Calcifying aponeurotic fibroma of the thigh: A differential diagnosis for infant knee contracture and leg length inequality (a case report).

INTRODUCTION: Calcifying aponeurotic fibroma is an uncommon type of tumor that primarily occurs in the distal extremities of young children. It usually appears as a slow-growing and asymptomatic soft-tissue mass and rarely causes complications, such as joint contractures. PRESENTATION OF CASE: We present a case of calcifying aponeurotic fibroma that caused knee contracture and leg length inequality, in which the affected leg was longer than the other. On the patient's first visit to our hospital at 8 years of age, a solid nodule was palpable at the medial distal area of her right thigh. Magne…

Induced leg length inequality affects pelvis orientation during upright standing immediately following a sit-to-stand transfer: a pre-post measurement study.

BACKGROUND: Leg length inequality (LLI) greater than 20 mm has been associated with low back pain (LBP) and its correction is clinically recommended. Much less is known about the biomechanical effects that LLI below 15 mm has on pelvis orientation. METHODS: Twenty-two adult participants (8 female) aged between 18 and 30 years without LBP were enrolled in the study and completed a series of sit-to-stand trials with no heel-lift (0 mm baseline) and heel-lifts of varying heights (5, 9 and 12 mm) placed in their right shoe. Three-dimensional kinematic data were obtained from the lower extremities…

Controlling Leg Length Change During Total Hip Arthroplasty Using Three-Dimensional Modeling and a Single Intraoperative Distance Measurement.

BACKGROUND: Leg length inequality following total hip arthroplasty (THA) is a major cause of patient dissatisfaction. This study evaluated the accuracy and feasibility of a workflow combining computed tomography-based preoperative planning with a single intraoperative linear distance measurement for leg length restoration in primary THA. METHODS: A consecutive series of 40 patients undergoing primary, mixed reality-assisted THA using a minimally invasive superior capsulotomy approach was analyzed. Preoperative three-dimensional (3D) planning calculated the distance between the prosthesis shou…

Perioperative management of leg-length discrepancy in total hip arthroplasty: a review.

Leg-length discrepancy (LLD) presents a significant management challenge to orthopedic surgeons and remains a leading cause of patient dissatisfaction and litigation after total hip arthroplasty (THA). Over or under-lengthening of the operative extremity has been shown to have inferior outcomes, such as dislocation, exacerbation of back pain and sciatica, and general dissatisfaction postoperatively. The management of LLD in the setting of THA is multifactorial, and must be taken into consideration in the pre-operative, intra-operative, and post-operative settings. In our review, we aim to summ

Proximal Femoral Focal Deficiency/Congenital Femoral Deficiency: Evaluation and Management.

Proximal focal femoral deficiency is a congenital transverse deficiency in which the femur is globally smaller with a typical proximal deformity at the hip that may include distal involvement of knees, leg, and feet. Congenital femoral deficiency (CFD) describes a broader spectrum of longitudinal deficiency inclusive of proximal focal femoral deficiency. CFD may also include lateral distal femoral hypoplasia, knee cruciate ligament deficiency, rotational instability, patellar dislocation, fibular hemimelia, ray absence, and contralateral limb involvement. Treatment intends to maximize function

Distance Between the Malleoli and the GroundA New Clinical Method to Measure Leg-Length Discrepancy.

BACKGROUND: The aim of this work is to introduce a useful method for the clinical diagnosis of leg-length inequality: distance between the malleoli and the ground (DMG). METHODS: A transversal observational study was performed on 17 patients with leg-length discrepancy. Leg-length inequality was determined with different clinical methods: with a tape measure in a supine position from the anterior superior iliac spine (ASIS) to the internal and external malleoli, as the difference between the iliac crests when standing (pelvimeter), and as asymmetry between ASISs (PALpation Meter [PALM]; A&D M

Limb-length measurement in total hip arthroplasty using a calipers dual pin retractor.

