BACKGROUND: In the present study we evaluated the influence of topical miotics on intraocular pressure and the blood-aqueous barrier after uncomplicated phacoemulsification and PC-IOL implantation.
PATIENTS AND METHODS: Fifty-two eyes were randomized into 2 groups: with miotics (n = 28) and without miotics (n = 24). The IOP was measured before, 6 h, 1 and 2 days after surgery. Timolol maleate (o.25% or 0.5% twice a day) was used in 13 eyes of 13 patients with secondary angle-closure glaucoma post penetrating keratoplasty uncontrolled on miotics, carbonic anhydrase inhibitors, or both. When added to each patient's current regimen, tomolol controlled 9 of the 13 eyes that no longer required cyclocryotherapy, and it was well tolerated locally and systemically. Its ocular hypotensive effect appears to supplement miotics and carbonic anhydrase inhibitors.

One man and 2 women, aged 67, 63 and 68 years, observed for 8, 6, and 2 years, developed a classical postoperative malignant glaucoma. After a second operation, the use of atropine drops was continued for 2 years in one case, whereas in the other 2 cases miotics and later acetazolamide (Diamox) were necessary in order to maintain normal intraocular pressure. All 3 patients refused a preventive operation in the fellow eye and the use of miotics proved both necessary and effective in maintaining normal intraocular pressure, normal visual acuity, and normal visual fields.

The paper analyses results of long-term general and local medicamentous treatment of 86 patients with open-angle initial glaucoma.
