Metabolic Myoglobinuria.

One large group of hereditary myopathies characterized by recurrent myoglobinuria, almost invariably triggered by exercise, comprises metabolic disorders of two main fuels, glycogen and long-chain fatty acids, or mitochondrial diseases of the respiratory chain. Differential diagnosis is required to distinguish the three conditions, although all cause a crisis of muscle energy. Muscle biopsy may be useful when performed well after the episode of rhabdomyolysis. Molecular genetics is increasingly the diagnostic test of choice to discover the underlying genetic basis.

[Rhabdomyolysis and myoglobinuria].

Rhabdomyolysis is a disorder characterized by acute damage of the sarcolemma of the skeletal muscle leading to release of potentially toxic muscle cell components into the circulation, most notably creatine phosphokinase (CK) and myoglobin, and is frequently accompanied by myoglobinuria. Therefore, the term myoglobinuria is often used interchangeably with the term rhabdomyolysis. This disorder may result in potential life-threatening complications such as acute myoglobinuric renal failure, hyperkalemia and cardiac arrest, disseminated intravascular coagulation, and compartment syndrome. The co…

[Myoglobinuria and the kidney].

The relationship between myoglobinuria and acute renal failure is analyzed patho-histologically in five autopsy cases of rhabdomyolysis and acute renal failure. The presence of myoglobin urinary casts was not a common finding, but various tubular damages due to factors other than myoglobin deposition were observed. These findings suggest that in addition to the toxic effect of myoglobin or its derivatives, preexistent or simultaneous renal or extrarenal tissue damages participate in the development of acute renal failure.
