Medicare Advantage Plan Bid Risk Underestimates and Corresponding Insurer Revenues.

OBJECTIVE: To estimate the extent to which Medicare Advantage (MA) plans underestimate risk, the additional Part A and Part B (A&B) revenue insurers receive as a result, and how this differs across insurers and over time. STUDY SETTING AND DESIGN: We compared projected risk in plan-level bids submitted by insurers to the actual plan-level risk as subsequently reported by the Centers for Medicare and Medicaid Services (CMS). We estimated the additional A&B revenue MA insurers received due to underestimates. DATA SOURCES AND ANALYTICAL SAMPLES: A total of 34,604 MA plans offered by 317 insurer

State Medicaid Programs Face Increased Spending On Medicare Premiums.

Medicaid covers Medicare premiums and out-of-pocket expenses for low-income older adults, who make up a growing segment of the US population. Using administrative data, we characterized the change in the number of beneficiaries dually enrolled in Medicare and Medicaid and corresponding changes in Medicaid spending on Medicare Part A and Part B premiums during the period 2013-24. The number of dually enrolled beneficiaries increased by 27 percent, while Medicaid spending on Medicare premiums nearly doubled, primarily as a result of growing Part B premium spending. We estimated that at least hal

Prescription Drug Spending in Fee-for-Service Medicare, 2008-2019.

IMPORTANCE: Prescription drug spending is a topic of increased interest to the public and policymakers. However, prior assessments have been limited by focusing on retail spending (Part D-covered drugs), omitting clinician-administered (Part B-covered) drug spending, or focusing on all fee-for-service Medicare beneficiaries, regardless of their enrollment into prescription drug coverage. OBJECTIVE: To estimate the proportion of health care spending contributed by prescription drugs and to assess spending for retail and clinician-administered prescriptions. DESIGN, SETTING, AND PARTICIPANTS:

Medication adherence as a measure of the quality of care provided by physicians.

OBJECTIVES: To assess the extent to which medication adherence in congestive heart failure (CHF) and diabetes may serve as a measure of physician-level quality. STUDY DESIGN: A retrospective analysis of Medicare data from 2007 to 2009, including parts A (inpatient), B (outpatient), and D (pharmacy). METHODS: For each disease, we assessed the correlation between medication adherence and health outcomes at the physician level. We controlled for selection bias by first regressing patient-level outcomes on a set of covariates including comorbid conditions, demographic attributes, and physician f

Falling Rates of Public Orthopaedic Surgery Reimbursements and Utilization, 2016 to 2024.

BACKGROUND: Declining reimbursement rates can lead to decreased access and utilization of common orthopaedic surgeries for patients on Medicare, which is a particularly vulnerable population for musculoskeletal injuries. METHODS: Using the Centers for Medicare & Medicaid Services Physician Fee Schedule Look-Up Tool from 2016 to 2024 and utilization data for Medicare and part B beneficiaries from 2016 to 2022, we analyzed reimbursement and utilization trends. Simple linear regressions were executed to measure the annual trends, and Wilcoxon matched-pairs signed rank test were used to analyze t

The Markup on Orthopaedic Services: An Analysis of 2014-2019 Medicare Data and the Potential for Surprise Billing.

BACKGROUND: Markups on charges for medical services have the potential to result in "surprise billing," especially for out-of-network and uninsured patients. Although previously analyzed in other surgical subspecialties, the distribution and level of cost-to-charge ratios (CCRs) for orthopaedic services have yet to be studied. Therefore, our analysis sought to evaluate the CCRs for orthopaedic surgery services provided to Medicare beneficiaries throughout the United States. METHODS: Orthopaedic services provided to Medicare Part B beneficiaries between 2014 and 2019 were identified in the Phy…

Spatiotemporal Medicare Reimbursement Trends and Economic Clusters in Otologic/Neurotologic Surgery.

OBJECTIVES: To characterize national trends in Medicare reimbursement for otologic/neurotologic procedures from 2012 to 2024, quantify the relative contributions of service volume, procedural mix, and within-code reimbursement to aggregate spending, and identify clusters of procedures with similar economic behavior. METHODS: Medicare Part B claims data from 2012 to 2024 were utilized and financial data were inflation-adjusted to 2024 USD. National trends in volume, spending, and reimbursement per service were evaluated. A Shapley decomposition framework quantified the contributions of service…

Subspecialization of Surgical Specialties in the US.

IMPORTANCE: Subspecialists-physicians with narrower clinical focus-play an increasing role in US health care, particularly in surgery, where most trainees now pursue fellowship training. Yet little is known about the degree of subspecialization among practicing surgeons and the evolving role of surgical generalists. OBJECTIVE: To quantify trends in surgical subspecialization and examine the geographic distribution of subspecialists and the procedural scope of surgical generalists. DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study using 100% Medicare Part B data from 2000, 2010,…

Continued growth in emergency department imaging is bucking the overall trends.

PURPOSE: The aim of this study was to examine recent trends in imaging utilization in emergency departments (EDs) in the Medicare population. METHODS: The 2002 to 2012 Medicare Part B databases were used. Imaging studies were categorized by modality. Medicare's place-of-service codes identified those studies performed in ED patients. Specialty codes identified the specialties of the interpreting physicians. Utilization rates per 1,000 Medicare beneficiaries were calculated. Trends were assessed in plain radiography (XR), CT, noncardiac ultrasound, MRI, and nuclear medicine. RESULTS: XR and C…

Long-term Trends in Open vs Endoscopic Carpal Tunnel Release Among the Medicare Population in the United States.

BACKGROUND: Carpal tunnel release (CTR) surgery is the most common surgery billed to Medicare by hand surgeons. As such, the purpose of this study was to evaluate trends for CTR surgeries billed to Medicare from 2000 to 2020. METHODS: The publicly available Medicare Part B National Summary File from 2000 to 2020 was queried. For both open carpal tunnel release (OCTR) and endoscopic carpal tunnel release (ECTR), the number of procedures and total Medicare reimbursement were extracted. For year 2020, the specialty of the performing surgeon was recorded. Descriptive statistics were reported. RE…

Trends in medicare reimbursement for the resection of spinal tumors: 2000-2021.

Despite recent fiscal expansion and rising healthcare expenditures, reimbursement rates for specific surgical procedures like spinal tumor resections remain underexplored. This study analyzes trends in real-value Medicare Part B reimbursements for spinal tumor resections over a 21-year period by querying the Medicare Part B database using relevant CPT codes from 2000 to 2021. Data included the aggregate number of procedures, allowed charges, and payments per procedure. Average payments per procedure and expected patient payments were calculated based on the 20% coinsurance of Medicare Part B. …
