Characterization of myeloid neoplasms following allogeneic hematopoietic cell transplantation.

We compared characteristics of myeloid neoplasms (MNs) following allogeneic hematopoietic cell transplantation (HCT) versus autologous HCT using a Japanese HCT registry database. Among 43 788 patients who underwent allogeneic (n = 18 874) or autologous HCT (n = 24 914) for non-myeloid malignancies or non-malignant diseases, 352 developed MNs. The cumulative incidence of MNs was lower after allogeneic HCT than after autologous HCT (0.3% vs. 1.8% at 10 years, respectively, p < .001). Compared with autologous HCT, MNs following allogeneic HCT developed in younger patients (median, 42 vs. 57 years

[Multiple cutaneous neoplasms in cyclosporine therapy after kidney transplantation].

We report on a 56-year-old renal allograft recipient receiving cyclosporin A immunosuppression. During this therapy he subsequently developed the following cutaneous neoplasms: squamous cell carcinomas, basal cell carcinomas, Bowen's disease, actinic keratosis, sebaceous hyperplasia, a dysplastic naevus and, finally a nodular malignant melanoma. Adverse effects of the cyclosporin A therapy are discussed, with special reference to dermatologic effects and the implications for patient and doctor.

Malignant Neoplasms of the Gastrointestinal Tract After Blood or Marrow Transplant.

IMPORTANCE: Survivors of blood or marrow transplant (BMT) are at increased risk of subsequent malignant neoplasms (SMNs). Cancers of the gastrointestinal (GI) system are of special interest because their clinical behavior is often aggressive, necessitating early detection by increasing awareness of high-risk populations. OBJECTIVE: To describe the risk of SMNs in the GI tract after BMT. DESIGN, SETTING, AND PARTICIPANTS: A cohort study of 6710 individuals who lived at least 2 years after BMT performed between January 1, 1974, and December 31, 2014, at City of Hope, University of Minnesota, o

How I Manage Transplant Ineligible Patients with Myelodysplastic Neoplasms.

Myelodysplastic neoplasms, formerly known as myelodysplastic syndromes (MDS), represent a group of clonal disorders characterized by a high degree of clinical and molecular heterogeneity, and an invariable tendency to progress to acute myeloid leukemia. MDS typically present in the elderly with cytopenias of different degrees and bone marrow dysplasia, the hallmarks of the disease. Allogeneic hematopoietic stem cell transplant is the sole curative approach to date. Nonetheless, given the disease's demographics, only a minority of patients can benefit from this procedure. Currently used prognos

Prevalence and outcomes of pancreatic cystic neoplasms in liver transplant recipients.

AIM: To determine the prevalence, characteristics and clinical course of pancreatic cystic neoplasms (PCNs) in liver transplantation (LT) recipients. METHODS: We retrospectively studied consecutive patients who underwent LT between January 1998 to April 2016. Clinical and laboratory data were obtained from patient medical records. Imaging findings on computed tomography and magnetic resonance cholangiopancreatography were reviewed by two radiologists. RESULTS: During the study period, 872 patients underwent cadaveric LT. Pancreatic cysts were identified in 53/872 (6.1%) and 31/53 (58.5%) wer

Plasma cell neoplasms in US solid organ transplant recipients.

Transplant recipients have elevated risk for plasma cell neoplasms (PCNs, comprising multiple myeloma and plasmacytoma), but little is known about risk factors in the transplant setting. Through linkage of the US solid organ transplant registry with 15 state/regional cancer registries, we identified 140 PCNs in 202 600 recipients (1987-2009). PCN risk was 1.8-fold increased relative to the general population (standardized incidence ratio [SIR] 1.80, 95%CI 1.51-2.12). Among cases, 102 were multiple myeloma (SIR 1.41) and 38 were plasmacytoma (SIR 7.06). PCN incidence increased with age, but due

[Renal neoplasms and renal transplantation: current problems and future perspectives].

Primary carcinomas of the kidney can develop in renal transplantation in four sets of circumstances: (1) detected in the donor, (2) detected as a pre-existing neoplasm in the recipient prior to transplantation, (3) as de novo malignancies arising post-transplantation in the native kidneys of the recipient, (4) or in the graft. In Italy, any renal mass detected during harvesting does not allow the use of any organs for transplantation; however, several reports from other countries have already shown the safety and efficacy of transplanting kidneys with small (<4 cm), unifocal, subcapsular tumor

Retrospective analysis of hematopoietic cell transplantation for blastic plasmacytoid dendritic cell neoplasm: conditioning intensity matters.

Blastic plasmacytoid dendritic cell neoplasia (BPDCN) is a rare myeloid malignancy with a generally poor prognosis. Although preliminary evidence suggests that hematopoietic cell transplantation (HCT) could improve outcome in patients with BPDCN, the individual contributions of conditioning and graft-versus-tumor (GVT) effects to HCT success are undefined. We present a retrospective study of 162 adult patients who underwent a first HCT (allogeneic 146, autologous 16) between 2009 and 2017, and were registered with the EBMT. Median age was 57 (range 20-73) years, and disease status at HCT was f

[Thiotepa-busulfan-fludarabine conditioning in allogeneic hematopoietic stem cell transplantation for blast-phase myeloproliferative neoplasm].

