Origins of the sensory examination in neurology.

Formal testing of sensation as part of the neurological examination followed the improvements in examination techniques as well as advances in neuroscience. By the 1890s, the observation that temperature sense was frequently impaired at the same time that pain was appreciated led to the supposition that the two paths traveled closely. Through the works of Brown-Séquard and Edinger the existence of a crossed afferent tract was verified. The distinction between two sensory pathways was clear by 1898, when van Gehuchten reported a case of syringomyelia and suggested that the pain and temperature

Evaluation of neurological examination of sheep.

One hundred clinically negative Merino sheep were examined neurologically in an attempt to establish normal responses. Dependable results were obtained regarding eye muscle integrity, corneal sensitivity, the optic papilla and righting reflexes and to some extent the knee jerk reflex. The elicitation of other reflexes was inconsistent and unreliable except the placing reflex as shown by raising and lowering the forequarters.

Emergent headaches during pregnancy: correlation between neurologic examination and neuroimaging.

BACKGROUND AND PURPOSE: Emergent evaluation of the pregnant headache patient requires rational selection of acute neuroimaging studies, yet … with fever (11%), meningismus (9%), or seizures (7%). A total of 43% of case subjects had abnormal neurologic examination findings. Emergent neuroimaging, including noncontrast head CT and MR imaging, revealed an underlying headache etiology in 27%, including cerebral venous thrombosis, reversible posterior leukoencephalopathy, pseudotumor, and intracranial hemorrhage. …

[Thermographic examination of patients with neurologic manifestations of lumbar osteochondrosis].

The article deals with thermography data collected for 94 patients with different clinical signs of lumbar osteochondrosis (reflex, radicular, vascular, radiculo-spinal). The features of thermograms were described as depending on the leading clinical syndrome of the disease. A lack of direct correlation between the severity of the pain syndrome and thermoasymmetry was registered. The data suggest wider use of the diagnostic capacities of IR-imaging techniques in patients with vertebrogenic pathology.

Neurologic examination of healthy adult African pygmy hedgehogs (Atelerix albiventris).

OBJECTIVE: To describe a modified approach to neurologic examination of African pygmy hedgehogs (Atelerix albiventris). ANIMALS: 12 adult hedgehogs (7 males and 5 females). PROCEDURES: Aspects of the standard neurologic examination of dogs and cats were evaluated for use with awake hedgehogs, and modified approaches to evaluating their normal behavior and mentation, select cranial nerves and refexes, and gait were then identified. Behavioral analysis and gait analysis were performed by using video recordings of hedgehogs in a novel environment. Performability and repeatability of all feasibl…

Neurologic Examination Findings Associated With Small Cerebellar Volumes After Prematurity.

To help clinicians understand what to expect from small cerebellar volumes after prematurity, this study aims to characterize the specific impacts of small cerebellar volumes on the infant neurologic examination. A prospective cohort of preterm newborns (<32 weeks' gestational age) had brain magnetic resonance imaging (MRI) studies at term-equivalent age. Cerebellar volumes were compared with neurologic examination findings in follow-up, adjusting for severity of intraventricular hemorrhage, white matter injury, and cerebellar hemorrhage. Deformation-based analyses delineated regional morphome…

Rater agreement on gait assessment during neurologic examination of horses.

BACKGROUND: Reproducible and accurate recognition of presence and severity of ataxia in horses with neurologic disease is important when establishing a diagnosis, assessing response to treatment, and making recommendations that might influence rider safety or a decision for euthanasia. OBJECTIVES: To determine the reproducibility and validity of the gait assessment component in the neurologic examination of horses. ANIMALS: Twenty-five horses referred to the Royal Veterinary College Equine Referral Hospital for neurological assessment (n = 15), purchased (without a history of gait abnormalit…

The Necessity of Upper Extremity Neurologic Examination while Evaluating Breast Pain.

INTRODUCTION: Breast pain is one of the most frequent complaints and occurs in 45-69% of all women. Cervical disc herniation is a common cause of cervical radiculopathy with an incidence rate of 18.6 per 100,000, and it should be considered as a cause of breast pain. OBJECTIVE: To identify the extent to which upper extremity neurologic findings and cervical root disorders accompany breast pain. METHODS: We prospectively collected clinical features of patients with breast pain. An upper extremity neurologic examination was performed in patients and controls. Patients who had neurologic findin…

Can neurologic examination predict pathophysiology of ulnar neuropathy at the elbow?

OBJECTIVE: To explore the utility of neurologic examination to predict the pathophysiology of ulnar nerve lesions in patients with ulnar neuropathies at the elbow (UNE). METHODS: We prospectively recruited consecutive patients with suspected UNE. Four blinded investigators took a history and performed neurologic, electrodiagnostic (EDx) and ultrasonographic (US) examinations. In patients with axonal UNE, conduction block and conduction slowing, the pathophysiologies of UNE and neurologic examination findings were compared. RESULTS: We found significant differences in muscle bulk and strength…

Predictive value of an early amplitude integrated electroencephalogram and neurologic examination.

OBJECTIVE: To examine the predictive validity of the amplitude integrated electroencephalogram (aEEG) and stage of encephalopathy among infants with hypoxic-ischemic encephalopathy (HIE) eligible for therapeutic whole-body hypothermia. DESIGN: Neonates were eligible for this prospective study if moderate or severe HIE occurred at <6 hours and an aEEG was obtained at <9 hours of age. The primary outcome was death or moderate/severe disability at 18 months. RESULTS: There were 108 infants (71 with moderate HIE and 37 with severe HIE) enrolled in the study. aEEG findings were categorized as nor

The avian neurologic examination and ancillary neurodiagnostic techniques: a review update.

The purpose of this article is to guide the avian clinician in the assessment of neurologic function in birds. Physical and neurologic examinations that evaluate cranial nerves, postural reactions, and spinal reflexes identify neurologic dysfunction and the corresponding anatomic location of the lesion. Ancillary diagnostic tests, such as cerebrospinal fluid analysis, diagnostic imaging, muscle and nerve histology, and electrodiagnostics, are tools to confirm and clarify conclusions from the neurologic examination and to identify the cause of disease. Once the disease location and pathologic p…

Should the Babinski sign be part of the routine neurologic examination?

BACKGROUND: The Babinski sign is a well-known sign of upper motor neuron dysfunction that is widely considered an essential element of a complete neurologic examination. Little is known about reliability and validity of this sign. A less well-known sign of upper motor neuron dysfunction, decreased speed of foot tapping, also has not been carefully evaluated. Scientific evaluation of findings of the physical examination is crucial in directing busy clinicians. METHODS: Ten physicians (five neurologists and five non-specialists) examined each foot of 10 subjects, 8 of whom had known unilateral …
