Migraine as a Cortical Brain Disorder.

PURPOSE: Migraine is an exclusively human chronic disorder with ictal manifestations characterized by a multifaceted clinical complexity pointing to a cerebral cortical involvement. The present review is aimed to cover the clinical, neuroimaging, and neurophysiological literature on the role of the cerebral cortex in migraine pathophysiology. OVERVIEW: Converging clinical scenarios, advanced neuroimaging data, and experimental neurophysiological findings, indicate that fluctuating excitability, plasticity, and metabolism of cortical neurons represent the pathophysiological substrate of the mi

Bipolar affective disorder and migraine.

This paper consists of a case history and an overview of the relationship, aetiology, and treatment of comorbid bipolar disorder migraine patients. A MEDLINE literature search was used. Terms for the search were bipolar disorder bipolar depression, mania, migraine, mood stabilizer. Bipolar disorder and migraine cooccur at a relatively high rate. Bipolar II patients seem to have a higher risk of comorbid migraine than bipolar I patients have. The literature on the common roots of migraine and bipolar disorder, including both genetic and neuropathological approaches, is broadly discussed. Moreov

Migraine aura without headache compared to migraine with aura in patients with affective disorders.

The characteristics of psychiatric comorbidity in migraine have been studied in migraine with aura (MA) and migraine without aura (MO). Little information is available concerning patients with migraine aura without headache. In a study of 201 patients with major affective disorders (DSM-IV) we have described the clinical characteristics of patients with these three sub-types of migraine (IHS criteria) and compared the MA and migraine aura without headache groups. Compared to patients having MA (n=57), the group with migraine aura without headache (n=18) had a higher age of onset of migraine (2

Migraine associated with gastrointestinal disorders: A pathophysiological explanation.

BACKGROUND: Migraine is a highly prevalent, disabling, and costly disorder worldwide. From a long time ago, headaches have been known to be associated with gastrointestinal (GI) disorders. Headaches originating from gastric complaints were appreciated by Persian Medicine (PM) scholars. Today, functional GI disorders are shown to have high comorbidity with migraines; however, a causal relationship is not accepted today and pathophysiological explanations for this comorbidity are scarce. Therefore, based on the PM philosophy and the existing evidence, we aimed to propose an explanation for the c

Gastrointestinal disorders associated with migraine: A comprehensive review.

Migraine is a recurrent and commonly disabling primary headache disorder that affects over 17% of women and 5%-8% of men. Migraine susceptibility is multifactorial with genetic, hormonal and environmental factors all playing an important role. The physiopathology of migraine is complex and still not fully understood. Many different neuropeptides, neurotransmitters and brain pathways have been implicated. In connection with the myriad mechanisms and pathways implicated in migraine, a variety of multisystemic comorbidities (e.g., cardiovascular, psychiatric and other neurological conditions) hav

[Visual disorders as key symptoms of ocular migraine].

Vascular disturbance of the anterior visual pathways characterized by monocular visual experiences are sometimes caused by migraine. Retinal or ophthalmic migraine, a variant of complicated migraine, is distinctly rare. Two cases of female patients with ophthalmic migraine are reported. The relationships between the use of oral contraceptives and pregnancy are discussed.

[Genetic Disorders Associated with Migraine and Cerebrovascular Diseases].

Migraine is a chronic neurological disorder that is clinically characterized by recurrent headache attacks. Approximately 30% of migraine sufferers experience transient neurological symptoms, termed aura. Several monogenic migraine syndromes present with hemiparesis/hemiplegia as an aura. Familial hemiplegic migraine types 1,2, and 3 are registered in the OMIM database. Although the discovery of the genetic abnormalities underlying these syndromes has advanced our understanding of migraine pathogenesis, especially regarding neuronal mechanisms, genome-wide association studies focusing on migra

Migraine prevalence in eating disorders and pathophysiological correlations.

The eating disorders (ED): anorexia nervosa (AN) and Bulimia nervosa (BN) are severe psychiatric and somatic conditions occurring mainly in young woman. Although the etiology is largely unknown, same evidences suggest that biological and psychological factors play a relevant role in the pathogenesis, along with monoamine, indole and same hypothalamic hormonal dysfunctions. Migraine is characterized by similar metabolic and psychological anomalies suggesting that a possible relationship exists between the two pathological conditions. In order to understand the possible relationship between migr

Migraine-like headache attributed to a causative disorder: primary migraine or secondary headache with a migraine-like phenotype?

