as an alternative option for stroke prevention in AF patients with contraindication(s) for OAC treatment. MATERIAL AND METHODS: A total of 42 patients underwent percutaneous LAA closure. In this report, we describe our experience with this procedure. RESULTS: The patients treated were AF patients with a high

Left Atrial Appendage Closure or Medical Therapy in Atrial Fibrillation.

BACKGROUND: Catheter-based closure of the left atrial appendage is an alternative to oral anticoagulation for stroke prevention in patients with atrial fibrillation. The effectiveness of this strategy, as compared with physician-directed best medical care, in patients at high risk for stroke and bleeding is unknown. METHODS: In this multicenter randomized trial conducted in Germany, we assigned patients with atrial fibrillation and a high risk of stroke and bleeding to undergo left atrial appendage closure or to receive physician-directed best medical care (including direct oral anticoagulant…

Left Atrial Appendage Closure: Past, Present, and Future.

Left atrial appendage closure (LAAC) has emerged as an important alternative strategy for stroke prevention in patients with non-valvular atrial fibrillation (NVAF), particularly when long-term oral anticoagulation (OAC) is contraindicated or poorly tolerated. Initially developed as a surgical technique based on ligation or excision of the left atrial appendage, LAAC has progressively shifted toward refined, minimally invasive transcatheter approaches. Randomized and observational data have established non-inferiority to warfarin, and non-inferiority compared with direct oral anticoagulants ha…

Update on left atrial appendage closure for neurologists.

A significant proportion of patients with atrial fibrillation (AF) who need thromboembolic protection are not treated with or discontinue or … F patients in the early 2000s. Due to significant improvements in procedural safety over the years, left atrial appendage closure (LAAC), predominantly achieved through a percutaneous catheter-based device implantation approach, is increasingly favoured for preventing thromboembolic events in patients who cannot achieve effective anticoagulation or have a high hemorrhagic risk …

Remote Tamponade After Percutaneous Left Atrial Appendage Closure.

BACKGROUND: The use of percutaneous left atrial appendage closure (LAAC) among patients with nonvalvular atrial fibrillation at high thromboembolic risk and contraindication to or high risk for bleeding with oral anticoagulation is increasing. CASE SUMMARY: The authors present a case of acute hemorrhagic tamponade 7 months after LAAC. Intraoperative findings showed device-related microperforation of the LAA and erosion of the adjacent pulmonary artery. DISCUSSION: Remote tamponade after LAAC has been reported with an incidence <1%, occurring more frequently in nitinol plug devices than in ni…

Computed tomography measurement for left atrial appendage closure.

Percutaneous left atrial appendage closure (LAAC) has been reported many therapeutic effects with regard to its safety and efficacy, and the number of patients with non-valvular atrial fibrillation undergoing LAAC is increasing worldwide. Although it is a highly safe procedure, further improvements are expected and preoperative planning is extremely important. For this purpose, transesophageal echocardiography has been mainly performed so far, however, nowadays, it is recommended to determine a more optimal treatment strategy combined with computed tomography. Preoperative CT predicts not only…

Follow-up imaging after left atrial appendage closure.

BACKGROUND: Because device-related thrombus (DRT) portends a poor prognosis after left atrial appendage closure with the Watchman device, surveillance transesophageal echocardiography (TEE) is recommended at 45 days and 1 year. However, oral anticoagulants are just discontinued at 45 days, rendering this early TEE unlikely to detect DRT. Indeed, DRT is most likely to occur after instituting aspirin monotherapy. OBJECTIVE: The purpose of this study was to evaluate the alternative strategy of first TEE imaging (or computed tomography) at 4 months post-Watchman implantation. METHODS: After Food…

Totally Thoracoscopic Closure of the Left Atrial Appendage.

The left atrial appendage (LAA) is a major site of clot formation in atrial fibrillation. Stand-alone thoracoscopic LAA complete closure can decrease stroke risk and may be an alternative to life-long oral anticoagulation. This report describes a technique for totally thoracoscopic LAA exclusion with an epicardial clip device. This approach provides a safe and likely more effective alternative to LAA management than other endocardial devices.

Percutaneous left atrial appendage closure in a patient with atrial fibrillation.

BACKGROUND: A 73-year-old woman presented with a history of persistent atrial fibrillation, which had lasted more than five years. She also had a remote history of transient ischemic attack and had received warfarin therapy. The international normalized ratio had been carefully maintained at slightly subtherapeutic levels because of recurrent gastrointestinal bleeding, which was severe enough to require frequent blood transfusions. With colonoscopy, the source of bleeding was localized to multiple arterial-venous malformations. The patient underwent catheter ablation of the atrioventricular ju

Safety of Amulet Left Atrial Appendage Occluder and Watchman Device for Left Atrial Appendage Closure in Patients With Atrial Fibrillation.

BACKGROUND: Left atrial appendage (LAA) closure is an alternative to chronic anticoagulation for stroke prevention in patients with nonvalvular atrial fibrillation. Multiple devices were used for LAA closure, with the Amplatzer Amulet LAA Occluder (Abbott, Chicago, IL, USA) and Watchman device (Boston Scientific, Marlborough, MA, USA) being the most commonly used in clinical practice. In August 2021, the FDA approved the use of the Amplatzer Amulet LAA Occluder. There is still a knowledge gap in the safety profile of the Amplatzer Amulet LAA Occluder device in comparison to the Watchman device

Should left atrial appendage closure be considered in resistant left atrial appendage thrombus cases? 'Former Foe, New Ally'.

BACKGROUND: The study explores left atrial appendage closure (LAAC) as a safe and effective alternative to anticoagulation for atrial fibrillation (AF) patients at high bleeding risk. Complications, such as cardioembolic events due to left atrial appendage thrombus (LAAT), highlight the need for alternative stroke prevention strategies. AIMS: This research assesses LAAC's safety and efficacy in patients with LAAT, aiming to offer valuable insights into its potential as a viable option for stroke prevention in such cases. METHODS: The study included 205 patients who underwent LAAC using speci…

The ABC of sealing following left atrial appendage closure.

BACKGROUND: Peri-device leak (PDL) following left atrial appendage closure (LAAC) can be diagnosed by both transesophageal echocardiography (TEE) and cardiac computed tomography (CCT). Yet, there is a substantial discrepancy in leak quantification. AIMS: We propose a novel ABC classification of LAA sealing for TEE and CCT based on the pattern of intra-device thrombosis (IDT). METHODS: A single-center observational study including patients undergoing LAAC with Watchman FLX or FLX-Pro between 2022 and 2024. Patients underwent CCT and TEE at 14 days, 45 and 90 days after LAAC. Images were analy…