BACKGROUND: Limb-length inequality is not uncommon after total hip arthroplasty (THA) and may cause subjective problems for patients. During THA a stable reference point must be established to determine changes in leg length. Several methods have been useful in minimizing the incidence and magnitude of this problem. The equalization of leg-length discrepancy during THA can be achieved through the use of a simple measuring device, the calipers dual pin retractor (CDPR). MATERIALS AND METHODS: The CDPR was developed to establish a fixed point on the pelvis that would remain constant throughout

Leg length discrepancies (LLD): An etiology to be considered in its proper measure. A critical and historical review.

INTRODUCTION: Studies of leg-length discrepancies (LLD) have been the subject of debate for almost 200 years. A large number of studies have emerged, most frequently using assessment criteria based on painful symptoms or joint damage. DEVELOPMENT: While many authors argue for a threshold of 10-20 mm to establish a link between pain and LLD, most publications based on radiography show lesion stigmata on lumbar, hip and knee joints as early as 6 mm. This would be linked to comorbidities. DISCUSSION: Some studies argue forcefully that leg-length correction below 20 mm is of no benefit. The auth

Chiropractic concepts of the short leg: a critical review.

There have been relatively few subject areas within the chiropractic profession which have led to such widespread misunderstanding, confusion and controversy. This paper will attempt to examine the research surrounding the development of our current state of understanding concerning the many factors affecting the short leg. Traditionally, research surrounding the short leg has moved in several discreet directions. The first has been toward determining a reliable radiographic method used for measuring structural short legs. Many papers have been published here; included would be the work of Heu

Something Doesn't Look Right: Recognizing and Managing Limb-Length Discrepancy.

Leg length discrepancy (LLD) is a frequently encountered yet often underrecognized musculoskeletal condition in children. Minor differences in leg length are common and typically benign; however, more significant discrepancies may reflect underlying congenital, developmental, or acquired pathology. Because many children adapt well, these discrepancies can remain unnoticed until gait changes, pain, or functional limitations appear. Early recognition is essential, as management depends on the child's remaining growth and the underlying cause. Epidemiologic studies suggest that up to 90% of child

Proximal femoral focal deficiency.

Proximal femoral focal deficiency (PFFD) is an uncommon congenital defect that involves the femur and acetabulum in varying degrees. It may occur with or without fibular hemimelia and can be unilateral or bilateral in presentation. Children with PFFD and their families are faced with many treatment decisions, both nonsurgical and surgical. Nursing care is central in the care of these children and their families both for psychosocial support and teaching during the decision-making process and for being a patient advocate to help meet postoperative and rehabilitation goals.

Unilateral lower limb hypoplasia in arterial fibromuscular dysplasia.

Hypoplasia or hyperplasia of limbs can be unilateral or bilateral and can involve the upper or lower extremities in any combination. The etiology is varied, and the presentation, clinical features, and investigative pathway differ to some extent among patients. Reported here is a child, referred for limb lengthening, with claudicating unilateral leg pain and unilateral hypoplasia of the same limb. Investigations revealed multiple arterial stenoses consistent with fibromuscular dysplasia. In such cases, early diagnosis and intervention with angioplasty may enhance subsequent growth of the limb,

Orthopedic surgical management of hip and knee involvement in patients with juvenile rheumatoid arthritis.

Juvenile rheumatoid arthritis is the most common arthritic disease of childhood and a leading cause of childhood disability, affecting an estimated 300,000 US children and adolescents aged < or =16 years. Approximately 10% to 30% of patients experience functional deficits resulting from both the articular and systemic manifestations of their disease, including leg length inequality and deformity, that are often more crippling than joint destruction. Surgical intervention to treat bone and soft-tissue deformity, leg length inequality, and joint destruction is indicated when medical therapy has …

The effect of simulating a leg-length discrepancy on pelvic position and spinal posture.

[Purpose] The purpose of this study was to examine how a leg-length discrepancy contributes to the pelvic position and spinal posture. [Subjects and Methods] A total of 20 subjects (10 males, 10 females) were examined during different artificially created leg-length inequalities (0-4 cm) using a platform. The pelvic tilt and torsion and the sagittal deviation of the spine were measured using the rasterstereographic device formetric 4D. [Results] Changes in platform height led to an increase in pelvic tilt and torsion, while no changes in the spinal posture were found with the different simulat
