Mohs Micrographic Surgery for Advanced Centrofacial Tumors.

BACKGROUND: Mohs surgery was developed for the treatment of advanced skin cancers. Advanced centrofacial tumors are among the most challenging lesions. OBJECTIVE: The objective of the study was to review the most complex midface cases from our practice and to delineate how to plan the approach to these lesions, how to remove them in a step-by-step fashion, and how the patients were managed in a multidisciplinary manner when indicated. METHODS: We reviewed 15 years of the most complex tumors to present to our practice for which Mohs micrographic surgery was performed. Follow-up for patients r…

Mohs Micrographic Surgery: Past, Present, and Future.

BACKGROUND: Mohs micrographic surgery (MMS) is a frequently used technique that provides total margin visualization for treatment of skin neoplasms. OBJECTIVE: To provide a comprehensive review of MMS literature, focusing on its origins, evidence behind present-day uses of MMS, and future directions. METHODS: A literature search was conducted using PubMed to identify articles pertaining to MMS. RESULTS: The fresh frozen technique led to widespread use of MMS in the 1970s. One randomized controlled trial and several large prospective studies have demonstrated low recurrence rates for treatme

East-West Flap After Mohs Micrographic Surgery.

Repair of distal nasal defects resulting from Mohs micrographic surgery is a challenge, but surgeons can choose from a number of techniques. We present 3 cases in which an east-west flap was used to close the defect. The east-west flap is an easily conceived advancement flap that consists of a superior triangle used to cover the defect and an inferior triangle whose base is located along the advancement line. The flap results in an optimal cosmetic outcome and involves minimum movement of tissue (preserving the nasal architecture) and well-camouflaged suture lines.

Sebaceous Carcinoma Treated With Mohs Micrographic Surgery.

BACKGROUND: Sebaceous carcinoma is a rare and potentially aggressive adnexal neoplasm with historic data indicating high rates of recurrence, metastasis, and cancer-specific mortality. OBJECTIVE: To evaluate the incidence of local recurrence, metastasis, disease-specific mortality, and all-cause mortality and to identify work-up approaches. PATIENTS AND METHODS/MATERIALS: Retrospective review of patients with sebaceous carcinoma treated with Mohs micrographic surgery between 2001 and 2013 at one institution. RESULTS: Thirty-seven patients had 45 sebaceous carcinomas located on the periocula

Superficial leiomyosarcoma treated with Mohs micrographic surgery.

In this retrospective study we employed real-time polymerase chain reaction (PCR) to analyse the occurrence of Mycoplasma pneumoniae among upper and lower respiratory tract infections (RTI) in the Central Region of Slovenia between January 2006 and December 2014. We also used a culture and pyrosequencing approach to genotype strains and infer their potential macrolide resistance. Of a total 9,431 tested samples from in- and out-patient with RTI, 1,255 (13%) were found to be positive by M. pneumoniae PCR. The proportion of positive samples was 19% (947/5,092)among children (≤16 years-old) and 7

Combined Mohs Micrographic Surgery in a Collision Tumor.

We present the case of a 73-year-old male patient with a collision tumor in the right ear, consisting of a basal cell carcinoma and melanoma in situ. He was managed with Mohs micrographic surgery combining paraffin-embedded sections and frozen sections. Multiple surgical stages were required to obtain tumor-free margins. The surgical defect was reconstructed by plastic surgery, achieving the preservation of the ear. The technique of combining two processing sections is useful in the treatment of the collision of basal cell carcinoma with malignant melanoma.

Koebner phenomenon: pemphigus vulgaris following Mohs micrographic surgery.

Koebnerization of pemphigus vulgaris (PV) is an infrequently reported reaction. We present a 65-year-old man with Koebnerized pemphigus vulgaris after Mohs microscopic surgery for a basal cell carcinoma. We present this case to heighten awareness of the phenomenon in the dermatological community.

Assessment of postoperative pain after Mohs micrographic surgery.

BACKGROUND: Mohs micrographic surgery (MMS) has the highest cure rate for the treatment of cutaneous malignancies and is usually performed in an outpatient setting with local anesthesia. Although most patients experience minimal discomfort during the procedure, postoperative pain after MMS has not been well-characterized. The objective of this study was to evaluate the amount of postoperative pain after MMS and to determine whether the degree of pain is correlated with factors such as tumor location, size, number of excisions, or age or sex of the patient. MATERIAL AND METHODS: One hundred fi

Mohs micrographic surgery for squamoid eccrine ductal carcinoma.

BACKGROUND: Squamoid eccrine ductal carcinoma is a rare cutaneous malignancy. There are no accepted standards for surgical margins in eccrine carcinomas. OBJECTIVE: We report a case of squamoid eccrine ductal carcinoma resembling squamous cell carcinoma and discuss Mohs micrographic surgery as a surgical modality for eccrine carcinomas. METHOD: The patient was a 30-year-old Korean woman with a 4-year history of a nodule on her neck. Following primary diagnosis of squamous cell carcinoma by punch biopsy, the tumor was completely removed by Mohs micrographic surgery with a 2 mm cancer-free mar

Syringocystadenocarcinoma papilliferum: successfully treated with Mohs micrographic surgery.

BACKGROUND: Syringocystadenocarcinoma papilliferum, a rare sweat gland carcinoma, is the malignant counterpart of syringocystadenoma papilliferum. OBJECTIVE: To demonstrate a rare case of syringocystadenocarcinoma papilliferum successfully treated with Mohs micrographic surgery. METHODS: A 60-year-old male presented with two verrucous plaques on his right auricle since childhood. These two plaques became ulcerated, more painful, and pruritic in 1 year. Histopathologic examination revealed syringocystadenocarcinoma papilliferum. RESULTS: Mohs micrographic surgery with reconstruction of right

Mohs micrographic surgery for fungal soft tissue infections.

BACKGROUND: Invasive fungal infections of the integument are relatively rare. In the immunocompromised patient, however, they may show an extremely aggressive biological behavior despite high dosed topical or systemic antifungal therapy. As the fungal tissue invasion usually reaches well beyond the area of clinical necrosis or other visible changes, standard surgical excision often proves to be inadequate, resulting in the need for repeated relatively wide excisions with the resulting substantial loss of initially healthy tissues. OBJECTIVE: To present the use of Mohs surgery as a safe and ef…
