Adult T-cell leukemia/lymphoma.

Adult T-cell leukemia/lymphoma (ATL) is a malignancy of mature CD4+ T-cells caused by human T-cell lymphotropic virus type 1 (HTLV-1). Twenty million people are believed to be infected throughout the world, mostly in Japan, Africa, the Caribbean, and South America, particularly in Brazil and Peru. ATL affects about 5% of infected individuals and is classified in the following clinical forms: acute, lymphoma, primary cutaneous tumoral, chronic (favorable and unfavorable), and smoldering (leukemic and non-leukemic). Although it is considered an aggressive disease, there are cases with a long pro

Adult T-cell leukemia/lymphoma.

Adult T-cell leukemia/lymphoma is a rare mature CD4(+) T-cell neoplasm caused by the retrovirus human T-lymphotrophic virus type 1. At present there are approximately 20 million people infected globally with this virus, and most of these individuals belong to the endemic areas in southern Japan, Africa, the Caribbean basin, and Latin America. In the United States, it is usually seen in immigrants from these endemic regions. Adult T-cell leukemia/lymphoma predominantly affects the adult population and is rare in children. Adult T-cell leukemia/lymphoma has 4 subtypes: acute, lymphomatous, chron

Adult T-cell leukemia/lymphoma.

The adult T-cell leukemia/lymphoma (ATLL) syndromes comprise neoplasms that arise in peripheral lymphoid tissues but a high frequency present with blood involvement mimicking T-cell leukemia. Clinically ATLL is sub-classified into four groups: acute, lymphomatous, chronic and smoldering. ATLL is etiologically linked to the human T-cell lymphotropic virus type I (HTLV-I). The diagnosis of ATLL is based upon a combination of characteristic clinical manifestations, morphological and immunophenotypic changes of the malignant cells, in addition to the confirmation of HTLV-I infection. ATLL is an ag

Adult T-cell leukemia-lymphoma.

Adult T-cell leukemia-lymphoma (ATL) was first described in 1977 as a distinct clinico-pathological entity with a suspected viral etiology. Subsequently, a novel RNA retrovirus, human T-cell leukemia/lymphotropic virus type 1 (HTLV-1) was isolated from a cell line established from the leukemic cells of an ATL patient, and the finding of a clear association with ATL led to its inclusion among human carcinogenic pathogens. The three major routes of HTLV-1 transmission are mother-to-child infections via breast milk, sexual intercourse, and blood transfusions. HTLV-1 infection early in life, presu

Cutaneous T-cell lymphoma/leukemia.

Effective treatment for cutaneous T-cell lymphomas (CTCL) requires an accurate and specific diagnosis based on the clinical presentation combined with evaluation of the histopathology, immunophenotyping, and gene rearrangement studies. Careful clinical and pathologic evaluation in centers familiar with the diverse forms of CTCL is most valuable for determining treatment options. The goals of treatment in mycosis fungoides (MF), which afflicts more than 50% of patients with CTCL, are the relief of symptoms and improvement in cosmetics. Despite some uncontrolled clinical trial results that have

[Adult T-cell leukemia (ATL)].

Adult T-cell leukemia (ATL) was first discovered and reported in Japan, where it has a high incidence in the southwest region. The retrovirus HTLV-I (human T-cell lymphotropic virus type I) is considered to be related to its etiology. In ATL endemic areas, HTLV-I carriers are found at a fairly high percentage even among healthy individuals. ATL shows diverse clinical features. It can be divided into 5 types (acute type, chronic type, smoldering type, crisis type, and lymphoma type). ATL cells originate from the CD4-positive subset of peripheral T cells; they show a characteristic notch in the

Caribbean T-cell lymphoma/leukemia.

Caribbean T-cell lymphoma/leukemia (CATL) is a recently described neoplasm similar to Japanese adult T-cell leukemia/lymphoma, but it occurs in patients born in a different geographic region. Four adult Black West Indian women with CATL were studied using a multiparameter approach, including clinical, hematologic, pathologic, and immunologic investigations. The disease was usually characterized by adenopathy (with or without hepatosplenomegaly), peripheral blood involvement, hypercalcemia often in the absence of gross bony lesions, and a poor prognosis. The neoplastic cells were pleomorphic wi

Adult T-Cell Leukemia/Lymphoma.

