Computer-assisted interactive three-dimensional planning for neurosurgical procedures.

We have used three-dimensional reconstruction magnetic resonance imaging techniques to understand the anatomic complexity of operative brain lesions and to improve preoperative surgical planning. We report our experience with 14 cases, including intra- and extra-axial tumors and a vascular malformation. In each case, preoperative planning was performed using magnetic resonance imaging-based three-dimensional renderings of surgically critical structures, such as eloquent cortices, gray matter nuclei, white matter tracts, and blood vessels. Simulations, using the interactive manipulation of thre

A Histopathological Study of Hemostatic Agents Retrieved During Neurosurgical Procedures.

BACKGROUND AND OBJECTIVES: Current knowledge on tissue reactions and resorption of hemostatic agents in neurosurgery is mostly derived from animal and non-neurosurgical studies. The limited available data in humans consist of small case series including only a few types of hemostatic agents. This study outlines the histological findings, including inflammatory response, and the degree of resorption observed in hemostatic agents retrieved during cranial and spinal reoperations. METHODS: Histological slides of hemostatic agents and surgical materials retrieved during secondary cranial and spina

Incidence of emergency neurosurgical TBI procedures: a population-based study.

BACKGROUND: The rates of emergency neurosurgery in traumatic brain injury (TBI) patients vary between populations and trauma centers. In planning acute TBI treatment, knowledge about rates and incidence of emergency neurosurgery at the population level is of importance for organization and planning of specialized health care services. This study aimed to present incidence rates and patient characteristics for the most common TBI-related emergency neurosurgical procedures. METHODS: Oslo University Hospital is the only trauma center with neurosurgical services in Southeast Norway, which has a p…

Continuous autonomic system monitoring during neurosurgical procedures -proof of concept.

Vegetative reactions are common during neurosurgical procedures. Known effects are mainly cardiovascular, including tachy- and bradyarrhythmia, hyper- and hypotonia as well as cardiac arrest. Computer-assisted real-time analysis of heart rate variability (HRV), baroreflex-sensitivity (BRS) allows for continuous evaluation of the autonomic nervous system (ANS). We analyzed ANS parameters during intracranial neurosurgical procedures. In this pilot study, we aim to provide proof-of-concept that ANS monitoring during surgery is feasible and yields stable results. We included 129 consecutive patient…

Same-day discharge after pediatric neurosurgical procedures: a single-institution experience.

BACKGROUND AND OBJECTIVES: Same-day discharge (SDD) after neurosurgical procedures in children remains underexplored. To our knowledge, this represents one of the first modern North American single-institution series to include SDD after major pediatric spinal instrumentation and procedures performed on children of elevated anesthetic risk. METHODS: A retrospective review of all neurosurgical procedures performed at an academic, tertiary referral center was conducted. Patients were identified as those who underwent surgery and were discharged on the same calendar day (length of stay [LOS] = 0…

Colonic perforation following neurosurgical procedures and corticosteroid therapy in four dogs.

Perforation of the proximal descending colon occurred in four Dachshund dogs, each of which died 5 to 10 days following neurosurgical procedures. The complication was believed to be associated with parenteral use of dexamethasone. Correct antemortem diagnosis was made in only one of the four cases, probably because signs of a perforated viscus were so modified by the antiphlogistic effects of corticosteroids as to prevent early detection of the complication.

The Pericranial Flap as a Salvage Procedure in Pediatric Neurosurgical Complications.

The pericranium is a vascularized structure that can be used for the treatment of complex scalp and skull defects. It is also utilized as a prophylactic measure to prevent wound complications in high-risk patients. In this study, we looked at the use of the pericranial flap in pediatric patients. A retrospective chart review was done in the American University of Beirut Medical Center from February 2010 to 2020. Ten pediatric patients were identified who required either prophylactic pericranial flap (n  =  3) or for treatment of a chronic draining sinus (n =  7). Patients were followed up for
