The Journal of Clinical Engineering has conducted a broad scope survey of facilities, staffing, wages, benefits, computer equipment, and quality control for Biomedical or Clinical Engineering department in U.S. hospitals. In this two-part, nationwide survey, data were collected from individual on: wages, job responsibilities, certification, union membership, years of employment, and wage increases. Potential sources of Legionella spp. in a university hospital were investigated over 3 years in order to gain better understanding of the ecology and transmission of this organism to hospitalized patients. The survey highlighted the contamination of the hot water system with high concentrations of legionellas (up to 10(6) cfu 1(-1]. Legionella pneumophila serogroup 6 was predominant followed by L. pneumophila serogroup 10. Serogroup 1 and other species (L. longbeachae, L. micdadei) were rarely isolated. Serogroup 6 was also the predominant cause of nosocomial legionellosis in 15 sporadic cases

The cause of an outbreak of Legionnaire's Disease at St. Vincent's Medical Center, Bridgeport, CT, has been traced to the hospital's hot water system after months of testing and decontamination efforts. A total of 28 patients were diagnosed with the disease between January and October 1994, with most of the cases occurring in June and July. Legionnaire's Disease is a kind of pneumonia caused by bacteria that thrive in warm water and can become airborne on tiny water droplets that, if inhaled, spread the disease.

Hospital-acquired legionnaires' disease arises from the presence of Legionella in hospital water systems. Legionella not only persists in hot water tanks but is also found in the biofilm throughout the entire water distribution system. Conditions within water systems that promote Legionella colonization include water temperature, configuration and age of the hot water tank, physicochemical constituents of the water, plumbing materials, and commensal microflora. Hospital-acquired legionnaires' disease has been prevented by instituting control measures directed at the water distribution system.
