Detecting Dental Caries Using General-Purpose Large Multimodal Models From Oral Photographs.

OBJECTIVES: To determine whether a general-purpose large multimodal model (LMM) can detect dental caries from intraoral photographs without task-specific training, evaluating image-level classification and tooth-level localisation under zero-shot (no reference examples) and five-shot (five annotated examples) conditions. METHODS: This diagnostic accuracy study used the benchmark test split of 1255 publicly available intraoral photographs. Gemini 3.1 reasoning and instant were queried via Vertex AI API using stateless calls and structured prompts for sequential, tooth-by-tooth scanning. Each m

Prognostic Clinical Predictive Models for Dental Caries Using Artificial Intelligence: Methodological Considerations.

BACKGROUND: Artificial intelligence (AI) is currently used to develop clinical predictive models for dental caries. However, most prognostic models lack key methodological components. Few reach clinical practice, and fewer demonstrate clinical benefit. SUMMARY: This narrative methodological review examines considerations for developing, validating, and implementing AI-based caries prognostic models. Based on established prediction model frameworks (TRIPOD+AI, PROBAST+AI, and PROGRESS), we contextualize eight critical phases for caries-specific application. Each phase addresses challenges spec

Development and application of a prediction model for dental caries.

The development and validation of a caries prediction model comprising 13 sociodemographic and dental examination variables on Grade 1 and Grade 5 children in the National Preventive Dentistry Demonstration Program are described. The objective was to derive a method of predicting children at high risk to caries early in order that preventive measures might be undertaken. True high risk children were defined in two ways: highest 25% of children based on their 4-yr DMFS increment, and their total DMFS score at the end of the study. In both cases, children predicted to be at high risk were define

Dental abnormalities in a mouse model for craniometaphyseal dysplasia.

Mice carrying a knock-in mutation (Phe377del) in the Ank gene replicate many skeletal characteristics of human craniometaphyseal dysplasia, including hyperostotic mandibles. Ank (KI/KI) mice have normal morphology of erupted molars and incisors but excessive cementum deposition with increased numbers of Ibsp- and Dmp1-positive cells on root surfaces. The cervical loops of adult Ank (KI/KI) lower incisors are at the level of the third molars, while they are close to the mandibular foramen in Ank (+/+) mice. Furthermore, Ank (KI/KI) incisors show decreased eruption rates, decreased proliferation

A test of Berggren's model of dental fear and anxiety.

Berggren's (1984) model of dental fear and anxiety predicts that dentally anxious individuals postpone treatment, leading to a deteriorating dental state and subsequently to fear of negative evaluations in relation to their oral condition. The present study aimed to test one of the core assumptions of this model, namely that deterioration of dental health status would mediate the effects of avoidance of dental care on self-reported fear of negative evaluation. Participants were 73 patients (mean age 38.5 yr) meeting the diagnostic and statistical manual of mental disorders - 4th edn - Text Rev

Analgesic agents and strategies in the dental pain model.

Pain following removal of impacted third molar teeth has proven to be a useful clinical model for evaluating oral analgesics. Moreover, as the underlying pathophysiology becomes elucidated the model serves as a tool for monitoring the relative contributions of different pain events, including peripheral and central sensitization. Non-steroidal anti-inflammatory drugs (NSAIDs) demonstrate high potency in this model, reflecting the large contribution that peripheral prostaglandins may make to the pathophysiology of postoperative pain. However, other analgesic agents, with weak activity against p

Trueness comparison of intraoral scans for diverse arch lengths in pediatric dental models.

BACKGROUND/PURPOSE: Spacing between teeth is a common trait across different stages of dentition. With the tide of the digital impression, the scanning trueness of the intraoral scanner (IOS) is a hot subject. This study aimed to determine the correlation between the level of the spaced dentition and trueness of the intraoral scanning. MATERIALS AND METHODS: Four arch lengths of deciduous tooth models (spacing Model 1, Model 2, Model 3, Model 4 = 0, 1, 2, 3 mm; maxillary arch length = 73.268, 81.922, 90.776, 97.698 mm; mandibular arch length = 69.092, 76.160, 86.228, 94.344 mm) were designed

An Overview of the Models in Reporting School Data on Dental Credentialing Examinations.

The development and dissemination of meaningful and useful performance reports associated with examinations involved in the licensure process are important to the communities of interest, including state boards, candidates, and professional schools. Discussions of performance reporting have been largely neglected however. The authors recognize and reinforce the need for such discussions by providing prototypes of performance reporting in dentistry with examples and recommendations to guide practice. For illustrative purposes, this article reviews and discusses the different reporting models us

Dental injury models: experimental tools for understanding neuroinflammatory interactions and polymodal nociceptor functions.

Recent research has shown that peripheral mechanisms of pain are much more complex than previously thought, and they differ for acutely injured normal tissues compared with chronic inflammation or neuropathic (nerve injury) pain. The purpose of the present review is to describe uses of dental injury models as experimental tools for understanding the normal functions of polymodal nociceptive nerves in healthy tissues, their neuroinflammatory interactions, and their roles in healing. A brief review of normal dental innervation and its interactions with healthy pulp tissue will be presented first

Assessing multimodal large language models for localizing dental implant fixtures on panoramic radiographs.

OBJECTIVES: To assess whether general-purpose multimodal large language models (LLMs) can localize dental implant fixtures on panoramic radiographs and to quantify false positives on implant fixture-absent images. METHODS: Using an open-source dataset, we evaluated 82 implant fixture-present panoramic radiographs (297 fixtures) and 82 implant fixture-absent images balanced by present or absent radiopaque restorations (41 each). We tested three multimodal LLMs (GPT-4o, OpenAI o3, and GPT-5T) with a fixed visual-grounding prompt across five independent runs per image. We scored the outputs usin

Development of Multiple Risk Prediction Models for Postoperative Symptoms Following Dental Implant Surgery.

BACKGROUND: Dental implants have become a widely accepted solution for replacing missing teeth. Despite their high success rates, postoperative discomfort such as pain, swelling and foreign body sensation is commonly experienced by patients. However, the prediction of such outcomes based on patient characteristics remains underexplored. OBJECTIVE: Develop and validate risk models for patient-reported postoperative symptoms after dental implant surgery. METHODS: A total of 506 patients who underwent dental implant surgery between January 2024 and December 2024 were included. Postoperative sym

Periodic detection and disinfection maintenance of dental unit waterlines in dental simulation head model laboratories.

Dental simulation head model laboratories are crucial for clinical simulation training for stomatological students, yet the maintenance of their dental unit waterlines (DUWLs) has been overlooked. This study investigated water contamination in DUWLs within these laboratories and proposed solutions. Water samples were collected from 12 dental chairs in three laboratories at three time points: the beginning, middle, and end of the semester. At the start of the semester, severe contamination was observed, with colony counts of 11,586[Formula: see text]1715 CFU/ml for high-speed handpieces and 537