Adult T-cell lymphoma/leukemia (ATL) is a rare T-cell lymphoproliferative neoplasm caused by human T-lymphotrophic virus 1. In its more common, aggressive forms, ATL carries one of the poorest prognoses of the non-Hodgkin lymphomas. The disease has clinical subtypes (ie, acute, lymphoma, chronic, and smoldering forms) defined by the presenting features, and therefore, the clinical course can vary. For the smoldering and lower-risk chronic forms, combinations involving antiviral therapies have shown some success. However, in many patients, the more indolent forms will evolve into the more aggre

Adult T-cell leukemia/lymphoma.

Adult T-cell leukemia/lymphoma is a rare malignancy associated with the human retrovirus human T-cell lymphotropic virus type 1. It is characterized by the proliferation of highly pleomorphic lymphocytes. Involvement of peripheral blood, bone marrow, lymph nodes, spleen, and extranodal sites such as skin, liver, gastrointestinal tract, and central nervous system can occur. There are four distinct clinical variants, and the prognosis and clinical course range from highly aggressive to a more protracted course depending on the subtype. We describe a man with de novo adult T-cell leukemia/lymphom

Adult T-cell leukemia-lymphoma.

Adult T-cell leukemia-lymphoma, an aggressive T-cell leukemia, is characterized by the presence in the peripheral blood of malignant T cells that have highly indented or lobulated nuclei. Phenotypically the cells are usually helper T cells, but functionally they behave as suppressor cells. Patients have skin and lung involvement, hepatosplenomegaly, moderate lymphadenopathy sparing the mediastinum, and various metabolic abnormalities such as hypercalcemia. The clinical course may be chronic or acute, usually followed by a rapidly progressive terminal course. Adult T-cell leukemia-lymphoma is n

Childhood T-cell malignancy resembling adult T-cell leukemia/lymphoma.

We describe two childhood cases of a T-cell malignancy indistinguishable from nonendemic, idiopathic adult T-cell leukemia/lymphoma. The cases illustrate the heterogeneity of nonendemic, idiopathic ATLL, including the presence of mediastinal masses and more heterogeneous immunologic markers than endemic cases, and represent the two youngest cases of ATLL yet described. The differential diagnosis of nonendemic, idiopathic ATLL is discussed.

Radiotherapy for human T-cell leukemia virus type 1-associated adult T-cell leukemia/lymphoma.

Adult T-cell lymphoma/leukemia (ATL) is an aggressively malignant peripheral T-cell neoplasm. Only a few studies have reported the use of radiotherapy (RT) for ATL. Therefore, the aim of this study was to clarify the efficacy of RT for ATL. We retrospectively reviewed 90 courses of RT administered to 19 consecutive ATL patients between 2008 and 2023. The subtypes included lymphoma (n = 8), acute (n = 7), smoldering (n = 3) and unknown (n = 1). Ninety lesions (cutaneous = 72, extranodal = 10, nodal = 8) were treated at a mean dose of 39.1 Gy/3.2 Gy (range, 24-60 Gy). Administration of RT for AT

Immunologic control of human T-cell leukemia virus type I and adult T-cell leukemia.

Host T-cell responses to human T-cell leukemia virus type I (HTLV-I) control the expansion of HTLV-I-infected cells and are determinants of the equilibrium proviral load in vivo. Insufficient T-cell responses are regarded as an immunologic risk factor for adult T-cell leukemia (ATL) because they allow increased proviral loads, which represent an epidemiologic risk factor for ATL. ATL cells from approximately half of ATL cases retain the ability to express HTLV-I Tax, a major target antigen of HTLV-I-specific cytotoxic T-lymphocytes (CTL), whereas Tax-specific CTL in ATL patients are inactive.
