There may be similarities in the anatomicophysiological mechanism of pain production and in the clinical picture in many of these conditions. Many of the vague and refractory cases of neck and shoulder pain and of migraine may be due to cervical disc disease. Scalenus anticus syndrome and cardiac disease can be diagnosed or differentiated from cervical disc syndrome only by thorough investigation. We describe a case of internal carotid artery dissection, an important cause of ischemic cerebrovascular disease. Presentations include a headache, which is usually unilateral to the side of carotid dissection, sharp, nonthrobbing in nature with radiation from the neck. The clinical presentations, investigations, and treatment of the disease are discussed.

A 45-year-old motorcyclist was hit by a car and presented to the emergency department with minor neck pain and radiating pain into both arms. Routine radiography did not show any pathology. Additional CT and MRI images revealed an anterolisthesis of Cvii-Thi with a bilateral facet dislocation and myelum compression.

We report a patient who had headache and neck pain after whiplash injury and subsequently developed cerebellar infarction due to vertebral artery dissection. This patient's pain was out of proportion to his apparent injury and it was a clue to the final diagnosis. Gross motor examination for cord injury may not be adequate for patients with minor neck trauma. Detailed cranial nerve and cerebellar examination should be performed for detection of circulatory insufficiency. Discharge advice for patients should also include that of stroke or transient ischaemic attack.
