Reactive lymphadenopathies often pose a diagnostic problem. The distinction of a reactive from a neoplastic lymph node requires the application of general histologic criteria that traditionally have involved the evaluation of cytologic features including cellular polymorphism v monomorphism and the presence or absence of cytologic atypia. Although these cytologic criteria remain valid, they are not inviolate and exceptions exist that may result in diagnostic ambiguity. In addition, the interpretation of a reactive process in a lymph node is predicated on the evaluation of the dominant histolog

Cervical lymphadenopathy is a common problem in children. The condition most commonly represents a transient response to a benign local or generalized infection, but occasionally it might herald the presence of a more serious disorder. Acute bilateral cervical lymphadenopathy usually is caused by a viral upper respiratory tract infection or streptococcal pharyngitis. Acute unilateral cervical lymphadenitis is caused by streptococcal or staphylococcal infection in 40% to 80% of cases. The most common causes of subacute or chronic lymphadenitis are cat scratch disease, mycobacterial infection, a…

Syphilis is well known as a great mimic, however it is not recognized as a cause of cervical lymphadenopathy. We report a case of a 21-year-old man who presented with marked unilateral cervical lymphadenopathy. He had no evidence of oropharyngeal chancres, skin or genital lesions. Computed tomography (CT) and magnetic resonance (MR) images showed multiple cervical lymphadenopathy, and serologic tests for syphilis were positive. Syphilis should still be a factor for consideration in the differential diagnosis of cervical lymphadenopathy.

2 patients with intrathoracic lymph node enlargement as their presenting symptom, were found to have amyloid lymphadenopathy. Both also had peripheral involved lymph nodes. The demonstration of amyloid deposits in the peripheral lymph nodes did not suffice to deduce that the intrathoracic nodes were of the same nature. A biopsy of the intrathoracic nodes was needed for final diagnosis. An additional 11 cases of intrathoracic amyloid lymphadenopathy from the literature are reviewed.

A report is presented on four cases of angioimmunoblastic lymphadenopathy. In this recently individualized entity, adenomegaly with fever and cutaneous eruption is associated with polyclonal hypergammaglobulinemia and autoimmunization, especially of anti-erythrocytic type. The lymph nodes are homogenized by a lymphoplasmo-immunoblastic granuloma with angiogenesis. The disease could be attributed to hyperplasia of B lymphocytes with a deficit of T cells, which would explain the autoimmunization. It appears in some cases to be triggered by accidents of drug intolerance. Prognosis is poor and the…

IgG4-related lymphadenopathy is a nodal manifestation of IgG4-related disease (IgG4RD) which is characterized by increased polytypic IgG4+ plasma cells and IgG4+/IgG+ plasma cell ratio in lymph nodes and morphologically manifested as various patterns of reactive lymphadenopathy: Castleman disease-like, follicular hyperplasia, interfollicular expansion, progressive transformation of germinal centers and inflammatory pseudotumor-like. It typically presents with solitary or multiple, mild to moderate lymph node enlargement in otherwise asymptomatic patients. The serum IgG4 level is frequently ele…

An elderly patient presented with supradiaphragmatic lymphadenopathy due to massive amyloid deposits. No other organ was involved. The urine contained minute amounts of monoclonal light chains. This isolated amyloid lymphadenopathy with monoclonal gammopathy is of obscure nature and pathogenesis.

A 6-year-old boy was referred to the paediatric infectious disease clinic with a 2-month history of enlarged, erythematous, painless cervical lymph nodes. He initially presented to his paediatrician with a painless lesion. At that time, he was treated empirically with clindamycin and azithromycin due to a history of cat exposure. Despite treatment, the lesion evolved into a non-healing linear ulcer with painless, ascending cervical lymphadenopathy. Serologies were negative for Bartonella henselae antibodies. Additional laboratory studies revealed eosinophilia and negative Toxoplasma gondii ant…

A persistent lymphadenopathy poses a diagnostic challenge for physicians. A wide variety of different diseases can manifest with enlarged lymph nodes. The most common causes are infections and other non-malignant conditions, but it is important not to overlook warning signs of a malignant disease to avoid delaying further diagnostics and therapy. This article provides an overview of the most important diagnostic steps as well as the most common differential diagnoses in adult patients.

Mediastinal adenopathies without pulmonary disease may be benign, lymphomatous or the metastases from intra- or extrathoracic malignancy or more rarely metastases with unknown primary site. We observed 507 patients with isolated mediastinal adenopathies: benign, lymphomatous and metastatic disease represented 41.4% (210/507), 26.8% (136/507), 31.8% (161/507) of them, respectively. Management of the latter was the most challenging. Surgery was generally diagnostic, restricted to confirming the metastatic process, because of too numerous and disseminated or unresectable lymph nodes in 84% of pat
