Since the discovery of human T-cell leukemia virus type 1 (HTLV-1) in patients with adult T-cell leukemia/lymphoma (ATLL), malignant neoplasms of mature (peripheral) T lymphocytes have attracted a great deal of attention. This type of neoplasm is more common in Japan than in Western countries, and may show distinct clinical pictures such as hypergammaglobulinemia, hypercalcemia, etc. T-cell lymphomas are more prone than B-cell lymphomas to become leukemic. Because of a marked intermingling of reactive cells (histiocytes, eosinophils, etc.), the histologic diagnosis of T-cell lymphoma is often

We report three cases of malignant T cell lymphoma of peripheral T lymphocyte origin. This morphological entity was first described by Lennert and Waldron. The diagnosis based on morphological criteria only is difficult because of the many lymphoid malignancies. Immunological characterization is necessary for the identification of the proliferative T lymphocyte subset with a panel of T monoclonal antibodies. Correlations were established between immunological phenotype and clinical and evolutive aspects which are particularly varied in T lymphoproliferative disorders.

A comparison of the detailed features and natural history of 12 patients with "Sézary Syndrome" with those in patients with classical "mycosis fungoides" has demonstrated that, with the exception of a leukemic phase in the former, the two syndromes are indistinguishable. Both disorders result from the proliferation of malignant thymus-derived lymphocytes with progressive involvement of skin, lymph nodes, and visceral organs; actual transition between these two syndromes have been documented. "Sézary Syndrome" and "mycosis fungoides" appear to represent variant clinical expressions of the same

Cutaneous T-cell lymphoma is a group of non-Hodgkin T-cell lymphomas that develop in and affect the skin but can potentially spread to other organs. There are many subtypes, the most common of which are mycosis fungoides, Sezary syndrome, lymphomatoid papulosis, and primary cutaneous anaplastic large cell lymphoma. Cutaneous lymphoma is a common cause of recalcitrant chronic skin rash and notoriously mimics other dermatologic and hematologic conditions, often resulting in diagnostic delays of months to years. This review provides an introduction to cutaneous T-cell lymphoma, with a primary foc

Angioimmunoblastic T-cell lymphoma (AITL) is one of the most common types of T-cell lymphoma, representing about 15-20% of cases of peripheral T-cell lymphoma (PTCL). It is characterized by a unique clinical presentation and distinct pathologic and molecular features. Classes of drugs particularly active in AITL are emerging; however, treatment of relapsed and refractory disease remains a challenge. This chapter reviews the epidemiology, clinical presentation, pathogenesis, diagnosis, and treatment of AITL.

Cutaneous T-cell lymphomas comprise a heterogeneous group of diseases characterized by monoclonal proliferations of T lymphocytes primarily involving skin, modified skin appendages, and some mucosal sites. This article addresses the basic clinical, histologic, and immunohistochemical characteristics of this group of diseases, with additional attention to evolving literature on dermoscopy, reflectance confocal microscopy, flow cytometry, and molecular data that may increasingly be applied to diagnostic and therapeutic algorithms in these diseases. Select unusual phenotypes or diagnostic example

Cutaneous T-cell lymphoma comprises a heterogeneous group of lymphoproliferative disorders characterized by clonal expansions of mature, post-thymic T cells that infiltrate the skin. This article discusses the staging, prognosis, and treatment of mycosis fungoides and Sézary syndrome.

Angioimmunoblastic T-cell lymphoma (AITL) is a rare and aggressive neoplasm clinically characterized by sudden onset of constitutional symptoms, lymphadenopathy, hepatosplenomegaly, frequent autoimmune phenomena, particularly hemolytic anemia and thrombocytopenia, and polyclonal hypergammaglobulinemia. The lymph node histological picture is also distinctive, constituted by a polymorphic infiltrate, a marked proliferation of high endothelial venules, and a dense meshwork of dentritic cells. The neoplastic CD4+ T-cells represent a minority of the lymph node cell population; its detection is faci

OBJECTIVE: To review recent information on the epidemiology, etiology, clinical presentation, staging, treatment approaches, and nursing management of CTCL.
DATA SOURCES: Research and review articles and textbooks.
CONCLUSION: Identifying the etiology and finding a cure for cutaneous T-cell lymphoma is promising as our knowledge and understanding of the T-cell biology evolves, creating more options for innovative therapies or treatment modalities.
IMPLICATIONS FOR NURSING PRACTICE: It is essential for the oncology nurse to understand the disease process, treatment modalities, and side effec

A 32-year-old man presented with a 5-year history of cutaneous nodules on his head and a diffuse, lichenified eruption. Histopathologic examination showed an atypical lymphocytic infiltrate. Immunophenotyping studies determined that the lymphocyte population to be CD4-positive, with partial loss of CD3 and CD7, and immunogenotyping studies showed a clonal rearrangement of the T-cell receptor. A positron-emission tomography scan showed increased uptake in cervical, axillary, and inguinal lymph nodes. A diagnosis of peripheral T-cell lymphoma was made, and the patient is undergoing chemotherapy.

Panniculitic T-cell lymphoma is a rare, aggressive variant of cutaneous T-cell lymphoma, with fewer than 100 cases described. The main problem is its diagnosis, as both the clinical and the histological features may simulate benign panniculitis. We present the case of a 34-year-old male patient, who had presented with an indurated plaque, sclerodermiform in appearance, on the front of the right thigh for 4 months, later accompanied by fever and constitutional symptoms. The initial diagnosis was cellulitis, but no clinical improvement was seen despite systemic antibiotic therapy. After two skin

Angioimmunoblastic T-cell lymphoma (AIL-TCL) is a rare subtype of lymphoma, making up only 1% to 2% of nonHodgkin's lymphomas; however, it accounts for a major subset of peripheral T-cell lymphomas. Angioimmunoblastic T-cell lymphoma has clinical and pathologic features that set it apart from other B- and T-cell lymphomas. More recent studies have delineated the immunophenotypic and genetic features of this unusual lymphoma, and have tentatively identified the cell of origin of this neoplasm.

We describe a patient with severe fatal histiocytic phagocytic panniculitis caused by a pleomorphic T-cell lymphoma. Analysis by polymerase chain reaction revealed clonality for both the T-cell receptor gamma-chain gene and the immunoglobulin heavy-chain gene. We also review 44 reported cases of primary or secondary lymphoma affecting the subcutaneous fat.

Cutaneous T cell lymphoma, previously known as mycosis fungoides, presents as either an aggressive disease (tumeur d'emblee type and the Sézary syndrome type) or a prolonged disease which evolves through patch, plaque, and tumor stages. Erythroderma is present either from the start or develops later in the disease process. Its incidence in the United States more than doubled from 1973 to 1984 (from 0.19 to 0.42 cases per 100,000 population). Staging classifications are being refined to reflect increasing knowledge of important prognostic indicators, e.g., absence or presence of lymph node invo