BACKGROUND: Migraine-like headaches can accompany or be the initial manifestation of many neurological disorders. When a migraine-like headache develops or worsens in the presence of another neurological disorder, there is often uncertainty about whether the headache is true migraine (i.e., the primary headache) or is a secondary headache with migraine-like symptoms. To answer this question, we analyzed headache characteristics and the effect of treatment in several neurological disorders. MATERIALS AND METHODS: The study was a retrospective analysis of five existing datasets collected by the

Migraine in childhood: biobehavioural or psychosomatic disorder?

It is well documented that headache is a multifactorial disorder which includes not only genetic, biological, medical and neuropsychological factor but also psychological and personality traits. The close relationship between stress and migraine attacks and the significant psychiatric comorbidities in migraine provide evidence of a "paradigm" of tight interaction between somatic and psychological aspects in paediatric migraine. In particular in younger children, an uncomfortable situation, a psychological problem or an emotional distress is rarely expressed directly but usually through physica

[The comorbidity of major depressive disorder, dysthymic disorder and anxiety disorders with migraine].

INTRODUCTION: Since migraine was first reported it has been associated with psychiatric disorders. This association has clinical repercussions and common genetic, environmental and psychological predisposing factors have been suggested. AIM: To determine the prevalence of anxiety disorders, major depressive disorder and dysthymic disorder as conditions that are comorbid to migraine in patients visiting due to headaches at the Pablo Tobon Uribe Hospital (Medellin, Colombia). PATIENTS AND METHODS: We conducted a cross-sectional analytical study that evaluated all patients aged between 18 and 6

Brain to Belly: Abdominal Variants of Migraine and Functional Abdominal Pain Disorders Associated With Migraine.

Migraine is one of the most frequent causes of primary headache and 9% of children suffer from migraines. Most children will continue to experience migraine attacks as adults, therefore it is imperative that we have a thorough understanding of this major health issue. This article considers the so-called abdominal variants of migraine, which are more commonly seen in children rather than adults: abdominal migraine, cyclic vomiting syndrome, and infantile colic. Other functional abdominal pain disorders such as irritable bowel syndrome and functional dyspepsia have also been linked to migraine

Migraine without aura and migraine with aura are distinct disorders. A population-based twin survey.

OBJECTIVE: To investigate the co-occurrence of migraine without aura (MWOA) and migraine with aura (MWA) in a population-based twin survey. BACKGROUND: Migraine without aura and MWA are multifactorial disorders. If MWOA and MWA share common genes, co-occurrence should be observed more frequently than expected, ie, the product of the prevalence in the general population. MATERIAL AND METHODS: The study population included all living Danish monozygotic (MZ) and same-gender dizygotic (DZ) twin pairs born between 1953 and 1960: 5360 twins (2026 MZ, 3334 DZ). The sample included 2840 men and 2520

Pathophysiology of Migraine: A Disorder of Sensory Processing.

Plaguing humans for more than two millennia, manifest on every continent studied, and with more than one billion patients having an attack in any year, migraine stands as the sixth most common cause of disability on the planet. The pathophysiology of migraine has emerged from a historical consideration of the "humors" through mid-20th century distraction of the now defunct Vascular Theory to a clear place as a neurological disorder. It could be said there are three questions: why, how, and when? Why: migraine is largely accepted to be an inherited tendency for the brain to lose control of its

Is migraine a disorder of the central nervous system?

The chicken and the egg problem is how, in Italian or in English, some time ago one would have referred to a question as the one to be approached here: "Is migraine a disorder of the central nervous system?" or of the peripheral nervous system? Till some time ago, even an honest basic scientist or clinician could almost equally sustain one or the other origin and find data that substantiate her/his opinion. Nowadays, however, our improved knowledge is pushing in the direction of the central nervous system, although many mechanisms remain unclear. The confusion originates from the important rol

Cross-prevalence of migraine and bipolar disorder.

OBJECTIVE: In two related studies, we explored the prevalence of migraine and its associated clinical characteristics in patients with bipolar disorder (BD) as well as psychiatric morbidity in patients treated for migraine. METHOD: The first study included 323 subjects with BD type I (BD I) or BD type II (BD II), diagnosed using the Schedule for Affective Disorders and Schizophrenia, Lifetime version (SADS-L) format, or the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID). Migraine history was assessed by means of a structured questionnaire. In a second sample of 102 migraine