INTRODUCTION: Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is the only curative option for blast-phase myeloproliferative neoplasms (BP-MPN), although relapse rates remain high. Recently, conditioning with thiotepa, busulfan, and fludarabine (TBF) has shown promising anti-leukemic activity in acute myeloid leukemia. CASE REPORT: A 64-year-old man with essential thrombocythemia that progressed to secondary myelofibrosis and subsequently BP-MPN underwent allo-HSCT after cytoreduction with azacitidine plus venetoclax. Peripheral blood stem cells were obtained from an HLA allele

Benefits of Preemptive Living Kidney Transplantation: Reduced Rates of Cerebro-Cardiovascular Events and Neoplasm Development.

OBJECTIVES: Reports on the benefits of preemptive kidney transplantation (PEKT) have been conflicting. We assessed postoperative unfavorable outcomes to determine the potential advantages of PEKT. METHODS: We conducted a retrospective multicenter study including 567 living donor kidney transplantation recipients registered in the Michinoku Renal Transplant Network database between June 1998 and June 2021. The recipients were classified into two groups: PEKT and non-PEKT. We compared overall survival, graft survival, rejection-free survival, cerebro-cardiovascular disease (CCVD)-free survival,

Successful treatment of a metastatic hepatocellular malignant neoplasm, not otherwise specified with chemotherapy and liver transplantation.

Hepatocellular malignant neoplasm, not otherwise specified (HCN-NOS) is a provisional entity describing a subset of rare malignant pediatric liver tumors with overlapping features of hepatoblastoma and hepatocellular carcinoma. We present a case illustration of metastatic HCN-NOS successfully treated with a backbone of hepatoblastoma chemotherapy, pulmonary metastastectomy, and liver transplantation, along with a literature review of the clinical outcomes of HCN.

Blood or marrow transplantation and cellular therapies in myeloproliferative neoplasms: A comprehensive review.

Philadelphia-negative myeloproliferative neoplasms (Ph-negative MPNs) are clonal malignancies, with a complex interaction of the malignant clone, extramedullary haematopoiesis, and chronic inflammation. In high-risk disease, transformation to blast phase remains a constant risk and outcomes are usually dismal. Allogeneic blood or marrow transplantation (BMT) remains the only potentially curative treatment to date. In this narrative review, we summarize the evidence available to date on the role of BMT in MPNs and the ongoing investigations exploring the use of mutant CALR-targeted immune-based

Progression of Intraductal Papillary Mucinous Neoplasms of the Pancreas After Liver Transplantation.

BACKGROUND: Intraductal papillary mucinous neoplasms (IPMNs) are the most commonly identified pancreatic cystic neoplasms. Incidentally detected IPMNs are common among liver transplant recipients. The risk of IPMN progression to pancreatic cancer in transplant recipients and the impact of immunosuppression on the risk of malignant transformation of IPMN are unclear. METHODS: In this retrospective study of consecutive liver transplant recipients across Mayo Clinic over a 13-year period, patients were assessed for possible IPMN by automated chart review. Pancreatic cystic lesions were character

Cord blood transplantation for adult mature lymphoid neoplasms in Europe and Japan.

To clarify the different characteristics and prognostic factors of cord blood transplantation (CBT) in adult patients with lymphoid neoplasms in Europe and Japan, we conducted a collaborative study. Patients aged 18-75 years receiving their first CBT (Europe: single CBT, n = 192; double CBT, n = 304; Japan: single CBT, n = 1150) in 2000-2017 were analyzed. Fewer patients with Hodgkin lymphoma (Europe vs Japan, 26% vs 5%), and older patients (≥50 years) (39% vs 59%) with a higher refined disease risk index (rDRI) (high-very high: 49% vs 14%) were included in the Japanese registry. Postrenal Transplant Metastatic Colonic Neoplasm: Posttransplant Lymphoproliferative Disorder or Adenocarcinoma?

Transplant recipients are vulnerable to a horde of infections and neoplastic conditions due to immunosuppression. Posttransplant lymphoproliferative disorder (PTLD) is a condition unique to the transplant recipient occurring due to monoclonal lymphocytic proliferation. It may affect any organ system with reportedly highest incidence in the gastrointestinal tract. The incidence of adenocarcinoma of the colon, however, has not been shown to be uniformly higher in transplant recipients. We report here an unusual case of adenocarcinoma of the ascending colon presenting with liver, lymph node and s

[Risk factors and development of skin neoplasms in patients after kidney transplantation].

Kidney transplant recipients, comparing to general population, have an increase risk of cancer development. The most frequent lesions, observed in this population, are skin cancers, especially NMSCs (nonmelanoma skin cancers) which make up to 50% of all malignant lesions. The incidence of neoplasm is influenced by demographic characteristics and the highest prevalence achieve in Australia where 52.5% of kidney recipients develop skin cancer after 10 years after transplantation and 82.1% after 20 years. Authors in the following article describe the most common cutaneous premalignant and maligna

Unrelated donor transplantation with posttransplant cyclophosphamide vs ATG for myelodysplastic neoplasms.

It has been reported in prospective randomized trials that antithymocyte globulin (ATG)-based graft-versus-host disease (GVHD) prophylaxis has benefits in the setting of allogeneic hematopoietic stem cell transplantation (allo-HSCT) with unrelated donors (UDs). However, the optimal GVHD prophylaxis strategy has been challenged recently by the increasing use of posttransplant cyclophosphamide (PTCY). We report from the European Society for Blood and Marrow Transplantation registry the outcomes of 960 patients with myelodysplastic neoplasms who underwent allo-HSCT from UD with PTCY or ATG as GVH
